When a child s life is interrupted by injury or illness, whether new or longstanding, dealing with the physical and emotional changes can be a challenge for your child and your family. It s hard for parents to see their active, happy and vibrant child suddenly slow down. At Nemours/ Alfred I. dupont Hospital for Children, our team of specialists will help you and your child regain momentum and lead you through the process of rehabilitation. Our team offers support and guidance to encourage independence and will reacquaint you and your child with the everyday business of living, growing and playing. In addition to promoting increased independence, we will help you adjust to the long-term changes you may be facing. We believe that the family s participation in educational programs, support groups and family conferences is critical to your loved one s successful rehabilitation. At dupont Hospital for Children you are the most important part of your child s treatment team. 1
4 The Rehabilitation Program At dupont Hospital for Children we provide medical, nursing and therapeutic services for children and adolescents with a wide range of rehabilitation needs. Programs are designed to return your child to home, school and normal activities under the best possible circumstances. The goals of rehabilitation are to maximize your child s ability to be active, to communicate and interact with people, and to live as independently as possible. Inpatient Care The inpatient program is composed of coordinated and integrated services that include evaluation and treatment, and stresses the importance of education and training for the child and family. The goal of the program is to improve your child s ability to perform daily activities. It is geared toward children who have severe functional limitations of recent onset or as a progression of their illness, disease or injury. Outpatient Care The Comprehensive Outpatient Rehabilitation Program (CORP) is designed for children and adolescents who require the intensity of an inpatient rehabilitation program, but who are medically able to live at home with their families and make progress toward specific functional rehabilitation goals. After discharge from inpatient care, the rehabilitation team continues caring for the child through outpatient visits and team conferences. 2
5 Family-Centered Rehabilitation Programs We offer a functional, comprehensive, familycentered approach to pediatric rehabilitation. This care is provided by pediatric rehabilitation physicians who are board-certified, along with occupational therapists, physical therapists, speech-language pathologists, child life specialists, rehabilitation nurses, neuropsychologists and teachers. Children and adolescents through age 17 receive coordinated, interdisciplinary care as inpatients on the rehabilitation unit and in our comprehensive outpatient rehabilitation program. Throughout the course of your child s rehabilitation, the team evaluates the child s progress and revises the individualized rehabilitation plan to meet his or her changing needs. We offer individual levels of therapeutic outpatient programming Monday through Friday. We review each child s progress, treatment plan, therapy schedule and goals in regularly scheduled team conferences. The family (and child, when appropriate) participates in the team conferences. We encourage you to be involved in your child s care, whether inpatient or outpatient, and we strive to create a supportive experience. Our comfortable, attractive setting offers a peaceful respite, while the depth and quality of services available provide reassurance. Our Family Resource Center offers a place to relax during your child s hospital stay, along with a library and business center, kitchenette and laundry room. We also have a free tram service that (weather permitting) offers a rejuvenating ride though the Nemours estate of Alfred I. dupont. The Ronald McDonald Family Room provides sleeping and personal facilities onsite and the 50-room Ronald McDonald House is across the street. Diagnoses Both the inpatient and outpatient programs treat a range of diagnoses including: Traumatic brain injury, including concussion Multiple trauma Congenital and acquired neuromuscular disorders including cerebral palsy, spina bifida, muscular dystrophy and juvenile rheumatoid arthritis Burn disorders Chronic musculoskeletal pain including myofascial pain syndromes and reflex sympathetic dystrophy/chronic regional pain syndrome (RSD/CRPS) Lymphedema, both congenital and acquired Spasticity management, including botox and intrathecal baclofen Prescription/management of assistive devices, bracing/splinting, wheelchairs, seating, adaptive equipment and prosthetics/orthotics Oncology disorders Orthopedic disorders Spinal cord dysfunction Respiratory conditions, including mechanical ventilation 3
