The current structure when creating Service Provider information requires that an individual record is created for each unique address. This results in many duplicate and inaccurate data records and increased workload during the batch merge processes. This SCR provides the following new Service Provider functionality: Users will have the ability to add/edit/delete multiple addresses for a single Service Provider. An address row is now required for each Service Provider. CWS/CMS has been expanded to allow for Service Provider searches using additional address information. Users will have the ability to end date a Service Provider address. Labels will display in the Service Provider notebook to encourage a search be performed prior to creation. Online and Batch Merge reports will display all addresses that are assigned to a Service Provider record. Online and Batch Merge reports will display a Service Provider Last Edit Date. Address Required Label An Address Required Label alert reminds users to update the address if mandatory data is missing. Service Provider Notebook, Address page At least one address is required for a Service Provider. All addresses are listed in the Address grid. Addresses that have an end date are placed towards the bottom of the grid. All other address are sorted by the creation date in descending order. Address frame Agency Name Other Phone Other Phone Ext Fax Number Address End Date The fields are found within the Address frame and are read-only. The fields are associated with the specific Service Provider Address selected by the user. Client Notebook, Select Service Provider Dialog The dialog has been updated to display two (2) new columns. Agency Address End Date The grid displays a row for every Service Provider Address associated to the Service Provider. When the user makes a selection the Service Provider chosen and the Address will both be associated to the client. The Service Provider Location column was renamed to Address. 6/3/2015 JLM 4
Provider Name and Address Display Users can now select a Service Provider and a specific address associated to the Service Provider. The Service Provider appears in the combo box once for every address they have. Service Provider Search The Service Provider Search dialog in the Search Notebook has been updated with four (4) new fields: Street Number Street Name County of Location (defaults to users County) State (defaults to California) The County of Location is disabled if the State is not set to California The Agency Name can be used to search for an agency entered in the Address and ID page of the Service Provider notebook. The Search Results Notebook, Search Provider Search Results page A new Address End Date column has been added. The Search Results grid displays every address associated to every Service Provider matching the search criteria entered. A Service Provider s address rows do not count against the total number of hits that may be returned during any given search. City and Zip Code columns have been removed. These values reside within the information displayed in the Address column. Service Provider search results first display a row populating only the Name and Agency Name from the ID page of the Service Provider. Clicking on any row associated to a Service Provider takes the user to the Service Provider abstract. 6/3/2015 JLM 6
Generate New Documents A Yes/No error message displays if the user attempts to generate a document that has an inactive Service Provider Address. The error is invoked if any client, via the Associated Services page of the Contact Notebook, is associated with an inactive Service Provider Address attached to the following: San Diego Field Worksheet Court Report Documents A Yes/No error message displays if the user attempts to generate a document that has an inactive Service Provider Address. The error is invoked if an inactive address is associated with any of the following: Automation for County User Defined Template Health and Education Passport Citation to Appear Merge Service Provider Notebook The Merge Service Provider Search Notebook has been updated to show that a Service Provider may have more than one (1) address. This is a manual process that relies on the user to determine what criteria need to be met to consider a Service Provider is a duplicate. When performing the merge, the application keeps all non duplicate addresses. These addresses are recorded to the Primary Service Provider. Two fields have been added to the dialog: Service Provider Address grids display all addresses for Service Provider 1 and Service Provider 2. The user can select the Service Providers to merge via a combo box. Changing the combo box changes the contents of the grid below it. Last Edit Date is a read-only field that has been added to reflect the date the Service Provider was last updated. 6/3/2015 JLM 7
Select Primary Service Provider Notebook Clicking the Merge button on the Merge Service Provider notebook opens the Select Service Provider notebook. The Select Service Provider Notebook now displays a Provider Type list box instead of a Provider Type field. This change allows all provider types associated to a Service Provider to be displayed. A Last Edit Date field has been added that is populated with the date the Service Provider was last updated. A rule has been added to keep all non-duplicate Service Provider Addresses. When duplicate service provider addresses are found, certain fields will bring over data of the second Service Provider if the primary Service Provider does not have that field populated. 6/3/2015 JLM 8
