Very Smart personal hygiene very good 2- Is the server clean shaven / make up free. Very minimal makeup, some permanent but not offensive 3- Is the server wearing any jewellery except watch or wedding band. No 4- Is the server clean in appearance no strong aromas. Very clean with a fresh smell - Does the server walk tall not slouched and happy in appearance. very happy always jolly and the guest react well 6- Does the server have adequate utensils and float. 3

Section 2 First contact (at the desk) 1- Was the guest greeted with a smile, and offered drinks in the bar "Ola table for two, are you coming straight through or you going in the bar" 2- Was the guest taken to there table with chit chat on the way. they talked about all the shopping they had with them 3- was the guest offered a table or told were they are to sit. "Is this table okay for you" 4- were the menus placed in front of the guest drink side facing No just placed in a pile 2 - were drinks offered and suggested. I.e. wine cocktails. "would you guys like some drinks from the bar" No suggestions 2 6- were the guests thanked for there order "I'll be straight over with your drinks" Each question is graded between 1 to 1 being poor and being excellent 24

Section 3 At the table (drinks) 1- Did the server open with "Hola my name is " when the guests were seated "my name is Malissa I'm your server today" 2- Did the server offer and suggest drinks ( if required score ) Drinks offered not suggested 2 3- If appropriate did the server up sell suggest bottles or draft () 4- On returning with drinks did the server suggest starters (by name) (nachos or wings) - On taking mains or did the server suggest (fajitas ect) "what are you guys having for your mains today Fajitas" 6- Did the server suggest side orders (i.e.. Mex rice, refried beans act) "would you guys like any extras any side orders" 7- Did the server repeat back the order 8- Thank you for your order your food will be over as soon as its ready, enjoy your drinks "enjoy your drinks I bring your food over when its ready" Each question is graded between 1 to 1 being poor and being excellent 37

Section 4 (the delivery) Starters 1- Just after the food order were Tortilla Chips delivered to the table multiple tables at once 2- Was the food placed in front of each guest delivered by another member of staff 3- Was it explained that plates are hot 4- Was the guest asked if they have eaten this before - Was all food explained (i.e. this is gauc this is sour ect) 6- Were and extra sauces or condiments offered by Malissa on the check back 7- were drinks needed and offered if so "refill cokes" 8- Final comment (i.e. enjoy your starters everyone and any empties removed) "enjoy your meals" Each question is graded between 1 to 1 being poor and being excellent 4

Section (the check back) 1- did the server check back (everyone enjoying there meals) "is everything okay" 3 2- Were drinks offered if needed and empties removed Offered but not needed 3- Was the table cleared at the appropriate time, all guests finished 4- Was fresh cutlery provided ready for mains No - were drinks offered if needed and empties removed Offered not needed 6- was there any conversation (small talk) "have you been to the movies) "about shopping" 7- If children were present did the server engage conversation with them 8- Final comment (i.e. enjoy your drinks everyone and any empties removed "enjoy your meals" Each question is graded between 1 to 1 being poor and being excellent 33

Section 6 (the delivery) Mains 1- Did the food runner or server say "hola" No was not the main server 2- Was the food placed in front of each guest after it was auctioned 3- Was it explained that plates and skillets are hot 4- Was the guest asked if they have eaten this before no was not the main server - Was all food explained (i.e. this is gauc this is sour ect) no was not the main server 6- Were and extra sauces or condiments offered yes "ketchup vinegar" 7- were drinks needed and offered if so not needed 8- Final comment (i.e. enjoy your starters everyone and any empties removed) Each question is graded between 1 to 1 being poor and being excellent

Section 7 (the check back) Mains 1- did the server check back (everyone enjoying there meals) "everyone enjoying your meals" 2- Were drinks offered if needed and empties removed not needed but empties removed 3- Was the table cleared at the appropriate time, all guests finished yes 4- did the server suggest desserts coffees by name "anyone having any coffees or apple cobbler" - were drinks offered if needed and empties removed 6- Were desserts placed in front of the guest with napkins & spoons 7- Was they a leaving comment (enjoy your desserts) yes "enjoy your puddings" 8- Was there a check back Each question is graded between 1 to 1 being poor and being excellent 4

Section 8 Clearing, payment and departure 1- Was the table cleared of all empties 2- were drinks offered " are you sure you wouldn't like anymore drinks" 3- was the guest asked if they would like the bill and by card No bill was offered 2 4- Was the bill presented with scratch card or offers - was the guest shown the pdq machine (tip) cash payment 6- Was the guest thanked for there payment 7- As the guest left was "adios" used 8- Was the table cleaned in appropriate time and seating computer updated Each question is graded between 1 to 1 being poor and being excellent 37
