Requirements of System The Server recommendations to run the Horizon POS system are below: Quad Core Intel Xeon X3430 @ 2.4 GHz, 8MB Cache 4 GB RAM 2x 146 GB 10K SAS; RAID 1 Windows 2008 Server DVD-ROM MS SQL Server 2012 Standard Open License Microsoft SQL User CAL 2012 A server and SQL licensing meeting the above requirements can be purchased from Horizon or purchased by client. A dedicated server is not required; however Horizon will need to approve server prior to implementation to ensure there are no conflicts. Software Modules Included in Proposal Additional Software Modules Available * * Available from Horizon at additional costs Manager s Office Back office management system. This required module allows users to edit menus, view/change patron information, manage meal plans, run reports, etc. Multi-User License Includes four licenses that may be installed on existing community desktop computers, so that users will be able to access the Central Office software. Additional blocks of licenses can be purchased for $250 per block of four licenses. General Ledger Interface and Module Allows exporting of transaction data with client defined billing codes to accounts receivable or billing modules. Interfaces by generating a delimited text file that can be imported to most accounts receivable or billing systems that allow importing of delimited text files. The delimited file can be user defined, including the delimiter type, field order, field formats and field constants The delimited file is typically imported as a batch charge file to the existing billing software Ability to separate billable items by groups or categories User defined billing codes can be assigned in the system Transaction data can be exported in either summary form by billing code, or detailed form by billing code Export of transaction data is not time sensitive, and can be run at any point for import into client accounting/billing systems. The export can be scheduled to occur automatically on a user-defined day. Additional Payment Options: The Horizon system can track meal plans, declining balances, inclining balances, and employee balances. Choose to pick up your order or have it delivered to you. Catering requests are handled quickly. Fine Dining: The Table Service module allows restaurant style order processing at the Point of Service terminal. Desktop PC can be used to run the software without the need for specialty hardware; however minimum specifications must be met. MyResidentOnline: MyResidentOnline provides a convenient online payment system plus the ability to monitor purchases. Take payments via credit cards, debit cards and e-checks. Have 24/7 ability to monitor purchases and account balances. Allow residents and family members to monitor eating habits. TempAlert Monitor temperatures anywhere with TempAlert, Horizon s wireless and paperless temperature management system. Horizon is pleased to offer this easy-to-use tool that will help you ensure food safety, maintain compliance for streamlined audits, and prevent food losses. Reduce the risk of food-borne illness and eliminate hand-written logs. SmartGuard monitors temperatures in hardware: freezers, refrigerators, hot boxes, storage rooms, etc. SmartShield monitors temperatures throughout the cooking process: Receiving to Serving SmartLink records and transmits temperatures to the Online Control Center All temperatures are recorded on individual devices before transmittal to the Control Center to ensure no data loss Cashless Vending Cashless Vending is a comprehensive, state-of-the-art solution that allows you to integrate existing vending machines throughout your operation, as well as the ability to add new plug and play machines, and have all of these machines seamless integrate with your existing Horizon Point of Service software. Serve healthy meals, snacks, frozen items, drinks, and/or commodities to your customers 24/7. Customers can use their ID Cards (bar code, mag stripe, or prox) to vend items Flexibility: Patrons can use either a decline/debit account, an incline/charge account, or a Payroll Deduct account Interface ready machines from Horizon are feature rich, providing expanded capabilities Costs not Included in the Proposal Proposal and cost estimate do not include costs associated with the following items, which could be additional expenses to the client: Any network setup costs associated with providing network access and computer configuration to the client network. These costs include running any necessary network cabling and power supply to the Point of Service locations, any required network switches, hubs or routers and providing Horizon with the required configuration information for adding the POS computers and server to the client Network. IT consultant fees need to be considered if the community does not have on-site IT resources. Any Additional End User training required after completion of the installation schedule. An on-site trainer for additional End User training can be scheduled through Horizon software. On-site training is $895 per eight hour day of training, plus travel related expenses. Additionally, remote web based End User training can be scheduled through Horizon at a cost of $150 per hour. Providing spare terminals for backup. Providing spare peripheral devices for backup or replacement. Page 5
