Dear Memorial Community: Welcome to the -16 school year! I hope your summer has gone well and I hope you all have had a chance to find time to relax. We look forward to this being another successful year. In this packet you will find directions and information for enrollment at Memorial High School. We ask that you take the time to read this so you and your student will know what documents and fees are required when you enroll. Incoming Freshmen and new students will become part of a Neighborhood as they enter Memorial, with other students across grades 9 through 12. Assistant Principals and Student Services staff are assigned to Neighborhoods, not grade levels. These individuals will be available during enrollment for any questions you might have. We will do our best to make enrollment run as smoothly as possible. Please understand that there could be unforeseen circumstances that result in backups during the enrollment process, so please plan accordingly in terms of the time you are setting aside for this task. Please also know that Tuesday, September 1 will be for 9th grade ONLY. The day will begin at 8:14 am with a meeting in the Buchhauser Auditorium. Lunch will be provided for the 9th grade students with the day ending at 3:36. All new students to Memorial in grades 10-12 will meet with Assistant Principal Radloff in the Buchhauser Auditorium on Tuesday, September 1st from 1:30-3:36 p.m. Classes for all students will begin on Wednesday, September 2 from 8:14 a.m.-3:36pm, which is our regular schedule. STUDENT ENROLLMENT JAMES MADISON MEMORIAL HIGH SCHOOL ENROLLMENT FEE AND TIME SCHEDULES - Students are able to enroll on either Tuesday, August 18 or Friday, August 21. Families can enroll all students on the same day. Students planning to be absent for any reason during enrollment may enroll by proxy (a friend or family member) or may enroll on the first day of school. The schedule for enrollment is: Friday, August 14 Students who have not enrolled and selected courses. Call Sherry Zander at 663-6054 for an appointment. Tuesday, August 18 All Grades 11:00 a.m. - 6:30 p.m. Friday, August 21 All Grades 7:30 a.m. - 1:00 p.m. The lines are always longest at earliest enrollment times, so plan accordingly. The school nurse will be available during enrollment or call 663-6071. The school cafeteria staff will also be available. Online Enrollment for the -16 School Year Get a jump on the new school year and save time on enrollment day. Begin the enrollment process online at home or at one of several designated neighborhood sites. At Home- Log in at www.mmsd.org/enroll. The website will be open on August 4 th to begin your -16 enrollment update. Complete the enrollment steps, print off your verification form and bring it with you when you come to Memorial on August 18 th or 21 st. In your Neighborhood - Several neighborhood sites will offer pre-enrollment days, providing access to a computer and people to help you with the online enrollment process. Then, be sure to bring your enrollment verification form when you come to Memorial on August 18 th or 21 st. Pre-enrollment days as of printing. If you don t know or remember your Infinite Campus user name or password, make sure to use the e-mail form at mmsd.org/enroll so you have it before going to school. MMSD now offers an online payment option. You may pay your fees with a credit card as part of the enrollment process. Be sure to bring the receipt with you on August 18 th or 21 st. 2

SCHEDULE CHANGES WILL BE MADE ONLY FOR THE FOLLOWING REASONS: Errors made by the school, Simple drop when no other changes are needed and the student does not fall below a minimum class load, Teacher-recommended level changes, Fulfilling graduation requirements for seniors, Students who have previously failed a course with a specific teacher may request not to study with that teacher again: however, no specific teacher may be requested, and Changes necessitated by enrollment in alternative programs. SCHEDULE CHANGE PROCEDURE Please note that schedule changes can only be made on the day of enrollment. You may request changes according to the reasons listed above. Requests must be made in person. Once made, changes are final. If you decide to have someone else pick up your schedule for you, and that person requests that allowable changes be made, those changes too are final. Parents and students may contact their Neighborhood Principal if they feel that an unusual set of circumstances might warrant an appeal of the above guidelines. FEE SCHEDULE Parents/Students must bring the correct cash amount or a check to cover fees, optional athletic card, optional yearbook and any outstanding obligations from the previous year. Checks should be made payable to James Madison Memorial High School and must be signed by a parent or guardian. On-line payment of fees is available this year. Information will be in Infinite Campus. Obligations from the previous school year must be taken care of when the student enrolls. These cannot be carried into the -16 school year. Parking permits for seniors/juniors will be sold to seniors/juniors on a first-come, first-serve basis during enrollment on Tuesday, August 18 th between 11:00 a.m.-6:30 p.m. or Friday, August 21 st between 7:30 a.m.