Summary of Services LiveWell 24/7 is a comprehensive wellness program that empowers employees of all capabilities to take control of their health. Through strategic development of a wellness program that fits the needs of your company and your employees, our wellness staff can help direct initiatives to their full potential at your company. LiveWell 24/7 has the tools to help raise employee awareness and knowledge about current and relevant health topics as well as providing online and self-care resources for employees and their families. Our wellness programs and staff will lay the groundwork for healthier choices you and your employees choose what parts of the program are most beneficial for your organization. With the proper planning and implementation techniques, our wellness staff can help devise a sustainable wellness plan that will yield a positive return on investment for your company and better health and productivity for your employees. 4

Informed@Work Our LiveWell 24/7 staff members present informational programs on relevant health and wellness topics. These interactive and informative PowerPoint presentations take 45 minutes and are followed by 15 minutes of questions and discussion. The topics can be delivered as seminars, webinars, or Lunch and Learns. Multiple sessions can be accommodated in one day and/or for multiple locations. The LiveWell 24/7 team will create flyers for your organization based on the topic presented. These flyers can be given to all employees, placed around the workplace, and announced via email. A Lunch and Learn is an educational event for your employees scheduled over the lunch hour. It is an informal way to present timely health education topics. Employees can bring their lunch and eat during the program; however, providing lunch can be an incentive to attract your employees. Lunch & Learn programs are recommended for 25 35 participants per session depending on space available. Topics include: Back Pain Prevention Cancer Prevention COPD Prevention DASH Diet Diabetes Management Financial Wellness Healthy Children Heart Disease Prevention Maternity Migraine Nutrition Sleep Disorders Smoking Cessation Stress Management Weight Management 5

12 Week Tobacco Cessation Program Outline This program targets all forms of tobacco addiction and is based on group participation. For participants to successfully complete the program the following will be required: Enrolled Participants Sign a commit to quit sheet to participate in 12 week program. Tobacco Cessation Homework Assignments completed weekly. Compliance of required meetings will be reported. Graduation Ceremony. Educational Group Presentation Meetings Held at Work-site Location with all other locations calling in via conference line. (1) Five Group Meeting Sessions - Five advanced sessions with educational presentations on the steps of how to successfully quit tobacco products. These meetings are full of support, encouragement, and education to help the participants during difficult times and days of relapse. (2) One one-on-one individual session with a nurse. (Each enrolled participant must meet with the Nurse for one 20 minute session for successful completion of the program. This meeting is scheduled at the second session. This allows each participant the opportunity to privately discuss nicotine replacement therapy options and the potential need to meet with their primary care physician regarding prescription medications.) Compliance - Clarke and Company Benefits will track all participants. - Will require Opt-Out Form, for those who drop out. - Send letter to non-compliant employees. - Final Reporting to Employer. Total Cost: $200/participant Clarke and Company Benefits will require a minimum of 10 employees to effectively run this program. Enrolled participants are to be established after the first meeting. Employer will provide space (tables and chairs) for the classes to take place at the worksite. Health Fair Organization Health Screenings our wellness staff has strong ties to hospitals in all of our clients areas. These relationships make it convenient for us to schedule free, onsite biometric screenings at your health fair. Carriers we bring a representative from your health insurance carrier on-site to talk to employees about the wellness and disease management programs they offer, as well as answer any questions they might have. We can also coordinate having your dental or vision carrier on-site. Employee Assistance Plan If applicable, we can arrange for someone to come and answer your employees questions about their EAP. Primary Care preventing problems using regularly scheduled visits with a primary care physician is one of the best ways to stay healthy. We can coordinate bringing a local primary care physician to your health fair to schedule well-checks, and to talk more about the importance of prevention. Fitness our wellness staff arranges for a local gym to send a representative to your health fair to talk about membership options, facilities, and general fitness. If you are interested, our wellness staff can also work on getting corporate group discount rates for your company beforehand, making it more convenient and less expensive to register at your health fair. Nutrition healthy eating is an important part of keeping your employees healthy, so we make sure that it is an important part of your health fair. We bring someone onsite from a local, healthy grocery store to bring samples of nutritious food, and to talk more about eating a healthy diet. Vision our wellness staff can find a local optometrist to do vision screenings and make referrals for your employees. Advocacy and Awareness our wellness staff connects with local chapters of national health advocacy organizations to come and share information at your health fair. We frequently invite organizations like the American Heart Association, the Juvenile Diabetes Research Fund, the American Cancer Society, and more. Safety Local fire departments and local American Red Cross chapters are great resources for safety at home and at work. Living Local it is important that your health fair is relevant to your employees. Bringing in local organizations and businesses specific to your community adds character to your health fair. Your local department of parks and recreation, a local outdoors recreation center, or a local community coalition can be great assets to your employees. LiveWell 24/7 our wellness staff can provide resources on different health topics that can help participants based on biometric results (weight management, cholesterol, blood pressure, diabetes, etc.), as well as more information on how to get involved with LiveWell 24/7 programs. 7

The Buzz Newsletter The Buzz! is our weekly wellness newsletter that includes a featured wellness article, health and wellness facts, healthy recipes, active events in your area, a fitness or health focused app of the week, and a stretch or move of the week all in one email. 8

LiveWell Blog The LiveWell 24/7 blog is written by our wellness department staff featuring national health observance information, local and current events in health and fitness, and up to date information regarding health and wellness. 9

Claims Data Analysis We have the resources to help you collect and analyze claims data from multiple sources. We will measure and monitor several data points over time to determine the success of your company s wellness programs and initiatives. Our claims data analysis and tracking is one of the key ways to determine your return on investment. 10
