Once received Telecom will contact you to discuss the relevant requirements for your submission. I agree to fully disclose the products specification, functionality and any defects prior to submitting this product. If this Telepermit application is successful I also agree to fully disclose any defects identified after the product has entered service. I acknowledge that : All information and devices supplied to Telecom for the purposes of testing and approving the device are at no cost to Telecom and will not be returned. This application will not be processed until all charges are paid in full and all submission requirements are satisfied. Submission and acceptance of this application in no way grants any priority to testing and approval of the device. Completion of this application form in no way deems acceptance for testing or Telecoms approval of the product. Granting of a Telepermit is not an endorsement of the product and must not be used as such Should for any reason this device no longer comply with the general and specific conditions of a Telepermit than Telecom may withdraw that Telepermit and no longer provide access for this device to its network. That in submitting this device all New Zealand regulatory requirements have been complied with. The applicant is responsible for notifying all changes to Hardware and Software specifications in writing to Telecom. Such notification must include detailed release notes outlining the specific changes made and any impact on the devices performance. SUBMISSION REQUIREMENTS: This following defines the deliverables required of all Vendors submitting wireless devices to Telecom New Zealand s Centre for the purpose of certification testing. All requirements defined in this document must be fulfilled before any testing can commence or certification issued. All s defined in this section shall be provided to Telecom New Zealand free of charge. As any continued regression testing will require the use of these devices all s outlined in this document will not be returned to the vendor. It is Telecom s preference that all documentation and software is supplied on CDR disks shipped together with outlined hardware requirements. Telecom can also provide an FTP server address and logon for uploading of these documents. The FTP server is located at: fttp://tnzptc.hopto.org Dispatch via email is also acceptable but Telecom requests that any data is sent in a compressed format (zip file). Programming Documented step by step instructions for all keypad programming/access EMC Power Supply compliance certificate (Electrical Safety & EMC) IEC60950 or AS/NZS60950 CDG Stage 1 Test reports with primary being 800MHz 2.0 Test Results CDG CDG Stage 2 Interoperability test reports with primary being 800MHz and Lucent infrastructure. ( UDMV2.) including full support for UTS Interface Specifications as it pertains to IS2000. UAPROF UAPROF file in either.rdf or.xml format Java SDK Copy of the Software Development Kit for Java Modem Soft Copy of Modem drivers specific to device Phone Synch Soft Copy of Phone synchronisation software specific to device Programming Programming interface cable to connect the PC to the device. 5.0 Test Tools Diagnostic RF 2 s Dual UART Y cables to permit simultaneous wireless data operation and diagnostic monitor (DM) air interface logging. RF adaptors/cables with measured RF insertion losses at 800MHz and 1.9GHz. Data 3 s Serial Data cable specific to device. 5 s USB Data cable specific to device. Flat File CSV document containing ESN, A-Key, and MSL codes for each device submitted. Formatted as outlined in TNZ ESN File documentation. 6.0 Documentation Existing Issues Detailed description of existing known issues, actions required for correction, and date corrective action will be completed. Service Codes Details of all secret codes, and access to diagnostic screens. User Guide Draft version of User guide for TNZ version. 7.0 Contact Details Primary Technical Consultant 1 copy Name and contact information for primary technical consultant for Device.

ESN NOTIFICATION: It is a requirement of this application and the granting of a Telecom Telepermit that all ESN's for the devices to be connected to Telecom's network must be supplied to Telecom in advance. Device ESN s must be supplied to Telecom in the format prescribed in the document CDMA ESN File Requirements. A copy of this document is available on request from mobileptc@telecom.co.nz. APPLICABLE CHARGES Charges apply to testing and certification of wireless devices. All charges are payable by the applicant in advance of testing and are regardless of whether or not a Telepermit is issued for the device submitted. Indicative charges are: Preliminary Assessment Acceptance Testing Re-assessment NZ$1000 NZ$5000 NZ$1000 Specific charges will be advised upon submission of an application.
