MyAdvocate Patient Portal A secure and easy way to connect to your Advocate Health Care team

We recognize that health care consumers can benefit greatly from capabilities like having access to their personal health records or being able to send messages to their physicians electronically. MyAdvocate offers these features and much more. Your feedback as a pilot participant will help us identify additional opportunities to enhance this important tool prior to making it available to all patients. What can I do through MyAdvocate? MyAdvocate is a free, password-protected and secure web-based interactive service. The version of MyAdvocate you will be testing as part of this pilot allows you to do the following: Ask your provider a medical question Request an appointment Request an Rx renewal View the following clinical information from your chart: Allergies Immunizations Lab results Medications Problem list Create your own Personal Health Journal, enabling you to: Upload documents Enter notes for your next visit Track your daily health measures We will be adding new features to MyAdvocate and welcome your feedback to help identify future enhancements. Request an appointment With MyAdvocate, you can request an appointment with your physician 24 hours a day, 7 days a week. A response will be sent to your private message box within 1-2 business days. Request Rx renewal MyAdvocate allows you to securely request a renewal for medications currently being prescribed to you without having to visit the physician s office. Keep in mind that all requests are carefully reviewed prior to approval. A response will be sent to your private message box within 1 2 business days. Ask a medical question There are times when you would like to ask your physician a non-urgent medical question without scheduling an office visit. Now, with MyAdvocate, you can submit a message and a response will be sent to your private message box. If you have a medical emergency or need immediate medical attention, please dial 911.
Ready to get started? Here are simple how-to instructions that will help you complete basic tasks using MyAdvocate. Logging on to MyAdvocate 1. Type in the following URL: https://myadvocate.advocatehealth.com* 2. Enter your user name (email address) and the password that you created when you initially set up your account. * For your convenience, we recommend adding this link to your favorites. Communicate Securely with Physicians/ Office Staff 1. From the MyAdvocate Secure Mail page, select the appropriate Secure Messaging template: a. Ask a Medical Question b. Request an Appointment c. Request Rx Renewal
2. Using the dropdown menus, select the desired office location and provider 3. Enter the appropriate details in the message and click the Send button Please allow 1 2 days for a response. You will receive an email alert letting you know that a response has been securely posted to your portal account. If you have a medical emergency or need immediate medical attention, please dial 911.
Access Your Electronic Clinical Summary from Your Secure Mailbox 1. Click on the Updated Electronic Clinical Summary secure message 2. Select the Electronic Clinical Summary attachment
3. View the summary Due to confidentiality requirements, some test results may not be included in your Electronic Clinical Summary. Please contact the office if would like information on any withheld results. Access Your Chart Data from Your Office Summary 1. Click on the My Office Summary tab
2. Navigate through the Overview tab to see data in individual applications 3. Click Last 6 Lab Results to see the Lab Result Flowsheet Due to confidentiality requirements, some test results may not be included in the Electronic Clinical Summary. Update your Personal Health Journal 1. Click on the My Personal Health Journal tab 2. Upload documents by clicking Upload and browsing to the document you would like to add 3. Enter any notes you may have for your next office visit by clicking New, filling out the template and clicking Save 4. Click Daily Health Tracking to enter your daily health information, such as your blood pressure You can customize what you choose to track by clicking the Customize button
