Describe Population Served and Annual Number of People Served: The Clinic s patients are uninsured, because they do not qualify for or cannot afford various insurance options, such as Medicaid, Medicare, or subsidized health Insurance plans through the marketplace. We also provide healthcare to underinsured people who are defined as those who do have health insurance, but the plans offers minimum standards of coverage including high deductibles, large co-pays, and/or high out-of- pocket costs, all of which inhibit them from seeking health services. Note: some plans have deductibles of up to $12,000 for a family of two. From June 1, 2014 through May 31, 2015, The Clinic provided 8,173 patient visits and 4,007 laboratory tests in addition to coordinating the provision of 1,000 diagnostic procedures through Paoli Hospital s charity care program and over $1,000,000 worth of brand name prescription medicines through our Pharmacy Assistance Program. There are still a substantial amount of individuals in Chester County without health insurance. According to a recent report by Independence Blue Cross, 40,000 people in Chester County are still without health insurance. Rev. 06/2015
Many of our patients fall within 150% of the poverty level. For that reason, The Clinic understands that people living in poverty use less healthcare services than most Americans. In fact, people living in poverty are less likely to have recent physician contact, receive preventive care such as immunizations or cancer screening, or to avoid hospitalization for serious conditions by receiving preventive, office-based care. People living in poverty have a higher prevalence of disability and chronic illness and shorter life expectancy than those at higher income levels. The Clinic continues to The Phoenixville region is home to a large growing Hispanic, often undocumented, population. In Phoenixville, the proportion of Hispanics rose from 3 percent in 2000 to 7.4 percent in 2010 (more than doubled). The Hispanic population in Chester County rose from 3.7 percent in 2000 to 6.5 percent in 2010. In Pennsylvania, Hispanics made up 3.2 percent of the population in 2000 and 5.7 percent in 2010. The Clinic provides great relief to the Emergency Room of Phoenixville and Paoli Hospitals as low income and little to no insurance residents typically use the emergency room for primary healthcare. The Borough of Phoenixville is one of only three (West Chester Borough and City of Coatesville are the other two) urbanized areas in Chester County. Theses urbanized areas provide a concentration of populations who are of low income, uninsured or under-insured. According to the US census, 6.9% of residents live below the poverty level (2008-2012) in the Phoenixville Borough (approximately 11,400 people). The cost or lack of transportation continues to be a prevalent issue for many Clinic patients. 40-50% of current patients served at The Clinic present with the medical condition of diabetes and pre-diabetes. 25% of patients who receive medical care also present with mental and emotional health issues. 92% of patients receiving care at The Clinic are between the ages of 18 and 64 years. Most recent data indicates that 21% of patients served in 2014 identified as Hispanic/Latino. 10% of patients seen at The Clinic are walk-ins - meaning they are seeking treatment without a scheduled appointment. Due to the numerous struggles of a typical Clinic patient (which may include homelessness, mental illness, substance abuse, inability to makes ends meet, inability to speak English, illiteracy, etc.), many patients are unable to plan for the future and address complicated life issues that lead to health problems. II. CHESTER COUNTY COMMUNITY FOUNDATION GRANT PROPOSAL NARRATIVE Provide clear, concise information. 3 pages maximum. Organization s history, goals, key achievements and distinctiveness Founded in 2001, The Clinic began as the dream of two women, Dr. Lorna B. Stuart and the Reverend Marie Z. Swayze. The goal was to create a medical center that would provide comprehensive health care to people who did not have medical insurance. These two women secured a gift of $200,000 from an anonymous donor and in collaboration with St. Peter's Episcopal Church in Phoenixville, they requested the Church Vestry's permission to accept the gift as 'start-up money' for The Clinic. An additional $300,000 was raised through various contributions from the community to furnish and complete the medical facility. Repairs were made to the former Rectory of St. Peter's, which the Church contributed to provide a home for The Clinic. St. Peter's provided a generous building lease for $1.00 per year for the first ten years of operation. Through donations of time/talent The Clinic began operation in 2002 and recently celebrated the milestone of providing 110,000 patients visits since its inception. The Clinic is currently in the third year of a three year lease, which was increased to $18,000 per year in 2013 from $1/year the previous 10 years. We have not yet begun lease negotiations for the next period. Funding request: Description of key initiatives The Clinic is requesting $5,000 for Capacity Building in the areas of Marketing and branding. Specific needs and issues to be addressed Although the Clinic has provided 100,000 patient visits since 2001, there are still a large number of individuals in need of comprehensive medical care. According to Independence Blue Cross, there are still 40,000 individuals in Chester County without