About Medicine Specialists of Florida Hospital: Medicine Specialists is part of Florida Hospital s Internal Medicine Residency Program. It is a practice of internal medicine physicians specializing in the diagnosis and comprehensive care of adults with chronic illnesses, infectious diseases and complex health issues. The practice consists of 18 physicians, both on-staff faculty and residents, working together to consider all parts of an adult s health. As a teaching facility, we are up-to-date on all the latest, cutting edge medical information, education, techniques, and procedures. You may see a resident physician whom has already graduated from medical school and is here for additional training and experience in a supervised setting. Each resident physician is overseen by an experienced faculty physician, so you essentially receive two opinions. You can expect your physician to be extremely thorough and take the time needed to answer all your questions. Prescription Refills: For your safety and efficiency, prescription refills may be requested only by your pharmacy. Please call your pharmacy to request the refill, and the pharmacy will fax us a refill request. When requesting a NEW prescription, or a change to an existing prescription, please remember that we need your name, date of birth, your phone number, and the pharmacy phone number. Please allow 24 48 hours for your prescription to be approved and filled. After parking, take the elevator to the 3 rd floor of the Medical Plaza in order to reach the North Tower elevator. Please take the North Tower elevator in the Medical Plaza to the 2 nd floor in order to reach the Family Health Center in Suite # 235.

Obtain your medical records from previous physicians: This is particularly important if you have any pre-existing conditions or chronic conditions. Complete your Medical History Form: The form attached below provides Medicine Specialists with you and your family s medical history. It also provides our staff with your current health maintenance, including medication and recent procedures or conditions. (Please feel free to bring the actual medication bottle(s) with you to the appointment).
