1 Rehabilitation services For older people and adults with a disability Revised July 09 Large print as standard
2 Cornwall Department of Adult Care and Support offer rehabilitation services to older people and adults with disabilities. Rehabilitation means assisting a person who has been unwell, or who has disabilities, to live as independently as possible, and to have confidence in their ability to carry out the normal activities of daily living. RATS Teams (Rapid Assessment Teams) There are RATS teams in each district of Cornwall who offer services that can last up to six weeks depending on need. The teams are made up of professionals from Adult Care and Support and the NHS. Anyone can make a referral to the RATS teams if they believe that this may prevent a person s imminent admission to hospital or into care, although most referrals come from health care professionals. 2
3 What services can RATS teams provide? The aims of the teams are to: Provide fast, intensive support and care to avoid a possible hospital admission Facilitate an early discharge from hospital Prevent a marked deterioration in a person s health and wellbeing. This is carried out within a person s home, or local community To do this, the teams may do some or all of the following: Arrange short-term careintensive packages, including night-sitting Provide physiotherapy or occupational therapy programmes at the person s own home Provide intensive nursing support within the person s own home, in addition to the existing District Nursing teams Rapidly deliver specialised equipment to the patient s home 3
4 At the end of the RATS team intervention, a care plan will have been made to meet the continuing needs of the person if appropriate. This will be passed on to the existing Adult Care and Support or Health community teams as appropriate, and normal charges for care and support will apply. You can contact the RATS team for your area via the Adult Care and Support offices, details at the end of this leaflet. District therapy teams Therapists from Adult Care and Support and the NHS work together in District therapy teams. They include occupational therapists, physiotherapists, therapy assistants, technical officers and support staff who are based across Cornwall in Adult Care and Support's 6 area offices (see end of leaflet) or at local NHS premises. 4
5 What services can the district therapy teams provide? District therapy teams aim to enable older people and adults with disabilities to remain in their own homes safely. They help prevent unnecessary admission to hospital or a care home, and work with other teams to make discharge from hospital or residential care easier. The treatment offered may be rehabilitation, adaptations or support for self management of your condition. What you can expect You will be contacted by the team as soon as possible; sometimes this will be by telephone to gather more information. You will be offered an assessment appointment either at your home or local hospital. Your therapist will assess your needs and help you to set your own goals for independence. This may be managing personal care/daily living activities or work and leisure activities. You can contact your district therapy team via the Adult Care and Support office for your area, details at the end of this leaflet. 5
6 What about the cost of Adult Care and Support services? There is no charge for an assessment by Adult Care and Support, and visits to your home, and advice from specialist workers are also free. There is a charge for some services, such as meals or personal care, but you will be given full information about this by staff before any service is set up. Is there anything else I should know? Adult Care and Support have to keep records about the people who use their services. These records are securely kept and remain confidential. If English is not your first language, a translator can be made available to help you understand this, and other, leaflets.
