SECTION 2:2 KEY IN YOUR {USER ID} AND MANDATORY DETAILS 9 * Complete mandatory details.

SECTION 2:3 SELECT YOUR PREFERRED COMPANY PARTICIPATION LEVEL 10 You can chose between two participation levels in Cisco Connect Rewards program: 1. Partner AM (PAM) level Participation: If you opt to join the program at Partner AM Level (PAM), all points earned will be first be awarded into the Company Account. The program administrator as appointed by your goodself will then re-allocate the points to the individual Account Manager s accounts. The amount of points allocated to the individual Account Manager s account or retained at the company account will be as defined by your company s policy. 2. Company level Participation: If you opt to join the program at the Company Level, ALL points earned will be awarded into the Company Account. The company account will be managed by the program Administrator.

SECTION 2:3 SELECT YOUR PREFERRED COMPANY PARTICIPATION LEVEL 11 If you choose Partner AM (PAM) level Participation: 1. Please select your digital signature 2. Please decide the following: a. Administrator manages 1 account: company account only and no personal account b. Administrator manages 2 Accounts: company account and his/ her personal account 3. Please decide the point allocation method. There are two methods which your account manager(s) can be incentivized by the program: a. Points allocation based on Auto Percentage Allocation The Administrator will need to set up the percentage breakdown of allocating points to each Account Manager s account and to the company account. This is a one-time setup and the percentage breakdown will apply to all subsequent points uploaded. The setup will remain in effect until the Company makes another change to the configuration. Please note in the event that there is no percentage allocation of points being assigned, points will be automatically awarded to company account.. b. Manual allocation of Points All points will be firstly award to the Company Account. The company appointed administrator is required to login to the website and manually allocate points to each of the Account Managers out from the company account. Every month, a monthly point statement will be emailed to the administrator notifying to perform the manual point allocation.

SECTION 2:3 SELECT YOUR PREFERRED COMPANY PARTICIPATION LEVEL 12 If you choose Company Level participation: (Note: Company Administrator 1 company account only, there is no personal account for him/ her)

SECTION 2:5 PRINCIPAL ACCEPTANCE LETTER 14 In the event that Partners employees apply to join Cisco Connect Rewards program at. The company principal is required to complete an electronic Cisco Connect Rewards program agreement letter consenting to the company to join the Cisco Connect Rewards program. In the agreement letter, the principal would determine the company participation level in the program. When the principal selects AM level participation, the principal would also decide how points would be distributed to account managers (by manual point allocation or by Percentage distribution).
