While every effort is made to maintain accuracy, all information contained is subject to change without notice. 1. AGENCY 1.1 Contact details Our Callao office handles all port agency requirements for passenger vessels and acts as a Head Office for all Peruvian ports. Freight destined to a vessel should be marked Ship stores in transit for MV c/o Milne Servicios Maritimos, and send to above address with forwarding details, a copy of the AWB must be sent by fax in advance. Correspondence addressed to ships should clearly show the vessel s name. 1.2 Identification Passes Agency staff are issued with laminated identification passes. It is at the discretion of each line to issue their own pass to the Agency staff and who are coming onboard on a regular basis in Callao and in other ports where the vessels are calling.

DOCUMENTATION The following sections outline the general procedures involved in ensuring a smooth inward clearance of a ship. These procedures sometimes vary locally, tolerance must therefore be exercised with the demands of various local Authorities in order to avoid souring the excellent relationships that our Principals and we have established with the port officials over many years. 2.1 Advance Documentation IMPORTANT: Three days prior to vessel's arrival at the First Peruvian Port as well as to the turnaround port (Callao) the vessel must sent, a PAX Nationality breakdown list, Crew list, disembarking passenger list as well as a preliminary embarking passenger list to our e-mail address operationsperu@iss-shipping.com. We in turn will forward same to the Immigration Headquarters and to the Passenger terminal and Port Authorities. 2.2 Arrival Documentation Upon arrival at Peruvian ports the following documents should be ready to be presented to the port officials : Port Captain - Arrival Statement. 1 - Ship s Certificates & Navigation Documents Control. This must be kept until arrival at the next port. The following documents are to be required by the Port Agent in order to prepare the clearance at each port, the documentation must be delivered at least 1 hour before sailing 5 Crew lists 5 Passenger lists The port clearance will be signed and stamped by Port Captain 3. IMMIGRATION REQUIREMENTS 3.1 Passengers in Transit Passengers can go ashore using the vessels ID card. Immigration Authorities do require all passports to be collected and kept by the purser s office 3.2 Passengers Disembarking The clearance of disembarking passenger will be done at the Ship 3.3 Passengers Embarking The Only requirement for embarking passengers is to have a valid Uruguayan Visa if required.

EMBARKING/DISEMBARKING CREW All crew members embarking in Uruguay must have a valid Seaman s Book and must comply with the Immigration Formalities 4.1 Crew Embarking Joining crew must hold, besides a valid passport, the corresponding valid Seaman s Book. In order to obtain from Immigration the permission for joiners to enter into our Country without Visa the following information must be provided, at least 72 working hours before they leave from their respectively Country: Full name, Seaman s Book numbers, Date and Place of Birth and full flight schedule. Once the permission issued by Immigration, it will be sent to Airlines Companies involved on crew transportation, who send OK TO BOARD in order to assure embarkation of crew. 4.2 Crew Disembarking Only Seman s Book/passport and Air Tickets will be required. Then the Agents will complete all formalities with Immigration. 4.3 Crew in Transit No restrictions 5. PORT REGULATIONS 5.1. Port Costs The calculation of the port costs throughout Peru are based on the following standard charges. The pilot and the tugboats must be arranged by the Port Agent in advance. 6. COMMUNICATION LOCAL TIME 6.1 ETA Message The ETA message for all ports should be sent to Callao office at least 24 hours prior to vessels arrival. Local time should be used. The message should include ETA pilot station / Anchorage and preferably which side the ship will berth. 6.2 Communication The Port Control TRAMAR works in channel 16 and they must be contacted 20 miles before entering Callao asking authorization. The pilots are working on channel 67 6.3.Local Time The local time in Peru is GMT 5.

Notes of 10, 20, 50, 100 and 200. Coins of 5, 10, 20 and 50 cents. The exchange rate to major foreign currencies is fluctuating and can be advised upon request. 7.2 Exchange Travelers Checks are only accepted by Banks, hotels, and selected shops, as well as the widely recognized credit cards Diners, Visa, MasterCard, American Express. Normal banking hours are Mon. to Fri. 0900 to 1600. 7.3 Postage When Peruvian postage stamps are ordered to be placed on board for sale to passengers, care should be exercised to avoid excessive stocks, since stamps cannot be returned or exchanged. ISS Cruise Services - Peru / General Information - Port Facilities Page 7 of 7 9. VACCINATIONS & SPECIAL HEALTH REGULATIONS 1. YELLOW FEVER AND / OR OTHERS Local Authorities adopted International Health Organization regulations, basis on these Yellow Fever Vaccination card have to be updated for all crew members in a compulsory way. For passengers, Yellow Fever vaccination is compulsory for all visitors coming or leaving from / to endemic areas. Maritime Health Authorities will require the Yellow Fever Vaccine to the crew members and to the guests going on overland tours to Peruvian Jungle area (Machu Picchu, Cuzco and Iquitos are included in this group). Two accurate Vaccination Lists must be issued by the ship s Doctor: one for the crew members and a second one for passengers going/coming to/from endemic areas. Those allergic to the vaccine must be holding a doctor s declaration in this sense. 2. SARS The SARS inspection is no more performed at Peruvian Ports.
