ADELPHI UNIVERSITY SCHOOL OF SOCIAL WORK FIELD EDUCATION DEPARTMENT NEW JERSEY AGENCY DESCRIPTIONS This directory of agency descriptions is intended to help graduate students and their faculty field liaisons identify field agency settings for the advanced year of field instruction in the M.S.W. program. PLEASE DO NOT CONTACT AGENCIES DIRECTLY. The agencies described in this booklet do not represent all of the field agencies which will be available during the academic year. In addition, unanticipated changes in agency program or structure may affect the availability of a particular placement or assignment. Please research the agencies online for further information. REMINDER: Students must be available at least one weekday (7 hours) during daytime hours for placement. (Monday-Friday 9am-5pm) Any student wishing to be placed in a school setting will be in placement until the end of the month of June to accommodate the needs of the students in the public or private school setting. If a student or faculty identifies an agency that is not listed in the booklet, please contact the field education office and provide the Assistant Director with the name of the agency. We will gladly research the agency to determine its appropriateness for placement.

Central New Jersey Agencies Advanced Behavioral Care Services (The Center) The Center is a drug/alcohol rehabilitation center with a primary focus on mental health. The facility provides substance abuse treatment services to the public. The treatment center provides partial hospitalization care. There are special groups and programs for persons with co-occurring mental and substance abuse disorders. No special language services are available. Arista Care at Whiting A provider of sub acute, post acute and long term care. B'nos Brocha Small private school located in Lakewood, NJ. Grades K-3. B'nos Yaakov Bais Yaakov Bnos Rivka Elementary School is located in Lakewood, NJ. It is a private school that serves 295 students in grades PK, K-5. Bais Yaakov Bnos Rivka Elementary School is all-female and is Jewish in orientation. Brainbuilders A Therapeutic Intervention Agency providing services to both individuals with developmental disabilities and their families. We work in the home, educational facility, private institution, or place of choice. Our highly experienced staff consists of Doctoral and Masters level supervisors. BCBA s specializing in the principles of applied behavior analysis; licensed occupational therapists and speech pathologists; social workers; and certified teachers trained in floor time, SPD, LPD, and more. Our goal is to provide exceptional services by developing unique and individualized programs for each child. Brainbuilders offers an array of therapeutic interventions, specializing in home services. Chai Lifeline Since 1987, Chai Lifeline s mission has been to restore the light of childhood to children whose innocence ended when life-threatening or lifelong illness was diagnosed. Through programs that address the emotional, social, and financial needs of seriously ill children, their families, and communities, Chai Lifeline restores normalcy to family life, and better enables families to withstand the crises and challenges of serious pediatric illness Concord Health & Rehabilitation Center Concord Healthcare & Rehabilitation Center is a modern state-of-the-art facility offering multi-faceted healthcare services. Concord has gained the acclaim of the medical community for providing the highest level of care and rehabilitation available. If your physician, social worker or case manager says you may require sub-acute care, long term care, rehab, nursing care or if you are in need of respite or hospice care, Concord has the expertise to provide you with the care required. CPC Behavioral Health Care Multiple sites Each year CPC Behavioral Healthcare gives hope to over 6,000 children, teens, adults, and families living with depression, mental illness, substance abuse or disabilities that impact learning. Our counseling centers and special education schools give hope for recovery and the opportunity to live happier more productive lives.

