1 Freedom of Information request & West Norfolk CCG response WN279 - Community Palliative Services Please note, tables are documented below 1. Do you commission a 24/7 dedicated and clinically staffed palliative service specifically designed for patients dying at home, and their carers? We do not currently commission such a service. At present, we commission a 24/7 generalist out of hours (OOH) support service. At this time, we do not commission a specialist palliative care visiting service 7 days a week; however we do commission a dedicated palliative care advice line for healthcare professionals. Our YourNorwich integrated work programme is looking at palliative care provision at weekends as priority with a view to supporting patients dying at home. 2. Please complete the table (see the second attachment to this ) detailing which specific palliative services you commission, the hours of operation and geographic areas that they cover for NB. For the purposes of our research, we would only consider you to commission these palliative services listed in the table if they are specifically designed for patients dying, and their carers, as opposed to a more general service that might only incidentally cover end of life care. Please find the completed table attached. 3. What is your annual budget for all dedicated end of life care services commissioned for the current financial year, ? If it is impossible for you to give a precise figure for whatever reason, please give a best estimate. There is no one specific palliative end of life service, and as such no ring fenced budget, particularly as the local community Provider is commissioned on a block basis (a block of funding rather than individual funding streams for particular services). Palliative end of life care is delivered via a number of specialist and integrated services across Norfolk, the Community Provider has given an indication of the resources (funding) they allocate to delivering palliative EoL care. For details, please see the attached word document. If you wish to have data on the services delivered by our acute provider, please contact Norfolk and Norwich University Hospital NHS Foundation Trust.
