From referrals to service questions to billing, the process may feel overwhelming. Rest assured. We are here to help. Our caregivers will gently and expertly guide you through the process and decisions that need to be made in order to keep you or a loved one safe at home.

Compassionate Care in the Comfort of Home Shared care services Private duty home care Shared Care Services is a private duty services program, offering you or your loved one additional support to promote independence and to provide the necessary care for maintaining an active lifestyle. No criteria needs to be met in order to receive these private duty services. Home Care Support Services Each client s support and care are managed by a Licensed Nurse and RN Program Director, ensuring a comfortable and happy lifestyle. In partnership with you and your family, our dedicated team of professionals and caregivers provides a full range of services including: Personal care and support with activities of daily living Companionship Light housekeeping and assistance with meal preparation and errands Medication reminders and assistance Respite care Support while recovering from a medical procedure or hospitalization Insurance Information Home care services through our private duty program may not be covered by insurance and are privately or self paid by the client. If you have Long Term Care insurance, our staff can work with you in determining if private duty services are a reimbursed benefit. How do I start receiving Private Duty Services? To start services and to address your questions or concerns, our staff will take your call 24 hours a day, 365 days a year. Call us at 734-222-4000 we would be happy to discuss your options and start your service. Shared services home health care Shared Services Home Health Care is a Medicarecertified, CHAP-Accredited home health care provider. Our team of Registered Nurses Dietitians Social Workers Therapists Support Staff provides the services you need, with your doctor s direction, when skilled assistance becomes necessary to remain home or to assist in the event of a serious health challenge. Our goal is to promote independence and maximize strengths and abilities while improving your overall wellbeing. Service Area Washtenaw County Lenawee County Wayne County Jackson County Monroe County Livingston County Oakland County Macomb County Service Criteria In order to qualify for home care services, the patient is required to: be under the care of a medical doctor have an illness or injury that causes the patient to be homebound have skilled nursing and/or therapy needs on an intermittent basis Insurance Information Medicare Home health care services can be covered by your Medicare benefit if you meet the service criteria. Other Insurance Other insurance plans we participate with include: Aetna, Blue Care Network, Blue Cross & Blue Shield, Co-finity, Medicaid, Private Insurance and Worker s Compensation. Other insurance plans sometimes do not cover home health care services unless certain conditions are met. We will work with your insurance company to determine eligibility and coverage.
