TO ALL EMPLOYEES & CONTRACTORS Following safety practices and procedures is the responsibility of ALL employees and contractors of your company Your company recognizes its leadership role in influencing work site situations and in promoting safety, health and environmental awareness, and we need your commitment to make it work. Activities within your company facilities and on Client s work site will be conducted on the principle that ALL INJURIES AND DAMAGE TO PROPERTY AND THE ENVIRONMENT CAN BE PREVENTED. Under no circumstances will the health and safety of the worker, or the environment be sacrificed for reduced cost or by attempting to complete a task in a shorter length of time. Never operate any tools, equipment, or machinery that you are not familiar with, you have the right to refuse unsafe work. LEGISLATIVE COMPLIANCE Your company policies and procedures have been developed in compliance with Occupational Health and Safety and Worker s Compensation Board Legislation. Compliance with legislation is stressed and expected for all your company employees, contractors, visitors and clients. ALL employees and contractors of your company must accept responsibility to comply with Health and Safety legislation and with established rules and procedures. OCCUPATIONAL HEALTH & SAFETY Occupational Health & Safety Legislation The Occupational Health and Safety legislation was created to help protect workers from risk of injury at work. As different people have different ideas as to what is safe this legislation defines basic safety standards for everyone, and provides for enforcement of those standards. Employer Responsibilities: The employer must do everything reasonably possible to protect the health and safety of its workers. This means workers must have the skills and the training needed to perform their jobs safely and hazards at the workplace must be properly identified, assessed, prioritized and controlled. The employer must make available the appropriate personal protective equipment (PPE) and train you in its use. Safe work procedures must be available for reference at the work site. It is up to the employer to develop codes of practice for the use and selection of Respiratory Protective Equipment (RPE) and for working in confined spaces. The employer must also train workers to follow these codes. Your employer must ensure that you re competent to do your job safely. Worker Responsibility: You also have duties under the legislation. You must work in a safe manner, be safety conscious on the job and co-operate with the health and safety measures your company or our Client sets in place. It is your responsibility as a worker to follow the safe work procedures established by your employer. It is your responsibility to operate forklifts and power jacks safely. 7
It is up to you to comply with all appropriate regulations after being informed and trained by your employer. You must report any hazardous or potentially hazardous situations to your supervisor. Injuries or illnesses must be reported immediately or as soon as possible. You must participate in health and safety training. It is your responsibility to wear the required personal protective equipment. One of your important responsibilities, if applicable, as a worker is to inspect all equipment and machinery before use. Defective equipment, including PPE, is not to be used and must be reported to your supervisor. Make sure you know the location, type and operation of emergency equipment at the work site or on your equipment. It is in your best interest to ask questions about your job, work procedures and safety concerns. IF YOU DON T KNOW ASK! You have the right to refuse work if you believe imminent danger exists. Imminent danger means any danger which is not normal for the job, or any danger under which a person in that job would not normally carry out his /her work. Once you exercise your right to refuse you need to discuss your concerns with your supervisor and provide reasons as to why you refused. 8
MANAGEMENT COMMITMENT AND ASSIGNED SAFETY RESPONSIBILITIES Warehouse Manager Job Description: Disputes: Improvement of Operations Determine that all employees are following the policies and procedures that you have established. Determine: That the existence of unsafe working conditions or practices is corrected; That fire equipment and fire exits are adequately maintained; That tools, equipment, machinery, and parts being used by the supervisor or other employees are in safe operating conditions. That the building, dock areas and racking comply with all government health and safety regulations. Record recommendations with respect to safety concerns that require further action and report these to the proper authority. Assist in the prompt investigation of accidents and injury claims through on-site interviews, inspection and consultations. Conduct thorough inspections of the work place or job site as established in this manual, at least monthly unless otherwise specified. Ensure that regular safety meetings are held at least once a month to discuss: Current accident and injury claims, their causes and means of prevention; Reports of inspections and investigations; Action taken on previous safety suggestions and the result to date; Where no action was taken, reason why they were not taken; New suggestions, concerns and complaints relating to safety policy and procedures; New policies being adopted by management. Submit to head office a complete, concise and accurate accident report (on the designated form) with respect to all accidents and/or injury claims. Review, edit, and forward a copy of the safety meeting minutes to head office. Implement and maintain any new procedures, regulations and innovations as prescribed in the area of accident prevention and safety by the corporation. Carry out such disciplinary action as required as set out. Review accident prevention measures and communicate concerns about these to head office. Receive, consider and act on safety concerns and problems. Where a dispute arises as to the application of a rule or regulation of the legislation, or a matter of safety concern is expressed of which no ruling exists, the Warehouse Manager shall be empowered to rule on the matter in dispute. Your company welcomes and encourages employee suggestions. Through this process, the value of our services to existing clients/customers will be enhanced and our ability to attract new clients/customers will grow. Any reasonable suggestions presented by an employee for the improvement of the 9
