RN to BSN Interest Survey The Institute of Medicine and Future of Nursing reports have a target goal of 80% of the RN workforce prepared at the BSN or higher level by 2020. Bellin Health and Bellin College are working together to develop RN BSN curriculum to advance the educational preparation of Bellin Health nurses. Your response to this survey will help in that planning process. The survey is short (10 1 minutes). Your assistance is greatly appreciated! * 1. Current Level of Nursing Preparation: ADN or Diploma without a prior bachelor's degree ADN or Diploma with a prior bachelor's degree BSN MSN DNP, PhD, DNSc

2. If you have a prior bachelor's degree, identify the field it is in (ie biology, business, English, etc.).

3. Where did you receive your ADN or Diploma degree? Wisconsin state technical college Other (Please specify) Other (please specify) 4. If you took college credits toward a bachelor's degree, but did not complete the degree, please identify how many general education credits you have that may transfer towards your BSN degree.. When did you complete your ADN or Diploma degree? Zero to years ago (2008 2013) 10 years ago (2003 2007) More than 10 years ago (prior to 2003). What is your current working status? Full time Part time

7. How strong is your interest in pursuing a BSN degree? High I am already enrolled in a program High I am actively looking at program options High I have not started looking but would like to get more information Moderate I have thought about it and may be interested within the next 2 years Moderate I have thought about it and may consider in the future (beyond 2 years) Low I am not interested in pursuing my BSN I am not interested in the RN BSN, but would be interested in a RN MSN program

8. If your interest is low, please identify why you are not interested in pursuing your BSN degree. (select all that apply) Cost Do not see a value for a BSN degree Do not see the need for a BSN degree Family responsibilities Lack of family support No incentive to pursue Retiring soon Time commitment Work schedule Other (please specify)

9. If your interest level is high or moderate, would you be interested in starting a program in: Spring 201 (January start) Summer 201 (End of May start) Fall 201 (August start) Not interested in starting in 201 Other (please specify) 10. What type of class delivery method do you prefer: (select all that apply) Traditional classroom setting at Bellin Hospital Traditional classroom setting at Bellin College On line classes Hybrid with some traditional classroom time and some online component It doesn't matter 11. What time of day do you prefer for classroom based classes? Weekdays Evenings Weekends

12. A BSN degree requires an additional 30 nursing credits (10 three credit classes) beyond the ADN or diploma program plus a variety of general education courses. Students with a prior baccalaureate degree may have all the required general education classes completed. Others may require a mix of nursing and general education courses. A 3 credit course requires 4 hours of class time (either on line or classroom setting). This does not include study or preparation time. In considering a curriculum schedule using just the 10 nursing courses as an example, would you prefer: One course every five weeks (9 hours of class time per week & would take approximately 1 months to complete the nursing courses) One course every eight weeks ( hours of class time per week & would take approximately 24 months to complete the nursing courses) Two courses every eight weeks (11 12 hours of class time per week & would take approximately 12 months to complete the nursing courses) Two courses a semester 1 weeks ( hours of class time per week & would take approximately 30 months to complete the nursing courses) Three courses a semester 1 weeks (9 hours of class time per week and would take approximately 18 months to complete the nursing courses) 13. Please type in questions or concerns you may have about a RN BSN program offering between Bellin Health and Bellin College. 14. Optional Information: Name: Work Unit:
