Prospectus for PG Degree/Diploma Course Admission- 2015 (Management/NRI Quota) I PREFACE The Ministry of Health and Family Welfare, Government of India / Medical Council of India have permitted the Academy of Medical Sciences, Kannur (Pariyaram Medical College) to conduct the following Post Graduate Degree and Diploma courses as shown in para II, at the Academy of Medical Sciences functioning under Kerala University of Health Sciences(KUHS). As per the orders of Government of Kerala and regulations of the Medical Council of India, the Academy of Medical Sciences, Pariyaram has surrendered 50% of the total seats to the Government for allocation under state merit quota. The Academy of Medical Sciences hereby invites applications for admission under management quota (including NRI quota) to various postgraduate courses conducted under the Medical Council of India and the Kerala University of Health Sciences as mentioned in Annexure III. This prospectus contains the rules and regulations applicable for selection and admission for the academic year 2015 to MD / MS / PG Diploma courses at the Academy of Medical Sciences and is subject to modification / addition / deletion as may be deemed necessary by the Institution and also subject to further orders, if any, passed by appropriate authorities/ courts of law. As per the MCI Postgraduate Medical Education Regulations, 2000 9(1) (a), students for Postgraduate medical courses shall be selected strictly on the basis of their inter-se academic merit and as per 9(2), for determining the academic merit, the University / Institution may adopt the following methods: (a) on the basis of merit as determined in the competitive test conducted by the State Government or by the competent authority appointed by the State Government or by the University / group of Universities in the same state; or (b) on the basis of merit as determined in a centralized competitive test held at the national level. As such, the applicants seeking admission to this institution under Management Quota other than NRI Quota must be from the rank list prepared by the Commissioner for Entrance Examinations, Kerala Government, on the basis of the Post Graduate Common Entrance Test 2015. As per the MCI Postgraduate Medical Education Regulations, 2000 9(2) (d), the minimum percentage of marks for eligibility for admission to postgraduate medical courses shall be fifty percent (50%) for candidates belonging to open category and forty percent (40%) for the candidates belonging to Scheduled Castes, Scheduled Tribes, Other Backward Classes etc. III DURATION OF THE COURSE All courses are full time residential courses, the periods of which have been fixed by the MCI The duration of the course including University Examination is 3 years for Degree Courses and 2 years for Diploma Courses. There shall be an exemption of one year in the duration of PG degree courses in respect of candidates who have passed P G Diploma course of two years in the same subject and awarded Diploma certificate on or before 30 th April 2015 [See Annexure 1(c)]. No other reduction in the duration of the course shall be granted. The date of commencement of the course as per the orders of the Hon ble Supreme Court of India and the last date of admission process will be as per the time schedule fixed by the Government of India/Medical Council of India/Supreme Court of India. 2
IV ELIGIBILITY FOR ADMISSION a) Academic: Applicants should have passed MBBS Degree from any of the Universities in India, recognized by the Medical Council of India or equivalent there to and should have obtained registration from Medical Council of India or State Medical Council. Applicants who may complete Compulsory Rotating Resident Internship (C R R I) on or before 30/04/2015 are also eligible for applying, but admission to such candidates will be given only after obtaining registration from Medical Council of India. b) Nativity: Indian Citizens are only eligible for admission (See Annexure I) Note: Applicants who have undergone M.B.B.S. course in the Medical Colleges of Kerala, a certificate revealing the above information from the Principal of the respective Medical College, where he/she has studied will be accepted, as nativity. c) Age: Upper age limit for all candidates will be 45 years as on 30/04/2015. V. FEES Fees for management quota (management open & NRI quota) will be fixed by the government of Kerala subject to approval by the Fee Regulatory Committee/Admission Supervisory Committee. The same will be published later since it has not been fixed by the Government so far. Fees consented at the time of admission shall be paid as and when due. No legal dispute / challenge in this regard will be accepted / entertained after admission. VI HOW TO APPLY- Candidate should submit Institutional application prescribed for admission under management quota a) Application form: The Application form is common for all candidates (including NRI) applying for the PG courses in any subject. Applicant need submit only one application form under one quota. Application form filled up along with necessary documents should be sent by Registered post or handed over in person to the Principal, the Academy of Medical Sciences, Pariyaram, Kannur - 670503 so as to reach or through his messenger / representative him before 5PM on 20/04/2015. Acknowledgement for delivery of application should be obtained from college office for delivery at the office in person by the candidate or his messenger / representative. Separate application is necessary for admission under Management quota as well as Management NRI quota. Applications are available at the office of the Principal from 01/04/2015 onwards. Application can also be downloaded from the website (www.mcpariyaram.com) of the Institution. Application submitted should be complete in all respects and certificate produced after the last date prescribed will not be entertained. Online submission of application is not permitted. 3
