No HNC 2020 objective for chronic pain/prescription drug abuse prevention, yet this is a priority in our community and therefore, is included in this objective. 4 INTERVENTIONS: SETTING, & TIMEFRAME INTERVENTIONS SPECIFICALLY TARGETING HEALTH DISPARITIES Intervention: Community prevention youth programs Intervention: Community prevention programs that involve middle and high school youth will be offered. Quantify what you will do One community wide prevention strategy plan will be developed and adopted by June 30, 2013 One good neighbor campaign will be launched to train community members in being the watchdog for underage drinking/teen alcohol abuse by June 30, 2013 Three action steps within the community wide plan will be achieved by June 30, Expected outcomes: Explain how A community wide strategy will support policy change across multiple sectors to prevent substance abuse prevention, and in particular, alcohol abuse in youth Intervention: Community Prevention- Project Lazarus Intervention: Project Lazarus Coalition will develop a prescription drug abuse prevention plan and implement the plan Start Date End Date (mm/yy): 06/12- Individuals _X_ Policy &/or 5 Lead Agency: Appalachian Regional Healthcare System Role: lead efforts in organizing the Project Lazarus Coalition and implementing the plan objectives Partners: Appalachian District Role: Assist in development of Project Lazarus plan, including prevention messages in the media and in clinical settings Partners: Watauga Medical Center Role: Adopt new procedures for physicians prescribing drugs that may be linked to prescription drug abuse in the emergency department Partners: NC Access Care of the Blue Ridge Role: implement chronic pain management toolkit for physician practices to use with patients Partners: Project Lazarus Role: Provide training, technical assistance to community partners through the Project Lazarus Coalition July, 2012-June, Quantify what you will do 100% of the physician practices participating in NC Access Care of the Blue Ridge will establish access to the CSRS (registry to see prescribed drugs and patient) will be completed by November 30, 2013 One permanent drop box location will be developed for routine prescription drug drop off will be established by June 30, 2013 Increase the number of prescription drugs collected in a permanent drop box location from 0 to 1000 by June 30, Expected outcomes: Explain how A community wide strategy will support policy change across multiple sectors to prevent substance abuse prevention, and in particular, alcohol abuse in youth
