You must complete an application for the Florida Medicaid Program by contacting the Department of Children & Families DCF services. HUD HOUSING INFORMATION October 2010 In response to your request of this date, we are pleased to provide you the information you requested on subsidized housing. Keep it on your Favorites list for future patients. A list of subsidized apartments is on the website www.hud.gov/apps/section8/index.cfm. Once you are on that website, choose the state you want to live in. Then click Next etc. If you have any questions please call us. Should you be interested in obtaining housing in a specific complex, you must apply at the project address. Project management personnel at the property will determine your eligibility and advise you when an apartment will be available. This list is subject to correction. If you have problems with the list or discover errors, please advise our office. Our telephone number is (904) 232-1777, extension 2503. You may also want to contact the local housing authority. The website for the national list of public housing authorities is www.hud.gov/pih/pha/contacts/index.cfm. We fund money to them to subsidize the rent through our Public Housing Program. After clicking on the above link to access the website, you will see, under the category marked Resources, the words Public Housing Agency (PHA) Contact information. Click on that line. Use the drop down box and click on the state you want to live in. Then click GO TO THIS PAGE. The next screen shows the list of housing authorities for that state. If you have any questions on this subject, please call the Public Housing Division in our office. Their telephone number is (904) 232-1777 x 2520. The State also has a website that lists affordable apartments. It is www.socialserve.com/tenant/index.html. Click on View Other States. Then you can choose Florida or any other state on that list. The above mentioned Housing Authority runs that program. Subsidized housing is also available through the United States Department of Agriculture (USDA), Rural Development in Gainesville, Florida. Multi-Family Housing - Rental Assistance Program (Section 515 and 514) are for individual residents of multi-family dwellings primarily in the form of rental assistance. Rent subsidies under the Rental Assistance Program ensure that elderly, disabled, and low-income residents of multi-family housing complexes financed by Rural Housing Service (RHS) are able to afford rent payments. With the help of the Rental Assistance Program, a qualified applicant pays no more than 30 percent of his or her income for housing. Please note that not all residents of RHS-financed housing developments receive rental assistance. Using the internet, you may access the web site at www.rurdev.usda.gov/fl for address and telephone number of the closest USDA office and listings of subsidized apartments Click on the term Area Offices. At this point a list of the offices will appear. To print the list of apartments, use the website http://rdmfhrentals.sc.egov.usda.gov/rdmfhrentals/mfh_state_text?srch=n&home=no. Click on the state. Then click on the county. That will bring up the list of apartments. Click on each one to get information on it.. If there is something that another agency of the Federal Government might be able to do for you call 1-800-FED- INFO (1-800-333-4636). The State of Florida has a program entitled Emergency Financial Assistance for Housing. You can call 1-850 488-3700 for more information. The agency is the Department of Children and Families. If you are in another State, your state may have assistance we do not know about. Depending on your needs, you may also be able to get assistance by calling your local United Way Office.
