2 UMR Health Risk Assessment Getting Started The UMR Health Risk Assessment helps you focus on making healthy lifestyle choices as well as evaluates your risk for developing a chronic disease so you can take action for disease prevention. To complete the Health Risk Assessment, simply follow these instructions to get started with the program! If you have any questions, please contact a Health and Wellness representative at

3 Step 1: Go to Step 2: First time users, click Need a Username? Register Here. If you have previously registered, please enter your username and password in the Member login box, and click Submit. Then, skip to step 5. If you do not remember your username and/or password, you can re-register. 3

4 Step 3: EMPLOYEES - select Yes to the question Do you have a benefits ID card? Select the option I am the employee/retiree. Click on Continue. If you are a SPOUSE go to Step 3.1 4

5 Step 3.1: SPOUSES, select Yes to the question Do you have a benefits ID card? Select the option I am the spouse or dependent. Click on Continue. If you are an EMPLOYEE go to Step 3 5

6 Step 4: EMPLOYEES - Provide the following information: If you are a SPOUSE, go to Step 4.1 First Name Last Name Member ID (located on your UMR Benefit ID Card, no dashes) Group Number (located on your UMR Benefit ID card, no dashes) Date of Birth Gender address (optional) Create a username and password. Select and answer two security questions. Be sure to record these for future reference. Click Continue 6

7 Step 4.1: SPOUSES - Provide the following information: If you are an EMPLOYEE, go to Step 4 First Name Last Name Employee ID (member ID number, located on your UMR Benefit ID Card, no dashes) Group Number (located on your UMR Benefit ID card, no dashes) Date of Birth Gender address (optional) Create a username and password. Select and answer two security questions. Be sure to record these for future reference. Click Continue 7

8 Step 5: Click on Get Started, located in the Visit the My Health Center box. 8

9 You will be re-directed to the Health Risk Assessment 9

10 Step 9: Click Start an Assessment The Health Risk Assessment takes approximately 20 minutes to complete. 10

11 Complete the Health Risk Assessment You will need your waist and hip measurement See next two slides on measuring waist and hip 11

12 Getting your waist measurement Wrap the tape measure around your abdomen, just above your hip bone. Be sure the tape is snug, (but does not compress your skin) and that it is parallel to the floor. Relax, exhale, and measure your waist. 12

13 Getting your hip measurement Wrap the tape measure around your hips, at their widest point. (Approximately 6 inches below your abdomen - around your buttocks) Be sure the tape is snug, (but does not compress your skin) and that it is parallel to the floor. Measure your hips. 13

14 Important Information When you have answered all of the questions on the Health Risk Assessment, you must select Lock HRA. If you do not lock the Health Risk Assessment, it will be considered incomplete and will not get processed. 14

15 Viewing your Health Risk Assessment Results Once the Health Risk Assessment has been locked, you can immediately view your results. Click on User Report to print a copy of your results. Click on Provider Report to print a summary of your results. 15

16 How to log in to review Wellness Tools Go to Click on Members Enter your member ID number, located on your UMR Benefit ID card (no dashes), and click Go to my online services Enter your username and password in the Member login section and click Submit Click on mycaremanagementcenter In the My Health Tools box, located in the lower, middle section of the page, click on Get Started 16

17 How to access your Health Risk Assessment results report You can review your Health Risk Assessment results by clicking on Review Results, located beneath the Assessments subtitle, or by clicking on View Report. 17

18 Action Plans Action Plans are interactive online educational sessions which offer plans for a variety of lifestyle behaviors, so you choose the one that best fits your health needs and interests. Available Action Plans: Healthy Eating Weight Management Smoking Cessation Exercise and Activity Stress Management Diabetes Prevention Heart Disease Prevention Risky Drinking Depression 18
