Review the CPAT TESTING SCHEDULE and decide when you would like to challenge the test. Open Test challenges are allowed at scheduled TIMED Course Practices and scheduled TEST dates only. 2. Send an email to CPAT@sufd.org to request an appointment time to challenge the test. (Appointment times are generally available from 8am to 9am on these dates.) Note: Appointments may be requested by email only. Do not call. 3. Complete the Open Test Challenge REGISTRATION forms that are attached. (Pages 4-7) 4. Bring the following to your appointment at the Testing Facility location (Map/Directions are attached): Completed Open Test Challenge Registration forms CHECK for $150 made payable to SUFD (cash, cashier s check or money order also accepted NO Credit Cards) Fee is non-refundable. ***PAYMENT MUST BE MADE BEFORE YOU CHALLENGE THE TEST*** Driver s License 5. Proper attire is required to challenge the test. For safety reason you are required to wear long pants, a t-shirt or sweatshirt, and gym shoes (footwear without an exposed heel or toe). Additionally you cannot wear any loose or restrictive jewelry, including watches. No shorts or sandals are allowed. Candidates not adhering to this dress code will not be allowed to participate in any CPAT activities. 6. You will receive your CPAT card on the day you pass the test. 7. If you do not pass and wish to challenge the test at a future date you will need to complete this entire process (requesting an appointment, completing another registration form, etc.) Southwest United Fire Districts highly recommends that candidates who wish to do an Open Test Challenge review the CPAT videos available on the website. Candidate Physical Ability Test (CPAT) Waiver of Claim of Injury This form must be signed before you are permitted to access the CPAT testing station for any practice session or test session. This form covers all practice events and actual test administration. You will be asked to perform eight (8) physical tasks and will be given specific instructions (by videotape and proctors) in the manner in which these physical tasks are to be performed. Candidate Physical Ability Test (CPAT) ACKNOWLEDGEMENT AND WAIVER OF CPAT ORIENTATION AND/OR CONDITIONING PERIOD AND/OR TIMED PRACTICE RUNS. This form must be signed before you will be permitted to take the Candidate Physical Ability Test (CPAT) if you did not attend the orientation & practice sessions prior to this test. A fire department administering CPAT as a condition of hire, must ensure that all candidates were provided full and equal access to a CPAT orientation and practice program. The orientation and practice program must commence at least eight (8) weeks before commencement of the official CPAT test date. This program is composed of two phases. 1. The fire department will provide each candidate a full and equal opportunity to attend at least two (2) orientation sessions during which candidates will receive hands-on familiarity with the actual CPAT test apparatus. These required orientation sessions will be provided by certified Peer Fitness Trainers, fitness professionals and/or CPAT- trained fire fighters (proctors). These individuals will familiarize each candidate with each CPAT task and the test apparatus. They will advise all candidates concerning specific conditioning regimens and techniques to help each candidate prepare for the CPAT test. 2. The fire department will provide each candidate a full and equal opportunity to attend at least two (2) timed practice runs of the CPAT, using CPAT apparatus. These required practice runs must occur within thirty (30) days before the commencement of the official CPAT test dates. Following each practice session, certified Peer Fitness Trainers, fitness professionals, and/or CPAT-trained fire fighters (proctors) shall help the candidates understand the test elements and how they can improve their performance and conditions. This two-phased orientation and practice program is a mandatory condition for candidates taking the CPAT test. However, it is recognized that some individuals may be capable of passing CPAT without participation in these programs. These individuals may excuse themselves from this mandatory condition upon the receipt by the fire department of a written and signed waiver, acknowledging that the fire department made available these programs on an equal basis to all candidates and that the candidate knowingly and voluntarily waived participation in the orientation and practice sessions. Orientations and practice sessions are designed to give each candidate identical information regarding the test so that each will have the maximum probability for success. During the classroom orientation, candidates are shown the CPAT orientation video and are given the CPAT Candidate Preparation Guide. The orientation and practice sessions provide an equal and full opportunity for each candidate to view the test events, talk with qualified professionals and instructors and physically examine and use test equipment, tools, and props in a controlled and consistent setting. Candidates are directed to familiarize themselves with all elements of the test. Further information regarding the orientation and practice sessions may be obtained from your fire department. I have read and understand the nature of the orientation and practice sessions and the time period between Orientation and actual CPAT administration. By executing this acknowledgment, I hereby knowingly and voluntarily waive my right to participate in the above-described orientation and practice sessions. LADDER CLIMB EXERCISE The State of Illinois enacted Public Act 097-0251 in August 2011 which mandates new firefighter candidate physical ability test requirements. Candidates are now required to participate in a ladder climb exercise and demonstrate an ability to operate from heights. Beginning January 2012 SUFD will offer a Ladder Climb Exercise which fulfills the new State law. Candidates participating in the SUFD CPAT (beginning January 2012) will not be charged an additional fee to challenge the Ladder Climb Exercise. The CPAT and Ladder Climb Exercise will be conducted during the candidate s scheduled CPAT test. Upon successful completion of the ladder climb exercise candidates will be sent a certificate of completion. The Ladder Climb Exercise is not related to the CPAT and will not have any impact in any way on a candidate s CPAT test result. Candidates who participate in the SUFD Ladder Climb Exercise will climb to the top of an extended ground ladder that is in a fixed, inclined position. Once reaching the top the candidate will be required to descend the ladder to ground level. All candidates will be given instructions before starting the ladder climb exercise. Instructions 1. Complete the Ladder Climb Exercise REGISTRATION form that is attached. (Page 9) This must be completed and signed in addition to the CPAT registration forms. 2. Upon your successful completion of the Ladder Climb Exercise a certificate of completion will be sent to you via email. Please be sure your email address is printed clearly on the registration form.