- 1:00 p.m. To be eligible to purchase a parking permit, students must have and be able to produce a valid driver s license and provide information regarding the car, including make/model and license plate number. Permits will NOT be sold to students who do not have a valid driver s license. The FEE for parking permits will be $40. Only one permit will be sold per family. Because the demand for parking at Memorial is significant, parking regulations will be strictly enforced at all times. Parking tickets are issued for rule infractions. Once given, tickets cannot be revoked. FEE WAIVER POLICY AND PROCEDURE To encourage all students to participate in all school activities, regardless of the family s financial situation, it is the policy of the Board of Education to waive the payment of part or all of student fees if the student or the student s parent/guardian demonstrates an inability to pay such fees, with the exception of the optional fees listed above. Principals will be available at enrollment for parents wishing to request a fee waiver. At any other times you should contact the Rock Neighborhood Principal, Lorie Wesolek. She will determine ability to pay the fee by considering factors that include, but are not limited to, student eligibility for Free and Reduced Lunch or AFDC. SCHEDULE FOR FIRST DAY OF SCHOOL, TUESDAY, SEPTEMBER 1 8:14 a.m.-3:36 p.m. 9th grade students report to the Buchhauser Auditorium. There will be morning and after school bus service. A free picnic lunch will be provided. 1:30 p.m.-3:36 p.m. New to Memorial 10th, 11th, 12th grade students report to the Buchhauser Auditorium. WEDNESDAY, SEPTEMBER 2 8:14 a.m.-3:36 p.m. For all students School locker assignments will be made on the 2 nd of September. Memorial High School has purchased combination locks for all hallway lockers. Parents will need to supply locks for physical education classes only. When your child is assigned a locker by his/her 4th hour teacher, they will be given a combination to the lock as well. We are doing this primarily for safety reasons, but there are other benefits. The school will have a copy of the combination for each lock and a key to open lockers in emergencies. We will also have a record of all lockers assigned to students. If for any reason your child is unable to use a combination lock, please contact Mr. Radloff at 663-6048. Advisory attendance is required, and parents will be notified of non-attendance. STUDENT BUS INFORMATION Bus schedules for the special routes will be provided by METRO TRANSIT and should be available at school during enrollment. EZ RIDER bus passes will offer a semester of unlimited rides, including weekends, as of printing for $150. These passes should be purchased during enrollment or early in the semester to take full advantage of savings. EZ RIDER PASSES NOW HAVE MAGNETIC STRIPS THAT ARE READ BY THE FARE BOXES. STUDENTS SHOULD SLIDE THE PASS EACH TIME THEY BOARD AND SHOW THE DRIVER THEIR PHOTO ID CARD UPON REQUEST. If the semester pass is lost or stolen, the pass-holder will be required to pay $10.00 for a replacement pass. Replacement passes will be issued only at Metro's Administrative Office, 1245 E. Washington Avenue, Suite 201, Monday-Friday from 7:30 a.m.-5:00 p.m. Passes will not be sold at prorated cost during the semester, and Metro will not make refunds to people who move out of its service area during the semester. Students may also purchase 10-RIDE CARDS for a cost of $10.00. Individual rides paid in cash will cost $1.25. IMPORTANT Picture Information: ID and yearbook pictures will be taken by Lifetouch during enrollment week. YOUTH OPTIONS The Youth Options Program (formerly PSEO) was established in 1992 to allow MMSD 11 th and 12 th - grade students to enroll at an institution of higher education in Wisconsin to take courses that lead to credit granted toward high school graduation. Students will be limited to earning a total of 18 postsecondary credits during their high school career at any institution of higher education or technical college. The Madison Metropolitan School District is required to pay for a course taken by a high school student at a post-secondary institute if a comparable course is not offered anywhere in the school district (including online courses). If you wish to participate in the Youth Options program during the second semester of the - school year or the first semester of the -2017 school year, please: 1. Discuss your plans with your school counselor, and 2. Complete a Youth Options Program Plan and Report application form and mail it tojen Wegner, Career and Technical Education Coordinator at 545 W. Dayton Street, Madison, WI 53703, to be postmarked by October 1,, for the second semester of -; or to be postmarked by March 1,, for the first semester of -2017. Youth Options Program Plan and Report applications are available in your school guidance office along with envelopes addressed to the Career and Technical Education Coordinator. Students admitted to the University of Wisconsin under this program are encouraged to take only one course per semester. (Exceptions may be granted by the University.) You should select one or more alternative courses in the event your first choice is not available. Students must be in good academic standing to qualify for Youth Options. Your school counselor can be helpful to you in making appropriate course selection