However, services rendered by any independently contracted PPO provider (such as Cadence Health and Cadence Health Hospital) for members of SUPERVALU, Inc., Albertson s, Jewel and Osco with BCBS Choice PPO coverage are considered in-network. The affected members live in the following Illinois counties: Cook, DuPage, Kane, Kankakee, Lake, McHenry and Will. The member ID card will indicate BCBS Choice PPO and the following: SUPERVALU Inc. RUH or Albertson s, Jewel and Osco NWY. Eligibility and benefits should always be checked prior to rendering service to determine in-network status.