overall efficiency of the operation shall be reviewed and considered. All suggestions pertaining to such matters should be forwarded (preferably in writing) to the Warehouse Manager. The process of improvement involves the following steps: Define the problem; Analyze the problem; Discuss and analyze the causes; Implement corrective actions; Assess the results of corrective measures with all personnel involved or affected; Draw conclusions; Standardize the changes. This process is relatively informal and does not require committee work. It does require direct involvement of personnel at all levels. There may be a lot of problems that are beyond your control but if you work on the ones within your control, it will better help the corporation serve its clients/customers and be successful. Site Administrator Responsibilities Duties Human Resources Purchasing Receiving/Shipping Reports to the Warehouse Manager directly Maintains a professional corporate image with client / customer representatives and employees Provides administration assistance to the Warehouse Manager and operations office staff Maintains the general filing system for the site office Completes other administrative tasks as required Assists in management of shift schedules and assessment of personnel requirements Assists the Warehouse Manager with employee contracts, Health Care Information, and various other items that are needed for new employees when hired on Maintains personnel files and ensures completeness Coordinates the reporting of accidents and injuries to the appropriate authorities. Performs the hiring function including arranging of interviews, selecting and hiring of new employees as instructed by the Warehouse Manager Obtain quotes and availability of items from suppliers Place orders including directing of delivery to or by 3 rd parties for furtherance Write-up and coding of purchase orders Expediting of orders Maintenance of parts information by unit Tracking of items for return to suppliers Tracking of back orders with respective suppliers Completion of all PO s when orders are received Follow-up on discrepancies between packing slips / P O s / P O information / 10

Assist in counting inventory at year end or during the year as required Assist in pricing inventory at year end including looking up part numbers on invoices and corresponding with suppliers Prepare inventory listing Keep warehouse manager appraised of equipment inventory, additions of equipment to sites, changes or additions of equipment to current sites Pick up supplies when needed when Warehouse Manager is unavailable to do so Mechanic (could be a contracted position) Job Description: Under general supervisor performs skilled journey level work in the mechanical repair/maintenance of light/heavy, gasoline/diesel driven mechanical equipment, and other related work as required. Operates and inspects forklifts or mechanical equipment to diagnose defects, analyze malfunctions and performs general overhaul, tune-up, and repair work on forklifts, automobile, light/heavy trucks, mechanical equipment, dismantles and re-assembles equipment using hoists and power hand tools. 11
Maintains detailed records of time, unit number, mileage, and materials used in each piece of equipment. Performs government inspections on mechanical equipment. Implement Health and Safety guide lines. Supervises workers in and around the shop. Maintenance Worker(s) Description: Perform skilled work in the mechanical repair and maintenance of equipment, tools, and forklifts and/or power jacks. Dismantle and re-assemble equipment using hand and power tools. Performs Arc, Gas and Acetylene welding. Use drill press, bench grinder, lubricates a wide variety of parts. Maintain a safe and clean work and yard area. Washing of equipment, house keeping, keep floor clean of debris, wash floors, clean spills immediately. Knowledge: Methods, materials, tools and techniques used in the repair and maintenance of equipment. Correct operation of equipment, occupational hazards and standard safety precautions in the work place. Principles and methods of Arc and Gas Welding. Warehouse/Dock Workers Description: You also have duties under the legislation. Use your legs, not your back. Get help when required. Ensure if using a box cutter, you are trained in its use. Be informed of the hazards of chemicals before working with them. Maintain proper storage on shelves and steel/racking Work from forklifts and stacked product whenever possible Stay out of forklift operating areas whenever possible. Safety Coordinators Description: Knowledge: Contractors Description: Are responsible for the daily administration of the safety program on site. Promote health and safety on the site through example and communication. Assist Management and the Health and Safety committee to investigate accidents, and to prepare accident reports and summaries. Ensure that pertinent safety reports are submitted as required. Prepare written descriptions of identified unsafe conditions and the steps taken to correct these conditions. Keep records of the type and number of safety equipment purchased. Prepare a copy of inspection equipment checklists. Participate in Health and Safety meetings. Ensure that corrective action has been taken whenever deficiencies are identified. Assist with safety seminars or training. Maintain current knowledge of safety literature, regulations and codes of practice. Establish inspection schedules. Review accident reports to stay informed about project and company safety performance All of your company s subcontractors and/or contractors shall observe all government Health and Safety regulations that apply to the work they may be performing. All contractors/subcontractors will observe all of your company s policies, regulations and procedures in place at any Company worksite. Contractors/Subcontractors: have the right to refuse unsafe work that could endanger themselves and/or others. will follow the Company Health and Safety policies and responsibilities. will be aware of all emergency procedures. will wear the appropriate Personal Protective Equipment where necessary Knowledge: Visitors Description: All visitors must be informed of the Company Health and Safety policy and conform to its rules and guidelines. A staff member, who is responsible, must escort all visitors for the visitor s well 13
Knowledge: being during the visit. Visitors must wear personal protective equipment that is appropriate to the nature of their work. Visitors must be informed of potential dangers and are prohibited from entering an area where their health and safety will be jeopardized. Supervisors shall ensure visitors follow company rules and are informed of the emergency procedures. 14
