2 How to use this performance report This performance report is intended to be both informative and easy to use. The guide below is designed to help you get the most out of the report and to answer the most common questions you might have. 3 Council Plan actions and measures reshape the balance of care for older people enabling them to live in their homes and community for as long as possible support carers to continue in their caring role Continue to implement Supporting Your Independence approach across adult and older people service Develop an intermediate care approach in both community and residential setting Quarterly progress on the impact of the implementation of the Carers Strategy Monitor and report on the outcomes after completion of SYI Integrated care arrangements are being developed in a test site in East Kilbride (Whitehill Care Home). These arrangements will support 24 hour care for a period of 6 weeks. Further work is being undertaken with regards to this as part of the wider programme of improvement activity to support the management of hospital admissions and delayed discharges. Carers Strategy Meetings are well established with partners providing update reports in advance of meetings. Monitoring and Performance meetings are held with our main Carers Organisations, PRT (which has now rebranded to be known as Lanarkshire Carers Centre LCC), and SL Carers Network. Both organisations have new staff and moved premises. An internal Carers issues group is being established in order to collate all carer activity and consider the implications of the Carers Bill. The Resource also has representation at a National level on the Implementation and Monitoring Group (IMG) of the National Carers Strategy. The Scottish Government have recently released "Scotland's Carers" an official Statistical Publication for Scotland. This has been circulated to our partners. A total of 408 episodes of Supporting Your Independence (SYI) were successfully completed in the period January to March In total a reduction of 30% in home care hours was achieved when comparing hours at the start of SYI and hours at the end of the intervention. The overall figure for the year April 2014 to March 2015 is 25%. 2

Continue to implement Supporting Your Independence approach across adult and older people services Maximise the use of our older people's day centres reshape the balance of care for older people enabling them to live in their homes and community for as long as possible Percentage of referrals for homecare go through SYI Monitor the percentage of vacancies in our Older People's Day Centres by Locality Following on from the success of ICST in East Kilbride, further roll out of ICST to other localities In Quarter 4 (January to March) a total of 80.5% of new referrals to homecare were suitable for a Supporting Your Independence (SYI) intervention. This is an increase of 3.8% on the previous quarter figure of 76.7%. However, the year to date position reports 82.0% which represents a 3.4% decrease over the full year. Currently we have 1606, day care places available across 16 units every week. At the end of Quarter 4 there were 1264 (79%) of places occupied, which resulted in 342 (21%) being vacant. As we are in the process of remodelling day care services with a view to ceasing evening services and reconfiguring weekend services, some vacancies have been held to allow for these changes to be implemented. We anticipate that these changes will reduce the percentage of vacancies. The successfully evaluated Integrated Community Support Team approach has been extended to all of the South Lanarkshire localities. This model will continue to be developed as a key delivery priority for health and social care integration Red 90.0% 82.0% % 86.4% Green % 0.0% 0.0% 0.0% Continue to work with Upgrade of existing amenity properties Phase 1 complete health and social care partnership to reshape services for older people Mainstream properties converted to amenity Phase 2: 20 properties upgraded (Q4). 151 properties upgraded since programme commenced. As part of this process, service users were offered 4 funding choices. However, the majority of people chose option 3 (the local authority arranges the support). In addition to the above and in quarter 4, 207 people chose option 1 (direct payment) and 1 service user chose option 2 (the person can direct their own support). This remains consistent with the previous quarters where we reported 209 in Q3, 212 in Q2 and 218 in Q1. Actions highlighted in annual review 2013/14 continue to be progressed appropriately. The 2014/15 annual review will be reported to RMT Summer This is an Annual Report and all MAPPA reports across Scotland are published simultaneously, in co-ordination with the National Report. This was formally published in October, and as a result the measure was reported in quarter 3 for the previous year. Green Green
