What is Frm Frm 1095-A is a tax frm that will be sent t cnsumers that have been enrlled in health insurance thrugh the Marketplace in the past year. Just like yu get a W-2 frm yur emplyer, yu ll be getting a frm frm the Marketplace frm 1095-A yu ll need it fr yur taxes. Keep it alngside yur W-2 and ther tax recrds. Similar t hw husehlds receive multiple W-2s if individuals have multiple jbs, sme husehlds will get multiple Frm 1095-As if they were cvered under different plans r changed plans during the year. 2. What is the purpse f Frm If anyne in yur husehld enrlled in a health plan thrugh the Health Insurance Marketplace in 2014, yu ll get a Frm 1095-A Health Insurance Marketplace Statement. Yu'll get it in the mail by early February. Use it t file yur 2014 federal incme tax return. Cnsumers receive Frm 1095-A because they r their family member(s) enrlled in health insurance cverage thrugh the Health Insurance Marketplace (Marketplace). Frm 1095-A prvides infrmatin that cnsumers need t cmplete Frm 8962, Premium Tax Credit (PTC). The Marketplace has als reprted this infrmatin t the IRS. 3. What d I d with Frm Cnsumers use the infrmatin included n Frm 1095-A t cmplete Frm 8962, Premium Tax Credit (PTC). Cnsumers file Frm 8962 with their tax returns if they want t claim the premium tax credit r if they received premium assistance thrugh advance credit payments made t their insurance prvider (whether r nt they therwise are required t file a tax return) (APTC) 4. What infrmatin is n Frm Frm 1095-A includes: Infrmatin abut anyne in cnsumers husehlds wh enrlled in a health plan thrugh the Health Insurance Marketplace fr 2014 Infrmatin abut the mnthly premiums cnsumers paid t their health plans The amunt f any advance payments f the premium tax credit that were paid t cnsumers health plans in 2014. These are the payments that lwered what cnsumers paid fr their mnthly premiums. The cst f a "benchmark" premium used t determine the amunt f cnsumers premium tax credit.

Wh is included n Frm 1095- A? 7. When will the FFM Frm 1095-A be available? All husehlds wh were enrlled in a Marketplace QHP, regardless f whether members f the husehld received financial assistance, will receive Frm 1095-A. Each member f a tax husehld, wh is n the same plicy, will be listed tgether n ne Frm 1095-A. The Marketplace will nt prvide Frm 1095-As t cnsumers wh were enrlled in nly a catastrphic cverage r a stand-alne dental plan because they d nt qualify fr the Premium Tax Credit. Frm 1095-A includes all members f a tax husehld wh enrlled in a qualified health plan (QHP) Members f the husehld will be included n the frm regardless f whether they received financial assistance (i.e., APTC). A separate Frm 1095-A is generated fr each plicy in which the husehld enrlled. The Marketplace will nt prvide Frm 1095-A t cnsumers wh were enrlled in nly a catastrphic cverage r a stand-alne dental plan because they d nt qualify fr the Premium Tax Credit.. All Frms 1095-A will be pstmarked by February 2, 2015. The FFM will print and mail Frm 1095-As fr cverage year 2014 t recipients and uplad frms t their nline HealthCare.gv accunts (if the cnsumer has ne). Cnsumers Frms 1095-A will be sent t the address they prvided the Marketplace and psted t their nline accunts. FAQS ABOUT RECEIPT OF FORM 1095-A 8. Will Frm 1095-A be available in languages ther than English? Currently Frm 1095-A will nly be available in English; hwever, the envelpe and the cver letter accmpanying Frm 1095-A will be available in English and Spanish Each Frm 1095-A will cntain a ntice tagline addendum that prvides instructins in 15 additinal languages fr cnsumers t call 1-800-318-2596 if they need assistance interpreting r understanding Frm 1095-A

