4 Establishing connectivity Hospitals are recognizing the benefits of having a single, common view of a patient s health information and history. This information helps caregivers make better diagnoses, provide treatment faster, and helps reduce the cost of healthcare by eliminating duplicate testing. Achieving this common view requires an enterprise Health Information Exchange (HIE). An HIE allows your hospital to share patient information with other healthcare entities in your community. Iatric Systems offers a variety of solutions to help your hospital establish this connectivity. Interface Engine Iatric Systems advanced interface engine, EasyConnect Jaguar, eliminates the data barriers between disparate healthcare systems. It provides virtually effortless links between multiple applications at a fraction of the cost of building individual point-to-point interfaces. EasyConnect Jaguar also lets you view and manage external interfaces from one convenient dashboard regardless of source. Your hospital has just installed a new bed management system that needs to receive ADT information from its EHR. To set up your own interface with the EasyConnect Jaguar interface engine, your hospital s IT analyst uses a simple intuitive Interface Configuration Screen to drag and drop in the right filters, templates, and functions while EasyConnect Jaguar does the rest. If your IT staff is limited, you can have Iatric Systems create and manage the needed interfaces for you. Physician Office Integration Iatric Systems Physician Office Integration solution lets your hospital share its EHR data and exchange lab results, radiology reports, and other patient information with physicians who are using EMR software systems. It also allows physicians to place orders in the hospital s EHR directly from their EMR. One of your hospital s affiliated physicians needs to conduct a Complete Blood Count for one of her patients. Her staff makes the request from the office EMR directly to your hospital and then sends the patient over to your lab for a blood draw. Once tests are completed, the doctor s office simply uses its EMR system to access the results. It s that easy.
5 Clinical Document Exchange Iatric Systems Clinical Document Exchange makes it easy for your healthcare organization to send and receive a summary picture of the pertinent clinical, demographic, and administrative data for a single patient. Clinical Document Exchange also meets the applicable Meaningful Use requirements for exchanging data. A physician affiliated with your hospital is seeing one of your patients for a new condition that has arisen since his discharge. With a Clinical Document Exchange document sent from your hospital upon discharge, the doctor or his staff can now electronically access the needed patient data from their EMR system and see it in an easy-to-read format. Instead of relying on the patient s memory and having to input the data, the physician can see the demographics, medications, treatments, and results right on his EMR so he can provide informed, appropriate care. Public Health Interfaces By compiling relevant patient information from hospitals and healthcare providers, your community, region, and the nation can better prepare for emerging medical threats and future immunization demands. Iatric Systems provides three interfaces that make it easy for your hospital to provide immunization, reportable laboratory data, and syndromic surveillance data to your region s public health agencies. A patient in the Midwest goes to his local emergency department with complaints of vomiting, body aches, stomach cramps, and fever. After his release, his symptoms and pertinent patient data are transmitted to his hospital s regional public health agency, where his data is aggregated with other patients from the same hospital and that of 20 other hospitals in the region. Once the data has been analyzed and reviewed, it reveals that there is a new kind of influenza outbreak that had not been previously identified. The agency then informs area hospitals and physicians to watch for the symptoms and report on effective treatments to help stem a widespread outbreak.
6 Connecting patients and providers Establishing a bi-directional exchange of patient data with physician offices and patients provides extensive benefits to your hospital, your physicians, and your patients. Exchanging data in real time helps engage patients in their own healthcare and brings patient clinical data to the point of care. Iatric Systems offers many solutions that help your hospital connect with patients and providers. Patient Portal PtAccess is a perfect addition to your enterprise HIE. It is a patient portal that provides an easy, low-cost way to give your patients access to their health information. PtAccess helps engage patients in their own healthcare by providing them a web-based portal to see their health information, enter updates, and share information with family and providers. PtAccess recently passed C-CDA testing at the IHE NA Connectathon Conference Your patient has just been released from your hospital and, as often happens, she loses her discharge instructions. Once home, instead of having to call your hospital for that information, she can easily get it from her private page on your hospital s patient portal. There she can also access and pay her bill, manage her medical history, and much more. Physician Portals Iatric Systems MDAccess and MDOrder allow you to bring the value of your HIE to physicians not using an Electronic Medical Record (EMR) system. As private portals, MDAccess and MDOrder let your physicians send and receive clinical data via the Internet and order patient testing electronically. Some physician groups that an area hospital works with on a regular basis have been reluctant to implement an EMR system because of the cost. Because they have MDAccess and MDOrder, they have the ability communicate with your hospital and to access clinical data and order lab tests over the web, which helps them improve patient care and increase your hospital s revenue. Clinical Data Repository To meet the increasing community demand for HIEs, Iatric Systems has developed IatriConnect Clinical Data Repository (CDR). This technology provides an HIE with an efficient, scalable, and cost-effective solution for storing, standardizing, and securing clinical data from multiple providers. When a hospital nurse needs to access historical records for a specific patient, she may have to log into three different systems, such as Allscripts for home health visits, MEDITECH for the hospital in-patient information, plus an office-based EMR system such as eclinicalworks. IatriConnect CDR eliminates this time-consuming task by aggregating all the data into one convenient, unified patient view.
