1 PERSONAL LEARNING DEVICE USE ACSC This agreement is made effective upon the receipt of a PLD, between Attica Consolidated School Corporation ( ACSC ), the student receiving a PLD ( Student ), and his/her parent(s) or legal guardian ( Parent ). The Student and Parent(s), in consideration of being provided with a PLD, software and related materials for use while a student at ACSC, hereby agree as follows: 1. Equipment: a. Ownership: ACSC retains sole right of possession of the PLD and grants permission to the Student to use the PLD according to the guidelines set forth in this agreement. ACSC retains the right to collect and/or inspect the PLD at any time, including via network remote access and alter, add or delete installed software or hardware. b. Equipment: PLD configurations are standardized throughout each school. The hardware meets all needs and requirements to participate in a digital learning environment. ACSC will document and retain all serial numbers. c. Substitution: In the event that a PLD is not usable, ACSC will issue a loaner to student. The Student will be responsible for the loaner device as they were for their originally issued PLD. The Student may NOT opt to keep a PLD or to avoid using the PLD due to loss or damage. d. Responsibility of Electronic Data: All data that is stored on PLD is to be backed up by the Student. ACSC is not responsible for any data loss. e. Responsibility of Software: The student may not, unless given prior approval by the Technology Department, install or uninstall any software on the PLD. 2. Damage to or loss of Equipment: a. Responsibility for Damage: It is the sole responsibility of the Student to maintain a working PLD at all times. The student will be required to bring PLD to repair center for service. The student will be required to adhere to guidelines in the Standards for Proper Care document. b. Repair Costs and Billing: The Student and Parent will be billed up to $50.00 for repairs. ACSC reserves the right to charge the Student and Parent the full cost of repair or replacement when damage is due to gross negligence. The following are examples of gross negligence: Leaving PLD unattended and unlocked. This includes damage or loss resulting from an unattended and unlocked PLD while at school. Lending equipment to others other than ones Parent. Using PLD in unsafe environment. Using PLD in an unsafe manner. c. Responsibility for Loss: In the event that a PLD is lost or stolen, the Student and Parent will be billed for the full replacement cost. Students are responsible for the charger that was issued to them. In the event of a lost charger the Student and Parent will be billed for the replacement cost in addition to any other damage or repair costs to the PLD. d. Students that withdraw, are expelled or transferred are required to return the PLD immediately. Any PLD that is not returned within two weeks from the date of such event is considered a loss and billed for full replacement.
2 e. Actions required in the event of damage or loss: Immediately report the problem to the Tech Staff. If the PLD is stolen or vandalized while not at ACSC or at an ACSC sponsored event, the Parent is required to file a police report and provide the Police report copy to ACSC. f. Technical Support and Repair: ACSC will provide technical support, maintenance and repair. Students and Parents should NOT attempt their own repairs. In the event that repairs are attempted the Student and Parent will be responsible for the full replacement cost. 3. Legal and Ethical Policies: a. Monitoring: ACSC will monitor PLD activity and use. There are different methods that will be employed to conduct monitoring. The methods used are to assure compliance with ACSC s Legal and Ethical Use Policies. b. Legal and Ethical Use: All aspects of ACSC s Acceptable Use Policy and School Board Policy Access to Electronic Information remains in effect. ACSC will provide content filtering within the ACSC network and outside networks. ACSC does not have full control of the information on the Internet or incoming from a non-acsc provider or other messaging services. c. File-sharing, File-sharing software and File-Sharing services: The installation and/or use of ANY file sharing is explicitly prohibited. They may not be used to facilitate illegal sharing of copyrighted material (music, video, images, etc.) d. Allowable Customizations: The Student is permitted to create, edit and store files on their PLD. The Student is also allowed to edit the graphical interface to their liking. However, third party software is NOT allowed to be installed for such purposes. Customizations must follow the Acceptable Use Guidelines. e. Reimaging and configuring: ACSC reserves the right to re image and reconfigure PLD at any time if any of guidelines for use are violated. This will return the PLD to its original state and erase all data that was added by users. Revised July 2015
