Welcome to Silver Chain Supporting the health and wellness of the community for over 100 years Silver Chain is a not-for-profit organisation and one of the largest providers of community, clinical and health care services in Australia, assisting over 60,000 people each year. We provide client centred care to individuals, eligible health fund members, corporate organisations and veterans in the community. Clients have been relying on the services of Silver Chain for professional care for more than 100 years. We are experts in providing quality home and community nursing, allied health and home care to people of all ages, with the aim of living independently for longer and maximising their health and wellness. Our clients range from newborn babies right through to our most senior individuals, which means our flexibility and experience caters for all needs in the community. With an ageing population requiring diverse services in health and wellbeing, Silver Chain is well placed to provide the best care when and where our clients need it. Our leadership in health services and education and training, allows us to provide key services to both Federal and State Governments and allied professionals as well as direct to our clients. This ensures that our networks and partnerships are extensive enough to provide a full service to our clients in every area of health. We are proud of our history, innovation and leading services. Our merger with RDNS SA (YourLife in Queensland), has made us one of the largest in-home health and care providers in Australia and we are committed to supporting the health and wellness of the wider community. 3

Our Services Silver Chain provides a 24 hour, 7 day a week service for our clients Our aim is to improve lifestyle choices and support individuals to continue to live independently through the extensive and diverse services we provide. Some of these services include: Specialist Nursing Services Wound care, chronic disease, medication, continence, diabetes, hospital in the home, disability support and dementia care management, as well as palliative care for those at the end stage of a terminal illness. Home Care Services We can provide assistance with an individual s domestic and personal care needs. These may include meal preparation, laundry, showering, house cleaning, shopping, garden maintenance and transport, and can be tailored to suit the client s needs. Home Therapy Services We provide in-home rehabilitation and restorative care to enable people with more complex needs, to remain at home. Our team of specialist health professionals ensure that client s reach their optimum level of health and wellbeing with their tailored treatment program. From Occupational Therapists, Physiotherapists, Podiatrists and Social Workers, right through to Speech Pathologists, Psychologists and Dieticians, Silver Chain can package your treatment to suit you. Our service includes a comprehensive rehabilitation program that may involve assessments, equipment and exercise programs, developed in partnership with the client s GP or specialist. 5

Education and Training Research Unit Fundraising Our Education Centre is a National Registered Training Organisation (RTO), offering nationally accredited courses such as Certificate III in Aged Care and the Diploma in Nursing for Enrolled Nurses. We have been delivering this education service for over 10 years, contributing to the specialist skills required by our health and care workers in our community. Approximately 400 students enrol in RTO status courses annually, with an average course completion rate of 95%. Around 2500 people receive health care training and qualifications in our Education Centre each year. The Centre s courses are offered using a range of delivery methods, including traditional, e-learning, distance education and self-paced learning. Our courses are highly sought after, as we provide credit recognition towards higher education courses in nursing, with Universities across Australia. Our education centre provides specialist consultancies and professional development for health and community care organisations and has state, national and international partnerships for education services. Our Research Unit is internationally renowned for its findings into the management of chronic illness. The Research Centre enjoys strong relationships with Universities nationally and internationally, other health and community care organisations and corporate businesses. Our research focus is always on improving outcomes for clients providing real value to the future of health and care services. The Silver Chain multidisciplinary researchers have published more than 100 journal articles, 7 books and undertaken more than 200 conference presentations nationally and internationally. Fundraising has always been important to Silver Chain. For over 100 years we have raised funds to provide additional care to those who need it most and it is only with the help of the community that we are able to make a difference. Donations to Silver Chain help us provide life changing care within the community including the provision of essential medical supplies and equipment for those that are unable to purchase items for themselves and access to nursing clinics to assist people struggling with homelessness.
