Instructions for Completing Venn Diagram and Student Level Data Worksheet Venn Diagram On the Venn Diagram write the names of your students that did not pass TAKS/STAAR. Put the non-passers for each subject in the corresponding section of the Venn Diagram. o Highlight each teacher s students in a specific color. You will see at a glance which students were in each teacher s class. o Circle the names of students that also failed previous year TAKS/STAAR. On the Discipline/Attendance Venn Diagram write the names of your students who have 3+ referrals for discipline-related incidents to the office in the Discipline section. Write the names of the students who have 10+ absences in the Attendance section of the circles. Students who have issues in both areas will then be complied in the middle. o Highlight the students in yellow who did not pass TAKS/STAAR Reading/ELA o Highlight students in green that did not pass TAKS/STAAR Math o Highlight students in pink that did not pass TAKS/STAAR Science o Highlight students in blue that did not pass TAKS/STAAR Social Studies o Some students may have multiple highlights! o Circle the students that failed previous year TAKS/STAAR Student Level Data Worksheet Name - Enter student s first and last name Grade - Enter the enrolled grade level from 2011-12; enter the current enrolled grade level for 2012-13 Accountability Groups & Special Programs Put a Y for Yes under the appropriate student groups; you can also put a N for No if you choose 2011 TAKS enter the student s vertical or scale score Met enter Y for met standard; N for did not meet standard and C if the student earned a commended score 2012 TAKS/STAAR enter the student s vertical or scale score Met enter Y for met standard; N for did not meet standard and C if the student earned a commended score Qs Away put the number of questions the student was away from meeting the passing standard Category put the number of questions the student answered correctly for each category over the number of total questions for each specific category (example: 4/6) Benchmarks/Grades put the benchmark score and the grade average for the 6 weeks period (example 74/85) Enter raw score data for 2012 STAAR or TAKS data for HS TAKS graduates Student Level Intervention Worksheet Complete one sheet per 6 weeks grading period Name Enter student s first and last name to correspond to the list of names on the Student Level Data Workshop Benchmark Enter the benchmark score for the 6 weeks period 6 Wks Grade Enter the student s grade average for the current 6 week period Intervention Enter the interventions provided for the current 6 week period
