Admission Criteria Admission to the Rehabilitation Center is determined by the Rehabilitation Physician with assistance from the Rehabilitation Admission Nurse once cleared by the referring physician. The patient should be able to participate in and benefit from the full range of rehabilitation services. Patients appropriate for referral include those with notable impairments in two or more of the following areas: General mobility: Difficulty with turning in bed, sitting up, balancing, transferring, or maneuvering a wheelchair. Self-care skills: Problems with bathing, dressing, feeding self, swallowing, toileting, home management and money/math skills. Perception: Poor memory, lack of insight into limitations, poor concentration, disorientation, difficulty with making decisions, impaired safety awareness, difficulty understanding their environment, and impaired judgment. Communication: Difficulty with speech or understanding words, reading and or writing. Endurance: Difficulty tolerating the energy demands of basic daily living activities and household mobility. Psychosocial functioning: Impaired ability to initiate or interact with others in a manner which is appropriate. Multiple Medical Conditions Major factors in determining appropriateness for rehabilitation admission are functional impairment, the ability to benefit from a comprehensive rehab program, the patient s social support network, and discharge disposition. Reimbursement Hospitalization costs for rehabilitation usually are covered by Medicare, Medicaid and other major health insurance programs for those who qualify for the program. Determination of reimbursement is made prior to admission. Financial counselors are available to assist in determining the level of coverage by an individual s insurance. Costs can be provided upon request during the pre-admission assessment. Rehabilitation Center The 21- bed Rehabilitation Center at Ministry Saint Joseph s Hospital is specifically designed for rehabilitation patients. Private and semi-private rooms are available. Professional rehabilitation nursing is provided 24 hours a day. These impairments may be the result of: Stroke Neurological Disorder Brain Injury Orthopedic Condition Spinal Cord Injury Multiple Trauma Amputation Professional therapy services are provided on the unit for maximum convenience and patient accessibility. Independence Square, a replica of a community setting, provides an additional treatment area. It is complete with a grocery store, deli, theater/church, bank, car and farm work area where you can work with your therapists to practice skills needed to increase your independence once you are discharged. 4 Ministry Saint Joseph s Hospital Rehabilitation Center 5
Accreditation The Commission on Accreditation of Rehabilitation Facilities (CARF) has awarded the following accreditations to the Rehabilitation Center at Ministry Saint Joseph s Hospital: Rehabilitation Team A team of health care professionals work to help every patient obtain the highest level of function as possible. Depending on the patient s needs, the rehabilitation team will include any or all of the following: Physiatrist (Physician specializing in rehabilitation) Inpatient Rehabilitation Adults (maintained since 1975) Inpatient Rehabilitation Children and Adolescents (through our Peds Program) Stroke Specialty Program Rehabilitation Nurse Physical Therapist Occupational Therapist Speech and Language Pathologist (from the Marshfield Clinic) Recreational Therapist Neuropsychologist Case Manager Social Worker Dietician Assistive Personnel Additional members as indicated We work together to help every patient obtain the highest level of function as possible. 6 Ministry Saint Joseph s Hospital Rehabilitation Center 7
Evaluation To assure early involvement in a comprehensive rehabilitation program, evaluation by the rehabilitation team is initiated upon admission to the unit to determine: Medical and neurological status Strengths and limitations in sensory, motor and speech/language abilities Psychosocial needs Level of cognitive functioning Special equipment needs Long and short-term goals Family support Rehabilitation Conferences Following the completion of the evaluation by your team members, a rehabilitation evaluation conference is held. You will be asked for your input prior to the conference regarding issues, concerns and your personal goals. At this conference the team will meet and discuss your care, goals, and the estimated length of stay appropriate for you. Your nurse, physician, or case manager will discuss the conference results with you and your family. After the evaluation conference, a rehabilitation conference is held weekly to evaluate progress, make changes in your plan of care if needed and plan for your discharge needs. Family conferences may be necessary during your stay. You and your family are asked to attend these conferences to review progress, goals, and time periods suggested in your rehabilitation. These conferences will be coordinated by the case manager and/or physician with your family and other team members. Skilled therapy services are provided Monday through Saturday with limited availability on Sunday. At least 3 hours of therapy will be provided each day during your stay. Follow-through by the rehabilitation nurses assures consistency on a 24-hour basis in meeting your goals. Family Participation What to bring While rehabilitation team members work to care for the entire spectrum of patient needs, the patient s family is expected and encouraged to participate actively in the treatment program. Family members are encouraged to maintain involvement in the rehabilitation process by attending therapy sessions, through discussions with team members and by working with the team on specific patient goals. The family is often the most important factor in determining the patient s future quality of life. Teaching and counseling the patient s family is an integral part of the rehabilitation program. A patient education binder is provided for each patient to keep important materials that you obtain during your stay. Discharge Discharge planning begins upon admission and continues throughout the rehabilitation program. Discharge will occur as determined by the rehabilitation physician, in collaboration with the rehabilitation team and the patient/family. Discharge arrangements are made when the patient is able to manage in a community setting or when it becomes evident that a sub-acute rehabilitation facility is needed for extended rehabilitation. The team assists the family in scheduling further rehabilitation or medical follow-up, making long- or short-term living arrangements, and obtaining information about services or programs available in the community. 3-4 sets of comfortable clothing 3-4 undergarments that you would change daily Socks and sturdy shoes (laces or Velcro closure) Belt and/or suspenders (if you would normally wear) Glasses and/or contact lenses (supplies) Dentures/partials Hearing aids Electric razor (if you have one) Personal toiletries that you would like daily (shampoo, conditioner lotion, make-up, comb/brush, toothbrush, etc.) Warm coat or sweater Any adaptive equipment used at home (walker, cane, etc ) Family support is encouraged throughout the rehabilitation process.
