Cmpliance Checklist fr Strm Water Requirements at Cnstructin Sites This cmpliance checklist summarizes Strm Water Management requirements fr Cnstructin prjects which invlve demlitin, clearing, grading and excavatin n land areas equal t r greater than ne acre. This checklist is used by the Office f Envirnmental Health and Safety (OEHS) t evaluate cmpliance with Strm Water requirements specified in State Water Resurces Cntrl Bard( SWRCB) Order N. 2009-0009-DWQ fr cnstructin activities. Training prgram Recrd keeping Nn-strm water management A nn-strm management describing all nn-strm water discharges and hw they are eliminated, cntrlled, treated, r reduced t the extent feasible. Cnduct visual inspectin fr nn-strm water discharge quarterly. Ensure that visual bservatins dcument the presence r evidence f any nn-strm water discharge, pllutant characteristics (flating and suspended material, sheen, disclratin, turbidity, dr, etc.), and surce. Ensure that Risk Level 2 and 3 dischargers cllect samples fr all nn-strm water discharges frm all discharge 2

pints and send all samples t a labratry certified by the State Department f Health Services. Nn-visible pllutant mnitring Ensure that a nn-visible pllutant mnitring plan includes a list f the name and telephne number f the qualified persn(s) assigned respnsibility fr cllecting sample (s). Ensure that nn-visible pllutants are listed in the plan. Ensure that a name, address and phne n. f the labratry are included in the plan. Ensure that a sampling kit must be n site. Ensure that the samples are cllected during the first tw hurs f discharge frm rain events that ccur during business hurs and which generate runff. Mnitring (1) Risk Level 1 Requirements Ensure that a mnitring prgram includes a list f the name and telephne number f the qualified persn(s) assigned respnsibility fr pre-strm, pst-strm, and strm event BMP inspectins. Ensure that all inspectin, maintenance repair and sampling activities at the prject lcatin shall be perfrmed r supervised by a Qualified SWPPP Practitiner (QSP) r an emplyees trained by QSP. Is there a recrd available fr review which indicates mnthly site inspectins as pursuant t LAUSD Strm Water Pllutin Preventin, Specificatins 01330? Is there a recrd available fr review which indicates weekly site inspectins as pursuant t SWRCB Order N. 2009-0009-DWQ? Is there a recrd f site inspectins cnducted befre and after each qualifying strm event? (Inspectin shall be cnducted at all discharge lcatins within 48 hurs after each qualifying strm event). Is there a recrd f site inspectins cnducted each 24-hur perid during extended strm event? 3

Is there a recrd f site inspectins cnducted fr quarterly nn-strm water discharge? Ensure that an inspectin checklist includes the fllwing: Inspectin date and date the inspectin reprt was written. Mnthly ( ), weekly ( ), pre-strm ( ), pst-strm ( ), during strm ( ), quarterly nn-strm water discharge ( ). Weather infrmatin including beginning and end f strm, and amunt f rainfall (inch). A descriptin f any BMPs evaluated and any deficiencies nted (Repairs r design changes t BMPs within 72 hurs). Observatin f all BMPs during inclement weather (if access is safe and pssible). Crrective actin required, including any SWPPP amendments and implementatin dates. Inspectr s name, title and signature. (2) Risk Level 2 Requirements Ensure that a mnitring prgram includes all the requirements fr Risk Level 1 plus the fllwing: Ensure that a QSP develp a Rain Event Actin Plan (REAP) 48 hurs prir t any likely precipitatin event (Likely precipitatin: 50% r greater prbability f prducing precipitatin). Ensure that REAP includes the fllwing: Site address. Calculated risk level (2 r 3). Infrmatin n site strm water manager including the name, cmpany, and 24-hur emergency telephne number. Infrmatin n ersin and sediment cntrl prvider including the name, cmpany, and 24-hur emergency telephne number. Infrmatin n strm water sampling agent including the name, cmpany, and 24-hur emergency telephne number. Activities assciated with each cnstructin phase. Trades active n the cnstructin site during each cnstructin phase. Infrmatin n trade cntractr. Suggested actins fr each prject phase. Cllect strm water grab samples frm all discharge pints where strm water is discharged ff-site. At a minimum, f 3 samples per day f the qualifying event (prducing precipitatin f ½ inch r mre at the time f discharge) and analyze fr ph and turbidity n-site (Numeric Actin Levels: ph 6.5 8.5, turbidity 250 NTU). (3) Risk Level 3 Requirements Ensure that a mnitring prgram includes all the requirements fr Risk Level 2 plus the fllwing Ensure that the sample shall nt exceed the Numeric Effluent Limitatins (NEL): ph 6.0 9.0, turbidity 500 NTU. Cnduct biassessment mnitring if a ttal prject-related grund disturbance exceeds 30 acres and the 4