Dottie s House Dottie's House is a transitional housing facility designed to aid women and children who have survived domestic violence. Advanced Year students preferred. Hillel Yeshiva: A school based program. Institute for Family Services in Somerset Advanced year Students only The Institute is highly acclaimed throughout New Jersey and the nation for its innovative programs that intervene with multiple family members concerning issues of empowerment and accountability creating rich connections for families. The IFS faculty teaches their expanded model of family therapy to postgraduates and offers training in the Cultural Context Model at the state, national and international level. Jersey Shore Med. Ctr. Hospital Social Work Discharge Planning, Group work Jewish Family & Children's Services Comprehensive Mental Health and Social Services Lakewood Community Service Corp A community and social service agency that provides a large array of services in Lakewood, NJ. Lakewood is a diverse suburban community. Services for the Elderly: Meals-on-Wheels, Caregiver Support, and Educational, Recreation and Socialization programs. Youth Programs: After School Mentoring, Youth Groups, Programs for Children with Disabilities. Counseling and Case Management: Ongoing Counseling and Case Managemnt by Licensed Social Workers; Information and Refferals, Entitlement Assistance, Advocacy and Crises Intervention. Economic Development: Urban Enterprise Zone programs, Microloans, Job Placement. Bi-lingual Services: The Largest ESL Program in Ocean County and the Hispanic Task Force. Community Building: Building Bridges between the Diverse Communities of Lakewood. Leisure Chateau Short and long term rehabilitation, 24-hour Skilled Nursing Care On-staff Clinical Psychotherapist, Hospice Care Services, Respite Care, Interdisciplinary Care Planning, Traumatic Brain Injury Care. Lev Rochel Bikur Cholim Volunteer driven non-profit organization providing many services to residents of the Passaic-Clifton Jewish community including: meals, transportation, respite services, weekly visitation, Shabbos hospital accommodations, medical equipment, community blood bank and financial aid for medical expenses. Long Branch Concordance Multi Service Family Agency.

Madraigos Madraigos provides a wide array of innovative programs, services and resources which have been specifically designed to help teens and young adults to cope with everyday challenges of life. Many of the children have social, emotional, academic or family issues. They work tirelessly not only to provide all of their members with the tools and skills to enable them to lead a healthy lifestyle, but to address the root of the problem and deal with the underlying issues. Services include: community awareness, 24 hour support line, evaluations and case management, evening recreation center, in-school drug testing and prevention and support groups. Monmouth Family Health Center *Advanced Year only Monmouth Family Health Center is a Federally Qualified Community Health Center with a mission of providing economically disadvantaged individuals in Long Branch and surrounding communities with high quality, comprehensive, affordable, culturally sensitive and linguistically appropriate primary and preventive health care. Ocean Mental Health @ Bayville: Outpatient Counseling, Day Treatment Services for Adults and Youth, Child and Adolescent Services, Family/Caregiver Support, Residential Services, Therapeutic Foster Care, Outreach Services, Ocean Academy, a Therapeutic School. O.C.E.A.N., Inc. A community action agency for the residents of Ocean County. Combats poverty throughout the county by identifying and overcoming barriers to self-sufficiency. Ocean Partnership for Children Ocean Partnership for Children, Inc. is a private, non-profit community based organization serving children primarily between the ages of 4-18 with behavioral and emotional challenges along with their families. Provides care management and service planning for children with complex needs. Oceans Harbor House Ocean's Harbor House is a safe haven for youth, ages 10-21 who have run away, are homeless or have been abused, neglected or abandoned. Pinebrook Nursing Home A 132 bed nursing and rehabilitation facility, offers various specialized services for the geriatric population. Pine Brook provides a full range of services to its residents and family members. With our staff of professionals, we provide skilled nursing care and sub acute rehabilitation. Specially designed programs such as respite and hospice care are also available. Providence House-- Catholic Charities *Must have experience Provides comprehensive services for victims of domestic abuse and their children free of charge at outreach offices and emergency safehouses in Burlington and Ocean Counties. Providence House Domestic Violence Services assists victims of domestic abuse and their children with an array of services that include: emergency shelter, a 24 hour hotline, counseling services, a comprehensive children s program, a parenting education program and a program specifically designed to assist victims age 50 and older. These services are offered at no cost to individuals and are invaluable to victims and their children who are struggling to break free from the cycle of violence in their homes.