Expanding the system to other venues. If the client wishes to expand the system beyond the scope of this proposal, a separate proposal will need to be developed based upon the specific configuration that is desired and/or attainable using the client network infrastructure. The proposal would depend upon the desired number and configuration of terminals that will be added to the system. Travel related costs for future certification training. If the client requires training of a replacement system champion in the future, the cost of travel to the training sessions is the responsibility of the client. The training will continue to be free of charge and offered on a monthly basis. Travel Expenses for System Engineer and Trainer. These will be billed based on actual expenses incurred. Page 6

Annual Maintenance and Subscription Technical Support (1-5 devices) This is a flat rate fee. It covers all toll-free technical support per year for up to five software licenses. Technical Support (Over 5 devices) This fee is charged for each license above 5. It also covers all toll-free technical support per year. Point of Service Maintenance This is broken into two parts. They both cover unlimited software upgrades, service packs and patches. o Maintenance - enhancements & upgrades Manager Workstations / Central Office Server is for each Central Office and Manger Machine license, basically, each site license. o Maintenance - enhancements & upgrades Line Terminals is for each Terminal license that is not upgraded to a manager machine. This would be each line machine at each site. Credit Card Authorization This is a flat rate fee. It covers all toll-free technical support per year, unlimited software upgrades, service packs and patches for the Credit Card authorization module. Software Maintenance General Ledger This is a flat rate fee. It covers all toll-free technical support per year, unlimited software upgrades, service packs and patches for the General Ledger module. Software Maintenance Payroll Deduct This is a flat rate fee. It covers all toll-free technical support per year, unlimited software upgrades, service packs and patches for the Payroll Deduct module. Software Maintenance Table Service This is a flat rate fee. It covers all toll-free technical support per year, unlimited software upgrades, service packs and patches for the Table Service module. Maintenance Back Office This is calculated at 18% of software price and covers all toll-free technical support per year, unlimited software upgrades, service packs and patches. TempAlert Annual Software Subscription This is calculated per group of devices (1-5, 6-10, 11-15, 16-20, etc.), counting all TempAlert hardware (Guards, Shields, and Links). It covers all toll-free technical support per year, unlimited software upgrades, temperature data storage for up to 7 years, service packs and patches. Annual Maintenance Vending This is a flat rate fee. It covers all toll-free technical support per year, unlimited software upgrades, service packs and patches. Horizon Terms and Payment Schedule Horizon s fees are categorized as follows: (1) Hardware (2) Software License Fees (3) Maintenance (4) Services (5) Taxes Payment of these fees in accordance with the following terms will prevent unexpected delays in implementation and/or service. HARDWARE: All hardware is billed upon shipment and payment is due within 30 days from the invoice date. HARDWARE RESTOCKING FEES: If you wish to return hardware, there will be a 20% restocking fee plus shipping and handling. Hardware returns will not be accepted after thirty days. SOFTWARE LICENSE FEES: Software is billed upon installation/delivery and payment is due within 30 days from the invoice date. Page 15
Your software is installed with temporary registration codes; upon receipt of payment you will be issued permanent registration codes. If payment is not received in accordance with the stated terms, your temporary codes will expire causing the system to shut down. This disruption can be avoided by insuring that payment is made in a timely manner. MAINTENANCE: Maintenance is an annual fee that covers technical support, upgrades and enhancements made to the software and any documentation updates. Enhancements and improvements to Horizon s system are passed on to you in the form of upgrades through your maintenance program. Please note that after the initial year Horizon reserves the right to increase annual maintenance fees by the CPI. Maintenance is billed at the time registration codes are installed/delivered and then annually on the anniversary date of your installation. Invoices are mailed early to allow ample time in which to make payment. SERVICES: All services are billed upon completion and payment is due within 30 days from the invoice date. TAXES: Customer is responsible for any applicable sales, use, or any value added or similar taxes (collectively, Sales Taxes ) payable with respect to the provision of the software, hardware, services and maintenance to customer, or arising out of or in connection with these terms, other than taxes based upon Horizon s net income. Unless expressly specified otherwise in any sales order, all fees, rates and estimates exclude Sales Taxes.