decisions and in providing information on the application procedures. If you have any questions, please contact your high school counselor. CREDIT FOR NON-MMSD COURSES In some cases high school credit will be granted for pre-approved educational experiences undertaken outside the MMSD. In ALL cases, the student must receive prior approval from both their Neighborhood Principal and the chair of the Memorial department to which the non- MMSD educational experience is related. Students who do not secure pre-approval may request to have courses shown on their transcript, but no credit will be granted. There is also specific information regarding fall sports at the end. We look forward to having another successful year with our student athletes. Please make sure to complete all requirements prior to the season to insure full participation opportunities. We do have a few sports that have limited spots so it is important that student-athletes are prepared to start on the first day and do not miss time in-season. 1. The Spartan Sports Kick-off meeting will be July 30 th, 5-7:00pm, at Mansfield Stadium and Memorial High School. At this time we will go through the required forms for participating in the -16 school year and meet with coaches for sport specific information. The meeting will cover the Athletic Code as well as student-athlete and parent responsibilities to the Code, school, and team. Failure to attend this meeting will require a make-up meeting before athletic eligibility is granted. All Memorial students who wish to participate in a WIAA sport during the -16 school year must attend with a parent. 2. Everyone who participates in interscholastic athletics, cheerleading, or pom pon must have a physical examination the first year out and then on alternating years thereafter. This examination must be attested to with the signature of a licensed physician or nurse practitioner or physician s assistant on the green WIAA physical examination card. Physicals should be obtained as soon as possible, BUT MAY NOT BE TAKEN BEFORE APRIL 1 for any upcoming school year. The reverse side of the green physical card is the parental permission form, which must be signed by parents to allow athletic participation for their children. On alternate years a yellow card is all that is required. As has been indicated above, all cards require more than one signature and cannot be accepted with less than the required number. You must also indicate the county and state in which you were born very important for eligibility. Properly completed cards must be turned in to your coach at the July 30 th meeting. The green WIAA card is for freshman should have been picked up from the Athletic Director at the eighth grade information meetings in the spring. However, they may be obtained from the main office during summer office hours of 7:30 am-3:30 pm. 3. It is also required that you remit an athletic participation fee for EACH sport, which is paid after final rosters have been determined and before the first date of competition. As of this printing, fees will be $115 for all sports with the following surcharges: Golf - $118.00; Gymnastics - $100.00; Wresting - $100.00; and Hockey - $800.00. There is a family cap of $500.00 not including surcharges. No refunds after the first date of competition. Memorial Athletic Online Information Eligibility forms: All required forms for athletic participation are now available on the web. You can access the site at https://webapp1.madison.k12.wi.us/webapp1/athletic_forms/ or through the link on JMM s website in the Athletics section. Parents and students will both login using their Infinite Campus User Name and password. There is a separate process for both students and parents, please make sure that both complete. August Dear Parent or Guardian, A screening questionnaire developed by Columbia University called the Columbia Depression Scale will be given to students enrolled in Health classes as part of the mental health unit. This questionnaire screens students for probable emotional health concerns, particularly depression. The Health class is usually taken during the 10 th grade year. The Columbia Depression Scale is a 22-item paper and pencil questionnaire which takes about 10 15 minutes to complete. If the answers to the screen indicate a probable emotional health concern, your child will meet with a Memorial support staff member. Go to School Night: A fun evening where parents meet teachers, learn about classes and take a tour of the school Volunteers needed to help organize, set up, and work at event 2. Office Help, Library Help: Volunteers needed throughout the year to help with office work, library work, and Culinary Arts (Shoppers, Lab Assistants, and Special Events Support) Volunteers needed to help throughout the school year 3. Tutor Center: Parent volunteers to help tutor students in the Tutor Center. Help especially needed in Math, Science and English Tutors needed before and after school as well as during the school day 4. 9 th Grade Immigration/Migration Project: Annual 9 th grade history project and open house Volunteers needed to help organize, set up, and work this fun event. 5. Monthly Treats for Teachers: Each month, volunteers sign up to bring treats for the JMM staff. Many volunteers will be needed each month to bring treats 6.
