1 Welcome to Loch Haven Center for Menstrual Disorders The most common complaint that women have when they seek help from a gynecologist relates to problems with menstruation. Abnormal periods can cause several serious health care issues for women, including anemia, fatigue, embarrassing accidents, difficulty becoming pregnant, blood transfusions, and visits to the doctor or emergency room. They can also disrupt a woman s life, interfering with her normal activities, her favorite pastimes and her intimacy. At the Loch Haven Center for Menstrual Disorders, our gynecological experts are dedicated to helping women who suffer from abnormal menstrual cycles. A specialized service of Loch Haven Ob/GYN, the Center for Menstrual Disorders is a one-stop destination center for the treatment of all women s health care needs. Our experienced physicians are respected leaders in the medical community and have performed thousands of advanced diagnostic and minimally invasive surgical procedures. In addition, they teach resident physicians and medical students, and are accomplished authors of numerous medical articles in esteemed journals and textbooks.

2 The Center for Menstrual Disorders provides an individualized evaluation and treatment plan for each woman, taking into account her preferences about preserving fertility and other sensitive matters. Treatment options may involve medical management, non-surgical solutions or minimally invasive or standard surgery. Conditions We Treat Heavy menstrual cycles Irregular ( skipping around ) cycles Irregular cycles and trying to conceive Heavy or irregular bleeding due to uterine fibroids or uterine polyps Painful period cramps Menopausal bleeding issues Diagnostic Testing We offer our patients one-stop health care by providing as many diagnostic options as possible right in our center, such as: Vaginal sonograms Saline infusion vaginal sonograms Endometrial biopsy Office flexible and rigid hysteroscopy Menopausal bleeding issues Other tests, like MRI testing, are available at many nearby radiology centers. Treatment Options At the Center for Menstrual Disorders we offer many state-of-the-art treatment options customized to the individual. Depending on the patient s condition, one of the following treatments may be recommended: Medication Intrauterine device (IUD) Endometrial ablation Hysteroscopic polyp removal Removal of uterine fibroids (myomectomy) Minimally invasive hysterectomy Referral for uterine fibroid embolization

3 Your First Appointment If you are unable to keep your appointment, please call (407) at least 24 hours in advance to cancel or reschedule. Your appointment is on M T W R F, The physician you will see is Check-in time: Appointment time: To expedite your visit, new patients should arrive 30 minutes prior to your appointment time. This allows us time to process any additional information that may be required. If you are late for your appointment time, it may be necessary for your appointment to be rescheduled, or your appointment may be delayed to accommodate patients who were on time. Please call the office at (407) if you are delayed beyond your control. Please review the list below to prepare for your first appointment: Complete the Patient Medical History form Obtain medical records from previous physicians If your insurance requires an AUTHORIZATION or REFERRAL, please call your primary care physician to obtain one. If an authorization is not obtained, it may be necessary to reschedule your appointment. Please ask to speak to our referral coordinator or to the practice manager if you have any questions about this process. Bring your insurance card and/or your current insurance information to your first appointment. We cannot verify your eligibility or benefits without this information. Prescription Refills For our patients safety, prescription refills may be requested only by a pharmacy. Patients should call their pharmacy to request the refill, and the pharmacy will FAX us a refill request. When requesting a NEW prescription, or a change to an existing prescription, remember that we need the patient s name, date of birth, phone number, and the phone number of the pharmacy. Allow 24 to 48 hours for the prescription to be approved and filled. Due to regulations, we are unable to prescribe narcotic medications, such as pain medications, after hours.

Our building sits on the corner of Princeton and Dade streets. Center for Menstrual Disorders is located inside Loch Haven OB/GYN on the second floor. Free parking is available. If there is no space near the building, the lot across the street is also available to patients.