9. What if I need help understanding Frm 10. Will I get a separate Frm 1095-A fr each plicy I enrlled in? 11. Why did I receive multiple Frms Fr mre infrmatin abut hw yur Marketplace cverage will affect yur taxes, visit HealthCare.gv/taxes/ r call the Marketplace Call Center at 1-800-318-2596 (TTY: 1-855-889-4325) Cnsumers can get free assistance with filling ut their taxes. This may include free access t tax sftware prgrams, r free in-persn assistance. Fr mre infrmatin, visit IRS.gv/freefile r IRS.gv/VITA. If yu have additinal questins abut yur taxes, need Frm 8962, r want t learn mre abut the fee fr nt having health cverage, visit www.irs.gv/aca. If cnsumers r their family members are enrlled in mre than ne QHP plicy thrugh the Marketplace, they will receive a Frm 1095-A fr each plicy. A separate Frm 1095-A will be generated fr each plicy in which the husehld is enrlled If cnsumers were enrlled in a health plan, reprted a life change, and then were granted a special enrllment perid (SEP), they will receive a Frm 1095-A fr the perid f enrllment prir t reprting the life change and a separate Frm 1095-A fr the perid f enrllment after reprting the life change (regardless f whether they change QHPs r stay enrlled in the same QHP). The Marketplace will nt prvide Frm 1095-A t cnsumers wh were enrlled in nly a catastrphic cverage r a stand-alne dental plan (SADP) because thse enrlled in these types f plicies d nt qualify fr the Premium Tax Credit. If cnsumers r their family members were enrlled in bth a QHP and a SADP the Marketplace will adjust the Mnthly Premium Amunts and Mnthly APTC amunts, if applicable, n their Frm 1095-A accrdingly (see questins belw abut hw these values are calculated). Just as sme husehlds receive multiple W-2s if individuals have multiple jbs, sme husehlds will get multiple Frm 1095-As if they were cvered under different plans r made changes t their husehld during the year. 12. Hw d I print Frm T print Frm 1095-A, cnsumers shuld: Lg int their nline accunts and navigate t the applicatin-landing page by clicking n the apprpriate applicatin fr the 2014 plan year. Click n the tax frm tab n the left navigatin panel. Cnsumers can view and print their Frm1095-A frm the Tax Frm sectin. Frm 1095-As will be psted t cnsumers nline accunts beginning January 9, 2015, but n later than February 2, 2015. All Frm 1095- As mailed t cnsumers will be pstmarked by February 2, 2015. The Marketplace will nt

13. What if I did nt receive Frm 1095-A frm the FFM? begin generating crrected Frms 1095-A until the end f February 2015. Cnsumers with existing nline accunts can als view the Frm 1095-A in their nline accunt frm the Message sectin n the glbal landing page, which is the same lcatin where ther ntices are psted. If cnsumers d nt have nline accunts, they can create an nline accunt t view and t print their Frm 1095-A. When a new nline accunt is created after Frm 1095-A has been generated, the frm will be available nly in the Tax Frm sectin, and will nt be psted t the Message sectin Cnsumers will have t pass identity prfing t view their Frm 1095-A electrnically. Frm 1095-A will nt be generated fr cnsumers: Enrlled nly in a catastrphic plan, (since they are nt eligible t receive APTC, nr can they claim the PTC n their tax return) Wh receive an exemptin and were nt enrlled in a QHP thrughut the year at any pint Enrlled in a plan utside f the Marketplace Yu will nt get a Frm 1095-A if yu have health cverage frm anther surce, like a jb, a plan yu bught utside the Marketplace, Medicaid, Medicare, r the Children s Health Insurance Prgram (CHIP). If yu enrlled in 2014 cverage thrugh yur state Marketplace instead f HealthCare.gv, yur Frm 1095-A will cme frm yur state Marketplace. If cnsumers were enrlled in a QHP thrugh the Marketplace and d nt receive a Frm 1095-A, hard cpy r electrnically in their nline accunt, then they shuld cntact the Marketplace Call Center. In the prcess f mailing initial Frm 1095-As t cnsumers, there may be situatins when cnsumers mailing addresses change, and they did nt reprt the updated address t the FFM. Additinally, sme Frm 1095-As may nt be printed and mailed if the mailing address the FFM has n file fr the husehld is identified as invalid based n an address validatin the FFM perfrms. This culd becme a prblem if the cnsumer, fr ne reasn r anther, is unable t access the electrnic PDF f their 1095-A, r they d nt have an nline accunt. Mst cnsumers shuld be able t access their Frms 1095-A frm their nline accunts. Cnsumers shuld btain Frm 1095-A frm their nline accunt if pssible. If cnsumers d nt have access t their Frm 1095-A frm in their nline accunt, it can be re-printed and mailed t cnsumers. 14. If I lse my Frm 1095-A, hw d I get anther cpy? Cnsumers shuld access their Frms 1095-A frm their nline accunts in the tax frm sectin.

