This information leaflet has been written to provide you with some information about the Oxfordshire Community Neurology Specialist Nurse Service. Aims of the Community Neurology Specialist Nurse Service This service was developed and supported by NHS Oxfordshire in partnership with the Oxford University Hospitals NHS Trust. The service provides advice, guidance and support for people and their families who live with long term neurological conditions.* The aim is to provide expert help and support locally, when you feel you need it. In order to facilitate management and continuity in your care, a close working relationship with other Neurology services involved in your care has been established. However, your full involvement in all aspects of your care is important to us. * There are some long term neurological conditions that are supported by other established services. If this applies to you, then we will ensure that we signpost you to the appropriate service.

Meet the team Jo Bromley (Team Leader) Sue Barnden Nicola Findlay Louise Ludlow Sonia Sherrell Sue Gunawardena (Administrator) Lee-Anne Hart (Administrator) Each nurse will take a lead in a particular neurological condition, but will work together as a team to provide collaborative advice, support and guidance where and when it is needed. Who can be referred? Anyone can access the service for information. However, you will need to be referred by a health care professional if you need to be seen and assessed by one of the Community Neurology Specialist Nurses. You need to be over the age of 18. You need a confirmed diagnosis of a neurological condition.* You must be registered with an Oxfordshire, Brackley or Byfield GP.

What the service offers Nurse-led clinics around Oxfordshire and South Northamptonshire to provide sensitive assessment, problem-solving and support. This may include referrals to other services where required. Home visits (where appropriate). Support, advice and education to community health care professionals and other residential and social care providers in the community. A passion and commitment to continually review this service and also develop other services to meet people s needs and help improve quality of life. A general telephone clinic on a Tuesday from 9.00am - 10.00am, where there will be a Nurse Specialist available to talk on the phone. Reasons for referral Your hospital-based Specialist Neurology Nurse may refer you for shared care support in the community. Community health care professionals may want advice and support (with your consent) to help with your care and treatment in the community. You are newly diagnosed. You may have complex needs and need support with managing your neurological condition in the community. You or your family / carers need information, support and advice about your condition. You may need a review of the management of your condition (for example, you may need referring to other services).

Contact information The service is available Monday to Friday (excluding Bank Holidays) from 8.30am - 4.30pm. If you require urgent advice outside of these hours, then you should contact your GP or out-of-hours GP service. Contractual hours are specific to each Nurse Specialist. However, the service attempts to be flexible where possible to meet the individual needs or people and their families / carers.