medical coverage. The problem is that many of these individuals are unaware of The Clinic as a resource to get comprehensive medical care. It was recommended that The Clinic develop a marketing plan to increase awareness of their services to both individuals living without medical coverage and potential donors. In 2013, The Main Line Chamber of Commerce created a marketing plan for The Clinic, but the plan was not implemented due to limited resources. Currently, The Clinic has no resources to devote to marketing. It is our hope to receive funding to implement the marketing plan which will increase patient visits and connect with more donors to support the mission of The Clinic. Organizational impact if initiative is undertaken How will this grant enhance your organization s capacity? This grant will enable The Clinic to reach more uninsured and underinsured individuals living in Chester County. By marketing within our targeted population, we can increase the visibility of the clinic and identify more individuals in need of comprehensive medical care. In addition, The Clinic will also be able to attract more donors and community partners to help build up development activities. How will this increase in organizational capacity be measured? The increase in organizational capacity will be measured by reviewing statistics of patient visits. Specifically, we will track if the marketing campaign has increased patient visits at The Clinic. These numbers will be tracked after quarterly to determine what strategies had the most impact on the increase in patient visits.
Activities to implement the initiative. Please include a description of the expected activities; timeline and costs to implement the initiative. If external consulting services are required, include the anticipated costs and expertise of the consultants to be hired The Clinic plans to implement strategies outlined in the Marketing Plan written by the Main Line Chamber of Commerce Leadership Class of 2013. These strategies include developing marketing initiative that focus on traditional media, social media, grassroots marketing, and strategic partnership formation. The Clinic plans to create radio, cable, television, and print outreach to traditional news outlets in English and Spanish. The Clinic also plans to implement an active online social media campaign and implement a grassroots marketing plan to attract individuals without access to radio and television. The Clinic will develop strategic partnerships with other organizations and potential donors to both identify new patients and support the mission of The Clinic. Estimated cost for the campaign is $5,000 which will include the cost to launch the marketing strategies in the plan. The Director of Development will lead the team responsible for implementing the strategy and will research cost to perform the tradition, social, and grassroots efforts of the plan. (See Attached Budget Proposal) We anticipate launching the marketing plan according to the following timeline: Phase #1: Meeting with board, staff, and volunteers to announce the funding to perform marketing Estimate Date: January 2016 Activity: Host a focus group to discuss best practices and creative strategies to reach targeted population. Phase #2: Create survey and host a meeting for current patients and community leaders Estimated Date: February 2016 Activity: Host a meeting and gather research from current patients and community leaders on the best ways to market to the targeted population. Offer incentives to attend meeting and complete survey. Phase #3: Launch Marketing Strategies Estimated Date: March 2016 Activity: Implement strategies for marketing plan and information gathered at the focus groups Phase #4: Review Statistics and Write Report Estimated Date: September 2016 Activity: Gather data from medical staff on patient visits to determine impact of marketing strategies Why it is important to fund this now Despite the implementation of the Affordable Care Act, The Clinic is still a critical community resource for the uninsured and underinsured population. According to recent information released by Independence Blue Cross, there are 40,000 uninsured residents in Chester County alone. Our goal is to be a resource to these individuals and to support their need for comprehensive medical care. Now more than ever, we need to market to those individuals that are without coverage. While many of our patients did gain access to insurance, the number of new patients increased by over 10 percent this past year. Many of our patients fall within 150% of the poverty level and we service a growing Hispanic population in the area. The Clinic understands that people living in poverty use less healthcare services that most Americans. In fact, people living in poverty are less likely to have a recent physician contact, receive preventative care, screenings and avoid hospitalization for serious conditions. Currently, 40-50% of our current patients served at The Clinic present with medical condition of diabetes and pre-diabetes, 25% of patients present with mental and emotional health issues, and 10% of patients walk in with no appointment seeking medical care.
How impact and results will be demonstrated Impact for patient services will be demonstrated through statistics of visits and ancillary services provided or coordinated, the number of Latino patients served, and demonstrated use of the translation services in the community in addition to recruitment of multilingual volunteers and staff.
