JOB DESCRIPTION POST: Patient Flow Manager BAND: 7 LOCATION: ACCOUNTABLE TO: REPONSIBLE TO: Altnagelvin Area Hospital Assistant Director of Acute Services General Manager, Unscheduled Care JOB SUMMARY The post holder will be responsible for providing an effective bed management service for Altnagelvin Hospital to ensure that patients receive high quality care and treatment and thus promoting a positive experience of care. To deliver on this responsibility, the post-holder will be required to facilitate the smooth journey of all patients admitted to Altnagelvin Hospital by ensuring that there are no delays in patients being transferred into the most appropriate beds. Other responsibilities encompassed within this role includes: To identify and unblock bottlenecks inhibiting patient flows. To identify patients likely to be discharged on the following day To monitor performance in relation to Estimated Date of Discharge To maintain an accurate daily bed state. The Patient Flow Coordinator will be required to develop key relationships with the following members of the multi-professional team: Ward Sisters/Charge Nurses Hospital Consultants General Practitioners N.I. Ambulance Service Infection Prevention and Control Team Diagnostic Services Social Workers Community Nursing teams Rapid Response Nursing Team

KEY RESULTS AREAS 1. Service Delivery 1.1. Establish good working relationships throughout the hospital with key staff members, including Ward Sisters/Charge Nurses and Service Managers, Heads of Departments, Medical Staff, Diagnostics Services and Allied Health Professionals. 1.2. Manage the utilisation of beds within Altnagelvin Hospital in accordance with formal bed utilisation policy and in collaboration with Medical and Nursing staff on wards where patients are required to outlie in other wards. 1.3. Work with clinical teams in ensuring that all patients requiring admission are placed in the most appropriate clinical environment available within the 4 hour standard. 1.4. Work in close collaboration with all disciplines to ensure that the discharge/transfer of patients is achieved in a way that meets the needs of the patient and complies with Priorities for Action targets. 1.5. Ensure delivery of Priorities for Action targets and subsequent action plans. 1.6. Contribute to the implementation of the Major Emergency Plan within Altnagelvin Hospital when required, and co-ordinate activities from the Incident Control Room until relieved by a member of Corporate Management Team. 1.7. Report relevant issues relating to clinical care and/or staffing to Ward Managers and Services Managers as required. 1.8. In conjunction with relevant Service Managers and Ward Sisters/Charge Nurses, ensure that all staff is familiar with and adhere to Trust policies relating to the management of the hospital environment. 1.9. Investigate accidents and untoward incidents, as appropriate and report to the relevant manager so that appropriate action plans to prevent

recurrence can be agreed. For major accidents/incidents inform the Senior Manager/Service Manager immediately. 1.10. Attend regular meetings with the Unscheduled Care Co-ordinator. 1.11. As required respond to patients complaints, learn lessons and implement improvements as a result of such complaints. 1.12. Monitor and regularly review the flow of emergency and elective patients to improve productivity and efficiency. 1.13. Work with the Northern Ireland Ambulance Service to achieve maximum benefits in respect of patients transport. 1.14. With the introduction of new IT systems you will be required to have a skilled knowledge base to maintain a live and up to date data system e.g.flow, PAS, Patient Centre, Sectra Labs, Symphony etc. 2.0 Quality 2.1 Ensure that the individual needs of the patients and their carers, are at the core of the way Altnagelvin Hospital delivers health and social care services. 2.2 Ensure that practice and service developments are underpinned by the most up to date evidence based practice and research. 2.3 Ensure public involvement of users and carers in the planning of services. 2.4 Actively seek and encourage feedback from patients and their families/ carers to enable continuous improvement and monitor service improvement. 2.5 Work in partnership with the Trust Governance Team to support the quality agenda. 2.6 Work in partnership with the Clinical Leads to ensure robust clinical governance and risk management systems are in place, ensuring corrective action plans are developed, implemented and reviewed. 3.0 PROFESSIONAL RESPONSBILITIES 3.1 To place patients safely from A&E/AMU/ Surgical Assessment into the most appropriate clinical area that reflects their need. Work closely with the medical and nursing teams within Altnagelvin Hospital, taking into account the requirement to place elective patients.

3.2 To lead on planning to ensure that beds are available for admissions as early in the day as possible. 3.3 Actively promote the service by being visible, helpful and proactive at all times. 3.4 Act on any problems relating to bed management, transfer and discharge. 3.5 Work closely with Directorates updating placement policies and guidance for admission and discharge. 3.6 To establish and develop effective working relationships with ward staff and all personnel involved in the admission, transfer and discharge of patients. 3.7 To be actively involved in audit projects relating to bed management and discharge planning. 3.9 To provide education and training programmes to all staff at ward level in collaboration with the Primary Care and Older People Directorate about the importance of discharge planning and the bed management process. 3.9 Identify the training needs of individual members of the team and ensure that all Trust and Professional standards of training are met in line with the Knowledge and Skills framework. 3.10 Participate in mentoring and teaching programmes. 3.11 Monitor and maintain the record of training and development for each member of staff in accordance with Trust policies. 3.12 Ensure that you maintain your individual professional portfolio and keep abreast of all changes within the nursing profession. This post requires that you attend yearly mandatory training. 3.13 Infection Control and Prevention is an essential component of the job and therefore you are required to attend regular updates. 4.0 PEOPLE MANAGEMENT 4.1 Liaise with the Ward Managers, Services Managers/Divisional Nurses and Heads of Department as appropriate to ensure that the delivery of patient services is efficient. 4.2 Initiate and participate in Clinical Audit projects as required to ensure safe delivery of care in the patient s journey. 4.3 Supervise, teach, advise, and support qualified and unqualified staff as necessary and, where required, participate in staff induction, in-service training and appraisal.

4.4 In conjunction with the relevant Ward Managers identify training and education needs of staff. 4.5 Initiate agreed Trust disciplinary Rules and Procedures, where discipline is breached and take the appropriate action within defined authority. 4.6 Participate in selection and recruitment of staff as required. 4.7 Promote good multidisciplinary team working, good interpersonal relationships and communications.
