PHS-Connect and My EMR Care Coordination - Results Delivery Orders to PHS Lab CCD / C-CDA document sharing and retrieval Meaningful Use - Transitions of Care (via Direct Messaging) Allows for seamless communications from directly within practice EMR system to another physician EMR, to a Clinical Portal physician with an attached C-CDA Secure Messaging from Provider to Patients (via Direct Messaging) Patient Portal View, Download and Transmit Physician must contribute C-CDA to PHS-Connect and provision patient users with access to the MyPinnacleHealth patient portal If Patient is provisioned for MyPinnacleHealth and views any CCD/C-CDA from any clinical source during the reporting period, the provider receives credit 12

Shortcuts To Use When Creating Secure Message Favorites Users can create their own favorite list by clicking on the heart icon next to the recipients name Snippets- A way to insert a macro or pre-defined template.