prject directly discharges int receiving water. Obtain any upstream/up-gradient receiving water samples and any dwnstream/dwn/gradient receiving water samples frm representative and accessible lcatins if the site exceeds an NEL and has a direct discharge int receiving waters. D. Numeric Actin Levels (NALs) Exceedance Reprt (1) Have yu exceeded NALs? (2) If yes, submit all strm event sampling results electrnically t the State Water Bard n later than 10 days after the cnclusin f the strm event. (3) If yes, has Reginal Water Quality Cntrl Bard (RWQCB) requested t submit NAL exceedance reprt? E. Numeric Effluent Limitatins (NELs) Vilatin Reprt (1) Have yu exceeded NELs? (2) If yes, submit all strm event sampling results electrnically t the State Water Bard n later than 5 days after the cnclusin f the strm event. (3) If yes, have yu submitted NEL vilatin reprt t the State Water Bard within 24 hurs after the NEL exceedance has been identified? F. Actin Treatment System (1) Is the site deplyed Active Treatment System (ATS)? (2) If yes, ensure that the site cnducts and meets the fllwing: Daily n-site visual mnitring f the system. NEL: 10 NTU Daily flw weighted average and 20 NTU Any Single Sample Prper persnnel training. G. Training (1) Have cnstructin site persnnel been trained pursuant t SWRCB Order N. 2009-0009-DWQ? H. Recrd Keeping (1) Ensure that all recrds f strm water mnitring infrmatin and cpies f all reprts are retained fr a perid f at least three years. These recrds include: Annual reprts Date, place and time f facility inspectins, sampling, visual bservatin (inspectins), and/r measurements, including precipitatin. Individual(s) wh perfrmed the facility inspectins, sampling, visual bservatin (inspectins), and r measurements. Date and apprximate time f analyses. Individual(s) wh perfrmed the analyses. Summary f all analytical results frm the last three years, the methd detectin limits and reprting units, the analytical techniques r methds used, and the chain f custdy frms. Rain gauge readings frm site inspectins. Quality assurance/quality cntrl recrds and results. 5

Nn-strm water discharge inspectins and visual bservatin (inspectins) and strm water discharge visual bservatin recrds. Visual bservatins and sample cllectin exceptin recrds. Recrds f any crrective actins and fllw-up activities that resulted frm analytical results, visual bservatin (inspectins). I. Nncmpliance Reprting (1) Ensure t reprt any nncmpliance issues t OEHS. J. Annual Reprting (1) Is the Annual Reprt prepared and electrnically submitted t SWRCB by September 1 f each year pursuant t SWRCB Order N. 2009-0009-DWQ? (2) Ensure that an Annual Reprt cnsists f the fllwing: Summary and evaluatin f all sampling and analysis results, including cpies f labratry reprts. Analytical methd(s), methd reprting unit(s), and methd detectin limit(s) f each analytical parameter. Summary f all crrective actins taken during the cmpliance year. Identificatin f any cmpliance activities r crrective actins that were nt implemented. Summary f all vilatins f the General Permit. Names f individual(s) wh perfrmed the facility inspectins, sampling, visual bservatin (inspectins), and/r measurements. Date, place, time f facility inspectins, sampling, visual bservatin (inspectins), and/r measurements, including precipitatin (rain gauge). Dcumentatin f all training fr individuals respnsible fr all activities assciated with cmpliance with this General Permit. Dcumentatin f all training fr individuals respnsible fr BMP installatin, inspectin, maintenance, and repair.