6 The Pediatric Rehabilitation Team The team starts with the family! The family is an integral part of the rehabilitation process. As the constant in a child s life, family input and sharing help shape the overall rehabilitation plan. During rehabilitation, your child will be cared for by an interdisciplinary team whose members work closely together to address the total needs of the child. The dupont Hospital for Children team includes: PEDIATRIC REHABILITATION MEDICINE PHYSICIAN (PHYSIATRIST): This boardcertified physician specializes in physical medicine and rehabilitation. Our rehabilitation physicians help to coordinate your child s care while they are participating in the rehabilitation program. Our physicians are trained to care for the physical and cognitive needs of children. They direct and work with the team to maximize therapeutic interventions. ADVANCED PRACTICE NURSE/REHABILITATION SPECIALTY NURSE: This nurse works in conjunction with the pediatric rehabilitation medicine physician to oversee the clinical management of your child in the rehabilitation program, conducting ongoing assessments and evaluations of the child s progress. REHABILITATION NURSE: This nurse monitors daily progress, develops 24-hour care plans, provides family teaching and coordinates the child s rehabilitation program during the inpatient stay. The nurse also encourages the child s independence and self-care through teaching. OCCUPATIONAL THERAPIST: This team member works to increase the child s independence in activities of daily living as well as improve arm function, fine motor skills, visual motor skills and functional cognitive skills. The OT also will address the need for upper extremity splints, casts, adaptive equipment and home modifications. 4
7 PHYSICAL THERAPIST: The physical therapist works to enhance the child s strength, gross motor skills, coordination, balance and endurance. This therapist will also assess the need for any special positioning equipment, lower extremity casts, braces and mobility aids. SPEECH-LANGUAGE PATHOLOGIST: This team member provides diagnostic assessment and treatment for speech, language, auditory and cognitive disorders, evaluates the child s oral motor skills for eating and swallowing and addresses the need for augmentative communication devices. NEUROPSYCHOLOGIST: The neuropsychologist evaluates the cognitive (thinking) and emotional status of the child/teen with specialized evaluations that target aspects of memory, attention, language and abstract thought. Emotional and behavioral issues are evaluated and treated. Teen support groups and individual therapies are used to promote awareness and coping with the challenges of rehabilitation. TEACHER: The teacher develops an individualized program based on each child s specific needs, working with the child s home school as appropriate. SOCIAL WORKER: The social worker supports the family throughout the entire rehabilitation process and helps to access community resources and facilitates communication. CHILD LIFE SPECIALIST: This team member helps the child cope with the social and emotional impact of hospitalization through therapeutic play, social, recreational and group activities. RESPIRATORY THERAPIST: This therapist provides evaluation and treatment services for all types of breathing complications. REHABILITATION ENGINEER: This team member uses innovative wheelchair adaptive devices, custom-designed aids and electronics to enhance the seating, positioning and specific mobility needs of the child. REGISTERED DIETITIAN: The dietitian assesses the nutritional needs of the child to determine appropriate foods and caloric intake. The dietician also teaches the child and family about nutritional matters after discharge. PHARMACIST: The pharmacist reviews all medications for drug interactions, appropriateness for medical issues and dosing, as well as ensuring accurate, safe preparation of all products, and meets regularly with the rehabilitation physicians. INTERNAL CASE COORDINATOR: This nurse is responsible for accepting referrals and communicating with insurance companies. They facilitate discharge planning with the family and the rehabilitation team. 5

10 Admissions and Referrals The rehabilitation program accepts referrals from parents, physicians, physician assistants, nurse practitioners, insurers, social workers and discharge planners. Our financial counselors are able to work with families to design an affordable payment plan. The Nemours Financial Assistance Program is available to help pay for care the family could not otherwise afford. To get this special assistance, families must complete an application. 11 Directions to the Hospital The Nemours/duPont Hospital for Children is easily accessible from Delaware, Pennsylvania, New Jersey and Maryland. Parking is free, and free valet parking is available at the inpatient entrance (near the playground) and visitor s lobby (behind the Hospital). From the North (Route 202) Take Route 202 South. You will see the AstraZeneca campus on the right. Make a right at the light onto Powder Mill Road (Route 141). Driving under a walkway overpass, stay on this road and it will become Children s Drive and take you through the hospital gates. From the South (I-95) From I-95 North, take Exit 8 Concord Pike/ Route 202 N exit. Stay to the left. Once on Route 202, get in the far left lane. Turn left at the third light onto Powder Mill Road (Route 141). Driving under a walkway overpass, stay on this road and it will become Children s Drive and take you through the hospital gates. From the West Take Route 141 North (Barley Mill Road), crossing the Tyler McConnell Bridge. Turn left at the dupont Experimental Station staying on Route 141 North (Powder Mill Road). Travel a mile and a half, making a right at the second light onto Children s Drive. The hospital is straight ahead through the gates. Directions for Rockland Center I, II, and Ronald McDonald House Follow the directions above. On Children s Drive, facing the hospital s stone gates, turn left onto Rockland Road. Rockland I is the first office building on your left. Rockland II is the second building on your left. A parking garage is located between I and II.
