- Nurse Family Partnership GENERAL STATEMENT OF DUTIES Performs technical and advanced practice nursing leadership and work in the coordination and administration of an assigned public health nursing program(s). Coordinates agency and community activities and assesses community needs relative to the services provided by the Tri-County Health Department. REPORTING RELATIONSHIPS Supervision Received: General supervision is provided by a Nursing Manager, Associate Director of Nursing or Director of Nursing. Supervision Exercised: Provides direct and indirect supervision through second-level supervisors, employees, and functions within selected nursing programs. Oversees policy and procedure standards and evaluates performance of direct reports and program operations. Recommends and administers actions impacting employee pay and status, hiring and discipline. May act as a resource or expert in program areas providing technical guidance and education to less experienced staff. DISTINGUISHING CHARACTERISTICS The is a unique position that provides overall nursing leadership and supervision to selected nursing program(s) within the agency and the community. ESSENTIAL DUTIES Any one position may not include all of the duties listed, nor do the listed examples include all tasks, which may be found in positions of this class. The Tri-County Health Department retains the right to modify or change the duties or essential and additional functions of the job at any time. Oversees staff development of all nursing program personnel, including orientation, inservice training. Coordinates and conducts staff trainings, instruct staff in the methods, techniques, procedures, and protocols utilized in assigned programs. Ensures compliance with program guidelines and objectives. Assesses community needs, assists with development of program objectives, and evaluates outcomes. Page 1

Responsible for monitoring compliance with program contracts, monitors budgets, expenditures and billing processes of assigned program. Assists in program evaluations and makes recommendations to nursing administration concerning planning activities; recommends program and policy changes. Participates in local, state and community boards and other community outreach activities to promote understanding and support of public health nursing programs and to promote local, state and national standards. Collaborates with professional and community groups to develop goals and increase access to public health care programs. Coordinates and integrates program activities with other nursing programs, TCHD divisions, allied agencies, service providers, and community groups. Provides consultation, leadership and management in specialized program areas. Provides technical expertise to TCHD staff in the interpretation of program policy, procedures, and protocols. Participates in nursing, emergency preparedness and other program staff team conferences in order to provide expertise and coordination of activities. ADDITIONAL DUTIES Specific scope of work related to the implementation of the Nurse Family Partnership program to assure fidelity to the model which is essential to secure and maintain program funding. Administrative Responsibilities Staffing and Education Oversees the recruitment and hiring of all nurse home visitors and administrative data support staff ensuring that they can implement the NFP philosophy and model. Ensures that all staff, including supervisor, attend all required NFP education sessions as outlined in the orientation materials and your contract with the Nurse- Family Partnership National Service Office. Assesses and develops strategies to address knowledge and skill gaps of nurse home visitors and support staff. Provides adequate orientation, professional development opportunities and study time to remediate gaps in necessary knowledge and skill. Assigns client caseloads and monitors nurse home visitor s ability to build and manage caseload. Provides useful developmental feedback, and confronts and develops a plan with staff to address performance and resolve issues. Builds confidence and skill of nurse home visitors. Provides supervision and evaluation of nurse home visitors and administrative data support staff (including annual leave, annual review, disciplinary action, etc.). Page 2