The School for Children with Hidden Intelligence The School for Children with Hidden Intelligence (SCHI) is a non-profit, fully accredited school that provides special education for the multiply-handicapped, mentally challenged, and physically disabled children of the Tri-State Metropolitan area who have challenges as varied as autism, Down syndrome, and cerebral palsy. Founded in 1994, it has succeeded in establishing itself as a progressive educational institution with an outstanding reputation. Sephardic Bikur Holim Sephardic Bikur Holim is a non-profit mental health and social service agency dedicated to caring for people in need with compassion, discretion, and respect. They provide for the health, sustenance, support, and well-being of children and adults, the infirm and the disadvantaged through an everexpanding spectrum of services, programs and resources. They work hand-in-hand with a network of devoted volunteers and professionals to address all needs, while maintaining a strong commitment to excellence. Their goal is to invest families and individuals with strength and self-esteem so that they can rise with dignity to their utmost potential. Tikvah at Ohel Tikvah at Ohel serves members of the community who have emotional, family, or educational problems. Their professional staff has training and expertise to assist individuals experiencing depression or anxiety; families suffering from marital strife; and children who may have behavior problems or difficulty adjusting in school. Their experienced professionals are sensitive to various cultural needs of the community. Trinitas Hospital Established in January, 2000 following the consolidation of St. Elizabeth Hospital and Elizabeth General Medical Center, Trinitas Regional Medical Center is a full-service teaching hospital serving those who live and work in Central New Jersey. Services include: behavioral health, cancer care, maternal and child health, etc. West Bergen Mental Healthcare West Bergen Mental Healthcare was founded as a child guidance clinic in 1963. Over the years, the Center has expanded to provide comprehensive mental health services in over a dozen specialties including: outpatient services, psychiatric services, partial care program, residential program, community services, SOAR Experience. Yachad Yachad provides individuals with developmental disabilities an opportunity to socialize and to be included within the Jewish community. New Jersey Yachad offers social and recreational programs for children and young adults with special needs on a monthly basis. NJ Yachad is proud to offer many ongoing support groups for parents and siblings who have a loved one with special needs. Yad Rachel Yad Rachel is an association with the mission to give children from poor and distressed homes the gift of learning. It operates after-school educational frameworks for young children (aged 5-10) who suffer from learning difficulties due to cognitive, behavioral and social factors. At the Yad Rachel centers, the multi-disciplinary staff deals mainly with school-related problems in order to help the children overcome their disabilities and realize their potential.

Yedei Chesed Yedei Chesed was founded in 1992 to advocate for the developmentally disabled of the community. Their commitment is to provide these individuals the opportunity of an improved quality of life and the potential for independence, inclusion, individualization, and productivity. Yedei Chesed advocates for the rights, choices and opportunities of people with developmental disabilities and assists them in attaining their rightfully deserved place in society Brooklyn Beth Jacob of Boro Park Beth Jacob of Boro Park is a private school in Brooklyn, New York. It is all female and Jewish affiliated, serving 1693 students in grades PK-8. Beth Jacob Day School Beth Jacob Day School is a private school in Brooklyn, New York. It is all female and Jewish affiliated, serving 784 students in grades PK-12. Guardians of the Sick Ezer Cholim Project Programs for at-risk youth and families in crisis, substance abuse counseling serving several areas of Brooklyn. Ho'ar Beacon School A Orthodox day school located on Nostrand Avenue in Brooklyn, established in 2007. Servicing grades 5-12, Ho ar Beacon is a self-contained program for boys with special needs. Children with social and/or learning difficulties, provides small classes and individualized instruction. Project YES A teen mentoring program based in Brooklyn. Its aim is prevention: intervention before a potential problem becomes a large, insurmountable issue. Yeled V Yalda Yeled V Yalda is a dynamic not-for-profit social service agency whose mission is to serve the whole child and the family. The agency provides the New York City area with a variety of educational programs and social services, offering a seamless blend of child care and developmental services, as well as health and nutritional guidance. They are ready to embrace new ideas and approaches while responding to the ever-changing needs of children and families. Rockland County Areivim Areivim s mission is to facilitate the healthy growth of the family unit. Areivim s focus is on teenagers and their parents. Areivim offers teenagers, their parents and their siblings, coping skills, educational resources and mentoring services, in order to help the teenagers succeed within the family and educational systems. Areivim believes that parents who cannot cope need more than a therapist; they need a third parent. They need a person who is as caring and involved as the parents themselves, but must be objective and experienced. Areivim is aware that in order to be successful, dozens and sometimes hundreds of hours must be dedicated to each family in crisis. As a parent, it realizes that there s no such a thing as too much time invested in one s children. Kiryas Joel

The officials of Kiryas Joel have presided over an experiment that can only be described as a miracle. Survivors of the Holocaust, their children and grandchildren have rebuilt a modern-day community in the spirit of the communities which were consumed in the European inferno more than a half century ago, while at the same time taking full advantage of the many opportunities this country offers. The residents of Kiryas Joel have benefited from the guarantees inherent in our Constitution as they raise their families with age-old traditional values. The function of the Village of Kiryas Joel has been to provide the growing census of residents with the essential modern services any local jurisdiction is expected to provide.