15. Hw d I get my Frm 1095-A mailed t me at my new address if I recently mved? If cnsumers d nt have nline accunts, they can create ne t view their Frm 1095-A. If cnsumers experience issues when creating their nline accunts r their Frm 1095-A is nt psted in their nline accunts, they shuld cntact the Marketplace Call Center. If cnsumers have nline accunts, they shuld lg int them and navigate t the applicatinlanding page by clicking n the apprpriate applicatin fr the 2014 cverage year. Cnsumers shuld then click n the Tax Frms tab n the left navigatin panel. Cnsumers can view and print their Frm 1095-A frm the Tax Frms sectin. Nte: If the accunt is created after the 1095-A was initially generated, then they will nly be able t view their frm in the Tax Frms tab and nt in the Messages sectin f their accunt. If cnsumers d nt have nline accunts, infrm them that they can create ne t view and print their Frm 1095-A. Cnsumers will have t pass identity prfing t view their Frm 1095-A electrnically. Cnsumers will then have t link their 2014 applicatin t their accunt using the find my applicatin functinality. Nte: If the accunt is created after the 1095-A was initially generated, then they will nly be able t view their frm in the Tax Frms tab and nt in the Messages sectin f their accunt. If cnsumers d nt have nline accunts, d nt want t create accunt, r have accunt creatin issues, they shuld cntact the Marketplace Call Center. If the cnsumer requests fr a Frm 1095- A t be mailed t a separate address frm the ne the FFM has n recrd then a crrected Frm 1095-A will be generated, and mailed t the updated address. The Frm 1095-A PDF will have the crrected check bx marked, and a crrected recrd will als be sent t the IRS. This will ccur even thugh n data elements n the Frm1095-A (PDF and IRS submissin) will have changed (Frm 1095-A cntains nly a permanent address, nt a mailing address) FAQS ABOUT FORM 1095-A CORRECTIONS 16. What if the infrmatin n my Frm 1095-A is incrrect because I didn t pay all f my premiums and my issuer terminated my enrllment? 17. If Frm 1095-A is incrrect, will I receive a crrected If the cnsumer received a grace perid frm their issuer and then was terminated due t nnpayment f premiums, the cnsumer may nt have an accurate terminatin date n their Frm 1095-A. It is expected that the issuer will prvide CMS with the accurate terminatin date and CMS will send the cnsumer a crrected Frm 1095-A in this scenari. The cnsumer s mnthly premium amunts fr the mnths they were enrlled will shw as the full mnthly premium amunt, even thugh the cnsumer may nt have paid the issuer that full amunt. Demgraphic infrmatin that is incrrect n Frm 1095-A can be updated directly by cnsumers when they file their federal incme tax returns.

Hw lng des it take t get a reprinted r crrected Frm 20. Will I receive Frm 1095-A if I enrlled in health cverage thrugh my state Marketplace instead f Healthcare.gv? The FFM will: Research the cnsumer reprted inquiry Update incrrect infrmatin when apprpriate Mail and uplad t a cnsumer s nline accunt a crrected Frm 1095-A Crrected versins f Frm 1095-A will be generated if certain data elements are updated by the FFM. Updates and regeneratin may ccur as a result f a cnsumer request r a data clean up. Cnsumers will als receive a crrected Frm if they request that their Frm be mailed t an updated mailing address, althugh nly the mailing address n the cver page will be updated since there is n mailing address included in the actual Frm 1095-A. This will ccur even thugh n data elements n the Frm1095-A (PDF and IRS submissin) will have changed (Frm 1095-A cntains nly a permanent address, nt a mailing address) When data elements n Frm 1095-A are crrected r cnsumers mailing addresses are updated, the FFM will mail cnsumers a crrected Frm 1095-A and pst the crrected frm t cnsumers nline accunts. The crrected Frm 1095-A PDF will have the crrected check bx marked and an updated recrd will als be sent t the IRS. The crrected Frm 1095-A will nt indicate which data element was crrected. When a crrected Frm 1095-A is generated, cnsumers will als receive an electrnic ntificatin (depending n cmmunicatin preferences) that the crrected Frm 1095-A (including the cver page, Frm 1095-A instructins, and language taglines) has been psted t their nline accunts. Beginning in early March 2015, crrected Frms 1095-A frm the FFM will be mailed and upladed t cnsumers HealthCare.gv accunts. The updated Frm 1095-A will have the crrected check bx marked. CMS will als reprt crrected infrmatin t the IRS. Yes, cnsumers wh enrlled in 2014 cverage thrugh their state Marketplace instead f HealthCare.gv will receive a Frm 1095-A directly frm their state Marketplace. Cnsumers in these states shuld cntact their state Marketplace directly fr infrmatin and questins relating t their Frm 1095-A.