Last date of acceptance of application will be 5 PM on 20/04/2015. However application will be available till last date of admission. If seats are vacant, the management will issue application form and admit students till last date of admission, as per rules of admission framed by the Medical Council/Supreme court of India. Seat position: Seat position applicable in 2015 admission is attached separately. (Annexure III) b) Cost of Application form: The cost of application form is Rs.2,500/- (Rupees Two thousand five hundred only). Applications are available at the office of the Principal, the Academy of Medical Sciences, Pariyaram. Application can be downloaded from the institutional website www.mcpariyaram.com also. Those who have download application forms from the web site should remit cost of application form & prospectus by DD for Rs 2500/- in the name of the Principal, ACME along with filled up application form or otherwise the application will be rejected. c) Certificates / Documents to be produced along with the filled up application form: Attested copies of the following documents may be attached: i) M B B S Degree pass certificate from the University concerned / Certificate of equivalency from the Medical Council of India ii) House Surgeoncy (CRRI) certificate. Candidates who are undergoing house surgeoncy / internship in Medical Colleges of the State, should attach a certificate from the concerned Principal to the effect that he/she will complete his/her internship by 30/04/2015. iii) Certificate to prove age (SSLC or Mark list of X Std of CBSE or other proof of age) iv) Mark list of each MBBS Degree examination v) 2 Passport size photographs vi) Nativity certificate from the Village Officer or Certificate from the Principal of college where the candidate studied for the last 6 years. [Annexure 1(a) and 1(b) respectively] vii) Copy of Certificate of permanent registration viii) Application form fee receipt (If application is downloaded from website, DD) ix) Conduct and character certificate from the head of the institution last studied. x) Attested copy of mark list or certificate of State PG entrance examination 2015 xi) Copy of the page of rank list of the applicant published by the Commissioner of entrance examinations. xii) Documents supporting NRI eligibility status of the applicant in the case of NRI Quota (relation to NRI/ employment details of NRI sponsor certified by the Indian Embassy) Admission given shall be cancelled if it is found later that false certificate has been produced or that the admission has been secured by illegal means. Copies of certificates attached are to be attested by a gazetted officer. 4
VII METHOD OF SELECTION Mode of selection/ method of examination a) Government quota: The Selection will be based on merit of PG entrance examination held by the State Government. Seats under government quota will be filled by government (Commissioner of entrance examinations) based on merit of State PG entrance examination conducted by government. b) Management quota: Seats under management quota will be filled on inter-se merit from the rank list of candidates (rank prepared on the basis of State PG entrance examination) who have applied for admission to this Institution under the said quota. Candidates who do not opt for the available seat while on initial counseling will not be permitted to participate in the subsequent counseling. If qualified candidates are not available in State PG entrance exam, qualified candidates in the list of NRI will be taken. c) NRI Seats 15% of seats under management quota will be filled under NRI quota and admission to NRI seats will be through the merit of a competitive examination conducted by the Admission Supervisory Committee. Eligibility to NRI seats will be as per government order published in the said regard. As per GO(MS) No.193/2013/H & FWD dt 22/05/13, an NRI applicant is he / she who depends upon his/her Father /Mother/ Husband/ Wife/ Brother / Sister/ Half Brother/Half Sister working abroad. The rank list of NRI candidates will be published by the Admission Supervisory Committee. Details of seats reserved under NRI quota will be published in the website of the institution and on the institutional notice board also. (Annexure III) VIII RANK LIST Rank list of candidates selected for admission under management open quota will be published on the website of the Institution soon after last date prescribed for the receipt of application. IX ADMISSION Admission will be made following a counseling based on the rank list; to ascertain the candidate s choice of subject and consent to pay the fees. Admission will start from 15/05/15. Actual date of commencement of the course will be announced later. Notice of admission / counseling will be published on the website of the Institution and the candidates shall diligently look for the same. Candidates who do not opt for the available seats while on initial counseling will not be permitted to participate in the later counseling. Tentative dates are shown as in the attachment. 5
Original documents required at the time of Counseling / Admission: While reporting for admission, all the relevant certificates / documents listed below should be submitted in original to the Institution and the same will be returned to the candidates only at the time of leaving the Institution. a) Admit Card and Score Card of the Govt. Entrance Examination b) MBBS Pass / Degree Certificate from the University concerned. c) Marks sheets of the University Examinations of M.B.B.S. course completely. d) Attempt certificate issued by the Principal of the College where the candidate had studied. e) Certificate from the Head of the Institution from where the candidate has passed his M.B.B.S Degree indicating the MCI approval, number and date. f) Completion of Internship certificate. If a candidate has completed his / her Internship from a non-teaching Institution, a certificate to prove the recognition of the center to conduct house surgeoncy may be submitted. g) Registration certificate from the MCI or State Medical Council. h) S.S.L.C. Certificate / Mark list of 10 th Std of CBSE or other proof of age i) Transfer Certificate from the Institution last studied. j) Migration Certificate from the University last attended (if applicable). k) Passport size photographs. (5 Nos.) l) Conduct Certificate from the Principal of the Medical College where the candidate had last studied. m) Originals of State PG entrance examination 2015, mark list and rank list If selected for admission, Demand Draft covering the tuition fees and special fees shall be produced. X. LIQUIDATED DAMAGES & EXECUTION OF BOND i) General Terms: The applicants selected for Post Graduate Degree and Diploma courses will have to execute a bond on Government Stamp Paper worth Rs.100/- at the time of joining the course to declare that he / she shall not discontinue the course after admission and is liable to pay the liquidated damages on vacating the course / college. Similarly, candidates shall execute another clause in the same bond paper to declare that he/she shall pay the fees fixed by Government for the year 2015 for the particular course which has also been informed to him during the course of admission and that he will not challenge the same in the Court after admission (specimen bond attached as Annexure II). ii) Conditions (a) If a Post Graduate student discontinues the P.G course after getting admission he / she shall pay the total fee for the course as liquidated damages. He / She should also refund the stipend / salary already received by him till the period of his studies in the institution. 6
