The detailed Tender Paper for the above purpose can be obtained from-the District Project Management Unit ( MDM), District Project Office, SSA, Sundargarh on payment of Rs.5000/-(Rupees five thousand) only(non-refundable) during the office hours on any working day between dt. 30.08.2013 to dt.&.o9.2013 be submitted along with the Tender Paper. and the money receipt thereof is to The Tender Papers completed in all respect may be sent in sealed cover superscribed as "Sealed Tender for undertaking Transportation of Foodstuff under M D M Programme" to the address of District Inspector of Schools, DPMU (MDM), District Project Office, SSA, SlJndargarh either by Registered Post /Speed Post only so as to reach on or before 1.00 PM of dt.l~.09.2013. The Tender shall be opened on date~~.09.2013at 3.00 PM by the District Level Tender Committee constituted for the purpose. DETAILED TENDER NOTICE FOR APPOINTMENT OF TRANSPORT AGENT UNDER MDM, FOR THE YEAR 2013-14. 1. Sealed Tenders in the prescribed Form are irivited from the intending Transport Contractors for undertaking Transportation of Foodstuff under MDM programme of Sundargarh District during 2013-14.The approximate quantity of Foodstuff to be handled annually is about Q~64.000.00 & the total approximate value of which is Rs.3.62 Crores (Rupees Three Crores Sixty Two) only @ Rs.565/- per Qntl of rice. The ceiling price for transportation of one quintal of rice from FCI Depot to School points in case of MDM is Rs.75/-. 2. The. sealed Tenders superscribed as 'Sealed Tender for undertaking transportation work under MDM for the year 2013-14' will be received by Registered Post/Speed post addressed to District Inspector of Schools, DPMU( MDM), District Project Office, SSA, Sundargarh till 1.00 PM of dt. 1,.09.2013 during office hours on all working days and will be opened in presence of Tenderers' or their authorized agents on dtl~09.2013 at 3.00 PM by the District Level Committee constituted for the purpose. 3. The detailed Tender papers along with prescribed Tender Schedule (Annexure-I) can be obtained on payment of Rs.5, OOO/-(Rupees Five thousand) only (Non-Refundable) from the District Project Management Unit(MDM), District Project Office, SSA, Sundargarh on any working day during office hours from dt.30.08.2013 to dt..t109.2013 and the Money Receipt thereof is to be submitted along with the Tender paper. However, Firms downloading Tender Paper from District Website to submit tender documents must enclose DD amounting to Rs.5000/- in favour of DPC, SSA, Sundargarh and District Inspector of Schools, Sundargarh payable at Sundargarh. 4. The successful Tenderer shall be required to lift Rice under MDM fromfci Depot BalUore/lharsugudalRourkela as the case may be and transport to School points. 5. No Individual Company, Firm and Corporation shall participate in the Tender process unless he/she obtains or possesses valid Agents License under OMV (License of Agents) Rule 1990. 6. The Tender Paper should be filled in properly and legibly without any correction/over writing. The rates to be quoted should be written both in figures and words in the Tender and it should be final. The Tenderer shall not be allowed to change the same under any circumstances and it should be valid for the year 2013-14.

7. (i) Recent Solvency Certificate for Rs.lS.00 Lakhs (Rupees Fifteen Lakhs) only from the competent authority should be furnished along with the Tender Paper failing which the Tender will be liable for rejection. (ii) Certificate from the District Collector/any Government Organization etc about past experience if any, in handling/storage Transport work may be furnished along with the Tender Paper. (iii) Earnest Money @1 % of total value of foodstuff i.e. Rs,3.62 Crores (Rupees Three Crores Sixty Two lakhs) i.e. Rs. 3,62,000/- only in shape of Postal Savings Pass BooklBank Pass Book/NSC/Term Deposit duly pledged in favour of Collector,Sundargarh should be furnished in original along witli Tender Paper. Tender without Earnest Money will be liable for rejection. No Tender shall be allowed to withdraw his tenderlemd until the Tender Process is finalized. In case the successful tenderer refuses to be Transporting Agent after acceptance of his Tender, Earnest Money deposited by him will be liable for forfeiture. (iv) Earnest Money deposited by the successful Tenderer will stand converted to Security Deposit, which will be refunded after successful completion of contract period and audit of accounts of the Transporting Agent. The whole or part of Security deposit will be forfeited for illegal performance or breach of any Terms and Conditions of Agreement during the period of contract. Earnest Money of suc~essful Tenderers shall be refunded after finalization of the tender process. (v) Up to-date V AT clearance Certificate shall be furnished along with the Tender. (vi) Copy of valid PAN Card in the name of individual/firm should be enclosed with the tender paper. (vii)the detailed Tender Paper in original should be returned with the tender duly signed by the Tenderer as a token of acceptance of the Terms and Conditions

8. OWNERsmp OF VEmCLE AND PAYMENT OF TRANSPORTATION CHARGES (i) List of Trucks owned /leased by the tenderer with up to date documents (Xerox copies duly attested shall be furnished along with the tender). (ii) The Tenderer should have minimum Three (3) Trucks standing in his own name/leased in favour of Firm only 'to avoid interruption in feeding programme during Strikes/Bandh or any unforeseen reasons in future. (iii) Payment shall be made against submission of bills on completion of delivery of stock as per orders of the CollectorlD.1. of Schools, Sundargarh on production of printed serially numbered delivery Challan duly acknowledged by the recipients of stock to whom it concerned as per approved rates incorporated in the agreement. The CollectorlDistrict Inspector of Schools concerned shall have the right to deduct and adjust any amount found recoverable towards loss and damage or both. No advance payment will be made. (iv) The successful tenderer shall have to enter into registered agreement with.-.. ',. the Collector, Sundargarh as per Terms 0and Conditions within 15 (Fifteen l)ays) of the intimation of acceptaj1ce of the Tender. (v) The authority may terminate the contract for violation of Terms of Agreement or for any other reasons to be recorded in writing. NATURE OF WORK AND PAYMENT 9. The work is to be carried out with due diligence and in accordance with the instructions to be issued from time to time by the School & Mass Education Department/Collector/District Inspector of Schools, Sundargarh. 10. The bills will be submitted at least once in a month to the District Inspector of Schools, Sundargarh on completion of delivery of stock as per orders of the competent authority on production of printed serially numbered delivery challanduly acknowledged by the recipients of stock to whom it concern with other documents for payment of dues. The District Inspector of Schools, Sundargarh shall have the right to deduct and adjust any amount found recoverable towards loss or damage of the foodstuff. 11. The Collector may terminate the contract for violation of terms of agreement. 12. The Payment shall be made as per approved rate incorporated in the agreement. 13. No request for enhancement of approved rates shall be entertained during the term of agreement. 14. The successful Transport Agent should carry, weighing scale and its devices in his cargo and give delivery of food stuff to School points on proper weighment in presence of Headmaster/President SMC / SHG as the case may be.