Budget and Contracts Becomes familiar with and actively participates in the preparation and monitoring of the NFP budget to ensure appropriate use of funds. Plans and expends budget to ensure compliance with fidelity to NFP model and contract agreements. Understands and actively participates in monitoring NFP contract with the agency. Is proactive with local, state, and national issues that may affect sustainability of program. Community awareness, support and referrals, and referral systems Based on agency policy, actively participate in, chair, or coordinate all NFP community coalition/advisory board meetings. Develops and maintains community referral sources for the NFP Program. Develops and implements external and internal systems to assure timely receipt and disposition of referrals to and from the NFP. Develops and implements strategies for using local print and electronic media to showcase the program. Utilizes agency policies and procedures or develops needed policy and procedure to guide implementation. Working within agency and within budget and contract specifications, ensures that NFP staff have infrastructure of space, computers, furniture, phones, etc., in place as quickly as possible. Develops and maintains appropriate materials for the community and client. Oversees the allocation and design of space for NFP materials to be stored. Ensures that all NFP forms and other materials are ordered in a timely manner, organized and available for use by home visitors. Develops an adequate budget for appropriate materials, gifts, and incentives for program clients. Order supplies as necessary to maintain inventory. Manages organizational structure and systems adequately so that home visitors may stay focused on clients. Quality Improvement and Evaluation Oversees and ensure timely and accurate data input and utilization of the web- based reports and other reports available through NFP. Reads and interprets reports; shares appropriately with team and partners. Ensures implementation of quality improvement strategies. Develops methods for ongoing assessment of staff development needs; compare available data to structure appropriate continuing education and guidance of staff. Provides routine surveillance of potential client care incidents. Evaluates implementing agency outcomes and interventions. Create and Maintain NFP Culture (Examples) Ability to maintain relationships based on trust, support and growth with team colleagues and community members. Page 3

Ensures that supervision of NFP nurse home visitors utilizes principles and concepts of motivational interviewing, coaching and reflective practice (modeling therapeutic relationships, change theory, self efficacy, client centered, strength based, and solution focused approaches). Becomes familiar and comfortable with principles and concepts of motivational interviewing, coaching and reflective practice. Develops and implement regular reflective practice supervision within the program and conduct field visits, both with frequency recommended by NFP. Ensures that own reflective practice and ongoing professional and personal development needs are addressed. Provides regular case conferences and team meetings as recommended by NFP. Ensures rewards and incentives are aligned with goals of program. Effectively assists nurse home visitors in analyzing complex problems and case situations. Experience (desired) Worked in public/community health, including home visitation. Provided maternal/child health services, particularly in community setting. Program management and one-on-one supervision. Worked with diverse populations, communities, and organizations. HIPAA ST ATEMENT Maintain sensitive & confidential client information according to the HIPAA policy. EMERGENCY PREPAREDNESS AND RESPONSE DUTIES Employee responds, as required, to support public health emergencies, incidents and events. Employee participates in all exercises and drills on emergency preparedness and response, as required. Completes trainings identified as appropriate for this level employee. KNOWLEDGE, SKILLS, OR ABILITIES Education and Experience: Requires the minimum of a Bachelor s Degree in nursing from a school accredited by the National League for Nursing Accrediting Commission or the American Association of Colleges of Nursing. Master s Degree in Nursing preferred and/or experience in specialty area. Requires a minimum of three years progressively responsible public health nursing experience including supervisory experience. Special Qualifications: Licensed to practice as a registered nurse (RN) in the State of Colorado. Possession of a valid driver s license. Knowledge of: Principles, practices, techniques, and protocols related to community health nursing and its application in a public health program. Page 4

Act as an official representative of TCHD in a professional manner with local and national agencies and organizations. Maintain sensitive and confidential client information according to the HIPAA policy. EQUIPMENT USED Standard office equipment including, but not limited to, personal computer and associated hardware and software, telephone, copier, calculator, and fax. Standard medical equipment such as blood pressure cuff, weight scales, stethoscope, thermometer and syringes, vaccines and blood borne pathogens kit, assessment and vehicles. WORKING CONDITIONS AND PHYSICAL REQUIREMENTS Work is primarily conducted indoors with occasional travel to other TCHD locations or outside agencies to provide program services or attend meetings. Incumbent experiences routine exposures and hazards of a typical office environment. May require frequent travel between office sites or to locations in the surrounding community for conducting educational presentations and attending meetings. Page 5

1. Requires sufficient hearing and speech ability to communicate verbally in response to inquiries, complaints, and to speak to groups of people in an educational presentation. 2. Requires occasional light lifting/carrying under 15 pounds. 3. Occasional use of automobile, usually no more than one hour at a time. 4. Requires manual dexterity to operate computer equipment and clinical equipment.