7 Direct Secure Messaging A key component of any HIE is the ability to transmit data securely. Iatric Systems DirectConnect Gateway allows users to know that their data is safe and the proper security is in place to protect their data. By integrating DirectConnect Gateway into their workflow, providers can easily and securely exchange information between systems and between different entities. DirectConnect Gateway meets all Meaningful Use requirements for Direct Secure Messaging and adheres to the Direct Project standards. A patient is ready to leave the hospital after a surgery. Upon patient discharge, the hospital needs to send the transition of care summary to the patient s primary care physician. They are able to do this securely using DirectConnect Gateway in conjunction with their other hospital systems. The patient then goes to see their primary care physician for routine follow-up and knows that their provider will have all the information they need. Establishing a community patient index HIEs are built for the exchange of patient data. This exchange goes beyond providing care to an individual patient and into building medical intelligence that supports a patient s care across multiple providers. To achieve this, an enterprise master patient index requires seamless connections between dozens of EHR, EMR, and other hospital systems. Iatric Systems offers a variety of solutions to help your hospital achieve these connections regardless of the vendor systems involved. Enterprise Master Patient Index True interoperability is achieved when a patient can be clearly identified from any EMR within the HIE. Iatric Systems MatchBuilder reconciles visit information from multiple EMRs into a single Enterprise Master Patient Index. This index then links a patient s visit history to an enterprise patient record so it can be queried and identified by multiple EMR systems. A Record Locator Service easily locates the records coming from multiple sources of clinical data and documents. A patient with a history of visiting her primary care provider and your acute care hospital now visits a GI specialist. To receive the patient s visit history and enterprise identifier, that specialist conducts an electronic query on the HIE MatchBuilder database. He now knows where the patient has been treated and can query the hospital and provider EMRs for her records. With a more complete understanding of the patient s medical history, the GI can provide better care.

Clinical Document Exchange, version 1.5, has been certified on September 26, 2013 as an EHR Module, for Eligible Hospital, certification number is CC This EHR module is ONC 2014 Edition compliant and has been certified by the Certification Commission for Health Information Technology (CCHIT ), an ONC-ACB, in accordance with the applicable Eligible Hospital certification criteria adopted by the Secretary of Health and Human Services. Clinical Document Exchange, version 1.5, meets the following certification criteria: (b)(1) Transitions of Care Receive, Display, and Incorporate Transition; (b) (2) Transitions of Care Create and Transmit Summary Care; (g)(1) Automated Numerator Recording; (g)(4) Quality Management System. The additional types of costs that an Eligible Hospital would pay to implement the Clinical Document Exchange version 1.5 in order to attempt to meet meaningful use objectives and measures include: relevant hardware if not already available. Public Health Immunizations, version 1.5, has been certified on September 9, 2013 as an EHR Module, for Eligible Hospital, certification number is CC This EHR module is ONC 2014 Edition compliant and has been certified by the Certification Commission for Health Information Technology(CCHIT ), an ONC-ACB, in accordance with the applicable Eligible Hospital certification criteria adopted by the Secretary of Health and Human Services. Public Health Immunizations, VERSION 1.5, meets the following certification criteria: (f)(2) Transmission to immunization registries; (g)(4) Quality management system.