5 About Our Physicians and Staff All of our physicians are board certified by the American Board of Obstetrics and Gynecology Scott A. Boone, MD Dr. Boone received academic scholarships to Duke University in Durham, North Carolina (Angier B. Duke Award) and to the University of North Carolina Chapel Hill (Morehead Fellowship) for undergraduate studies and medical school respectively. He subsequently trained in family practice at the University of South Carolina, Columbia, South Carolina, and then completed his OB/GYN residency at the University of Florida in Jacksonville. Dr. Boone has been involved in multiple research studies as chief investigator through Southeastern Clinical Research. He is the former chairman of the Florida Hospital system s Department of OB/GYN. Michael L. Cacciatore, MD Dr. Cacciatore received his medical degree at the University of South Florida College of Medicine in Tampa, Florida, where he also completed his residency. In medical school, he was ranked number one in his class, was very active in honor societies, and won awards including the Outstanding Young Men of America award. During his residency, Dr. Cacciatore also won several awards for teaching. Prior to joining Loch Haven OB/GYN Group in 2006, Dr. Cacciatore ran a private OB/GYN practice in Georgia for 10 years. He has been active in research during his practice and has published a number of articles in various medical publications. He also serves as the Chief of Obstetrics and Gynecology at Florida Hospital Orlando. Mark Crider, MD Dr. Crider received his medical degree from the University of Kentucky College of Medicine, Lexington, Kentucky. He completed his general surgery internship at Wake Forest University in Winston-Salem, North Carolina, and his obstetrics and gynecology residency at Bethesda North Hospital in Cincinnati, Ohio. Prior to joining the Loch Haven OB/GYN Group in July 2007, Dr. Crider practiced in Celebration, Florida, for three years. Since then, he has been a champion supporter in the introduction of electronic medical records into physician offices. Dr. Crider has been honored for his infertility research, one of his areas of expertise. D. Ashley Hill, MD Dr. Hill received his medical degree from the University of South Florida College of Medicine in Tampa, Florida. He completed an internship at Louisiana State University s Charity Hospital in New Orleans and residency training at the University of South Florida College of Medicine. Since 1998, Dr. Hill has been both Medical Director of Loch Haven OB/GYN and Associate Director for the Department of Obstetrics and Gynecology of the Florida Hospital Graduate Medical Education Program. Dr. Hill is an Associate Professor at the FSU and UCF Colleges of Medicine. He has been awarded Teacher of the Year at several institutions. Dr. Hill has also published a number of papers in medical journals and on the Internet and volunteers to write for Internet education sites.

6 Kristin Jackson, MD Dr. Jackson received her medical degree from Hahnemann University in Philadelphia, Pennsylvania, and completed her residency training at Abington Memorial Hospital, Abington, Pennsylvania. As part of her medical education, she performed medical research and won scholarships and awards. She also served as an OB/GYN faculty member for resident physicians at Abington Memorial Hospital. Dr. Jackson is the Clerkship Director for OB/GYN at the Florida State University (FSU) College of Medicine and has been voted outstanding educator of the year for the past two years by the FSU medical students. She is a member of Florida Medical Association, Association of Professors of Gynecology and Obstetrics (APGO) and Perinatal Pediatric Ethics Committee. Gargey Patil, MD Dr. Patil has expertise in routine and high-risk pregnancies, cervical dysplasia and chronic vaginitis. He received his undergraduate degree from University of Pennsylvania, Philadelphia, Pennsylvania, and his medical degree from Jefferson Medical College, also in Philadelphia. He completed his residency in obstetrics and gynecology at Medical College of Pennsylvania/ Hahnemann University Hospital. During his residency, he won the Obstetrical Society of Philadelphia s Resident Bowl Award, given to the resident finishing first in their annual tristate academic competition. Prior to joining Loch Haven OB/GYN in 2009, Dr. Patil was in private practice in Orlando for 10 years. He has also served as clinical faculty with Florida State University College of Medicine since its inception in He is board certified by the American Board of Obstetrics and Gynecology. Christopher Walker, MD Dr. Walker received his medical degree from the University of the West Indies and practiced family medicine in Jamaica. After moving to the United States, he completed a year of training in family medicine at the University of Texas, Southwestern Medical Center in Dallas. He then completed his obstetrics and gynecology residency training at the University of Kansas Medical Center, Lawrence, Kansas. Dr. Walker joined the Loch Haven OB/GYN Group in 2003 and currently serves as the Medical Director for the Mother-infant Postpartum Unit at Florida Hospital. Dr. Walker has published papers in medical journals and has won research awards. In 2006 and 2008, he was honored by his colleagues and nurses as the The Most Outstanding Obstetrician at Florida Hospital. Melanie J. Reis, ARNP, CNM - Midwife Ms. Reis is an Advanced Registered Nurse Practitioner (ARNP) and Certified Nurse Midwife (CNM). She received her nursing degree from Florida State University, and her ARNP and CNM from the University of Florida. Prior to joining Loch Haven OB/GYN in 2009, she worked in Orlando as a labor and delivery nurse for 18 years and a nurse midwife for 10 years. She is a member of the American College of Nurse Midwives and the Central Florida Advanced Nursing Practice Council.
