1 Introduction to Dexcom Portrait on the Web 1.1 Overview of Portrait on the Web Portrait on the Web is an accessory software program intended to allow the transfer of glucose data stored by the Dexcom Continuous Glucose Monitoring System to remote servers for data management. The software can be used by healthcare professionals in their office environments as well as by end users in their home environments. After the glucose data transfer, the information is displayed in charts and tables to help the end users and the healthcare professionals analyze trends and patterns in glucose levels. Reports can be downloaded, saved or printed. At-home end users can also reports. Caution: Federal law restricts this device to sale by or on the order of a licensed healthcare professional. WARNING: The software does not provide any medical advice and should not be relied on for that purpose. A progress bar will appear on the following screens: Page 18 of 49

Upload in Progress Page 19 of 49

When the progress bar is complete, the data upload has completed, and the Trend Patterns Report will show on your computer screen. Page 20 of 49

If the upload was not successful, you will see the following screen: The screen will feature suggestions on what to check (for example, the Receiver is not connected) in a yellow box across the top of the window. Follow these directions to correct the problem. Then click Try Again to try the upload process again. 4 Charts/Reports The Trend Patterns Report will show on your computer after the upload is complete. Page 21 of 49

4.1 Trend Patterns Report Page 22 of 49

The Trend Patterns Report helps identify patterns of high and low glucose trends. The pattern map shows daily trend traces so that healthcare professionals can quickly visualize the high and low trends over time and discuss these patterns with patients. The Trend Patterns Report is divided into four sections: Daily Trends The Daily Trends graph lets you see several days of sensor glucose readings over each hour of the day, all on one 24-hour graph. Each day is a different color dot line on the graph. This can help you see patterns and trends in sensor glucose readings from hour to hour and from day to day Pattern Map A lighter shade of color shows a low/high pattern. Lows are shown in shades of red. Highs are shown in shades of yellow. A darker shade means that the glucose values are farther Page 23 of 49

from target (distance), and how many of those glucose values (frequency) are outside the target range. Target is the target glucose range defined for the glucose data displayed. Distance is how much the glucose value is different from the target range. Distances farther away from the target are given more weight than closer distances and are displayed in a darker shade. The distances are averaged over the range of days selected. Frequency is how many times in the given range the glucose value is over or under the target. A significant pattern occurs when the glucose values are repeatedly outside the target range for frequency and average of weighted distances. Page 24 of 49

4.1.3 Pattern Insights Summary The Pattern Insights Summary shows how many patterns are found for each of the four preset patterns (Nighttime Lows, Daytime Lows, Nighttime Highs and Daytime Highs). It also shows when the most significant pattern occurred. If no significant patterns are found, the No significant patterns detected message is displayed. Page 25 of 49

The Some Possible Considerations section lists actions to consider if certain patterns are detected. They are displayed next to the appropriate pattern in the Patterns Insights Summary. Page 26 of 49

4.1.4 Statistics The Statistics section shows different statistics for the selected days. The Sensor Usage statistics show the number of days in which sensor readings were taken out of the total number of days in the report. The Calibrations / Day statistics show the total number of calibrations divided by the sensor usage days. The Standard Deviation statistics represent a measure of variability in sensor glucose readings around the average. The larger the standard deviation, the larger the variability. The Target Range is the target glucose range. See Section 5.2 to change the target range. The Nighttime Range is the fixed time range for nights. WARNING: Patients must consult a healthcare professional before making any medical interpretations and therapy adjustments from the information in these reports. Page 27 of 49

5 Customizing a Report With Portrait, you can customize the Date Range and the Target Glucose Range used to create reports. 5.1 Date Range You will see data from the Last 7 Days when the report is first shown. You can choose a different date range to see more data by using the drop-down box at the top of the report screen for Date Range. To change the Date Range, click the down arrow in the Date Range box, and choose from Last 14 Days or Last 30 Days. The software will automatically update the report to the new date range. Page 28 of 49

5.2 Target Glucose Range The Target Glucose Range of mg/dl is the range shown by default on the report. To change the Target Glucose Range, click the down arrow in the Target Range dropdown box and choose the target range of mg/dL. The software will update the Report automatically to show the new range. Page 29 of 49

NOTE: If you click on the Help link in the top right hand corner of ANY screen, you will find helpful tips. The Help link in Trend Patterns gives you tips on viewing the data. 6 Downloading, Saving, Printing and ing Reports 6.1 Report Formats Page 30 of 49

Trend Patterns Report If you download/save the Trend Patterns report, the report shown on your computer screen is how it will appear in PDF. Page 31 of 49

Daily Plots If you save the Daily Plots, the software will create a PDF report with daily glucose plots. This report shows a separate graph for each day of glucose data. It is laid out the same way as the Trend Patterns report, except that only one day s worth of glucose values is shown on each graph. If you choose Last 7 Days as the Date Range, the Daily Plots Report will show up to seven daily graphs. If you choose Last 14 Days as the Date Range, the Daily Plots Report will show up to 14 daily graphs. If you choose Last 30 Days as the Date Range, the Daily Plots Report will show up to 30 daily graphs. Page 32 of 49

Excel Data If you save glucose data as Excel Data, all the data contained within the Receiver will be included in the Excel file. There are five different tabs in the Excel worksheet: Accounts Tab This tab includes information such as Dexcom account identifiers. CGM Tab This tab includes information such as CGM readings, time of CGM readings, serial number of device, unit of measurement, and account information. NOTE: Note: The Device Serial Number is only shown on the at-home end user report. Page 33 of 49

Events Tab This tab includes information of events entered into device, such as CGM calibrations and exercise information. NOTE: The Device Serial Number is only shown on the at-home end user report. Device Tab This tab includes information about the device type and serial number. NOTE: The Device Serial Number is only shown on the at-home end user report. Meta Tab This tab includes information about the software product. 6.2 Downloading and Saving a Report You can download and save a Portrait report on your computer in PDF (portable document format) or as Microsoft Excel data (.xlsx files). Page 34 of 49

Note: You must have a PDF reader installed on your computer to view a PDF report from Portrait. (An example is Adobe Reader, which can be installed for free online.) You must have software that can open Microsoft Excel Data to view Excel data reports. (An example is Microsoft Excel Viewer, which can be installed for free online.) To download and save a report, click on the Save Report drop-down box at the top of the Report. You have the option to save the Trend Patterns, the Daily Plots, or the Excel Data. Page 35 of 49