(b) A candidate leaving one course to join another as his / her higher option in this institution itself need not pay the liquidated damages. (c) No Admission shall be made without getting the bond as detailed above executed at the time of joining. (d) Fees once paid will not be refunded under any circumstances except when he is leaving the institution on getting a seat elsewhere on Government merit. XI. CANCELLATION OF ADMISSION As per the MCI Postgraduate Medical Education Regulations 2000 9(3) (ii), there shall be no admission of students in respect of academic session 2015-16 beyond 31 st May 2015 for Postgraduate Courses. Admissions will have to be closed on or before 31st May 2015. If any student intends to discontinue the studies on or before this date, such student should give a notice in writing to the Principal of the college at least on or before 25 th May 2015. If any student discontinues the studies or fails to give notice as stipulated above he/she will be liable to pay the tuition fees for the entire course period. In such cases, certificates obtained at the time of admission will be returned only after payment of pending dues including tuition fees for the entire course. XII. ACCOMMODATION: Limited accommodation only is available. Available accommodation will be provided, subject to the terms and conditions of this institution. XIII. STIPEND Stipend will be paid as per norms of the Institution. XIV. VALIDITY OF THE LIST The rank list (management list & NRI list) will be valid till 5PM on 31/05/2015. The last date of admission will also be 31/05/2015. XV. DURATION, DATE OF JOINING, ATTENDANCE, LEAVE AND RESPONSIBILITIES OF POST GRADUATE STUDENTS, GENERAL CONDUCT etc. i) The duration of the Degree and Diploma course including University Examination will be three years and two years respectively. For the purpose of reckoning the duration of the course, any candidate will be deemed to have joined the course on the date on which the candidate actually commences the academic programme as certified by the Principal. This date should be given as the date of joining in the Post graduate register maintained by the University. He / She will have to complete the total duration of the course before being given the course certificate. 7
ii) Attendance & leave: The candidate should have minimum percentage of attendance as stipulated by the University. All the 365 days of the year are working days for post graduate students. Attendance required by the University and presence of the individual at the Institution for studies may vary. However, they will be permitted to avail casual leave for 20 days in a year, but not more than 10 days at a stretch. Any other leave taken will entail in extension of the course. The candidate will be eligible for leave under exceptional circumstances, supported by medical certificates (subject to verification by a medical board) and recommended by the Head of Department / sanctioned by the Head of the Institution. Those who take leave without prior sanction are liable to be treated as on unauthorized absence. If a student is unauthorisedly absent for more than 10 days, he / she will be terminated from the course and liquidated damages will be levied. Those who are absenting themselves apart from the eligible leave will not be entitled to get stipend for the period of absence. Female students are allowed to avail maternity leave for 2 months once during the course and leave will be sanctioned on production of medical certificate. The date of delivery should fall within the leave period. iii) All PG students are eligible for weekly off for one day. This will be granted by the Head of the Department concerned without affecting the routine functioning of the Department. Weekly off cannot be allowed to accumulate. PG Students are not exempted from working on holidays / sundays. They have to take duty, if duty falls on such days by rotation. Heads of Departments will control the duty rotation. iv) CMEs / Workshops / Conferences and other academic programs conducted by recognized academic bodies are essential part of PG training programme. All PG students may be permitted to attend such programs without affecting the routine of the Department concerned. The Heads of Departments shall sanction special casual leave to PG students provided they apply prior to the programme and the Head of Department is convinced about the genuiness of the programme and utility of the particular course. The student on return from attending the programme should submit attendance certificate to the Head of the Department and make a presentation on same. All postgraduate students should attend one National/Regional conference every year. v) No Post Graduate student shall leave the country without prior sanction of the Principal. Violation of this condition will be viewed seriously and may invite termination of the course. vi) All the Post Graduate Students of Degree and Diploma Courses will be full time residents of ACME. They will stay at the College campus or within 10 KM radius of the institution. PG students staying outside the campus has to show their residential address failing which certificate of residency status will not be given. They will be assigned duty of full time residents and are liable to undertake the duties assigned by individual department or unit where they are posted. G.O(MS)20/09/H & FWD dated 13/01/09 stipulates that all the Post Graduate students are designated as Junior Residents. The Junior Residents have to undertake academic, teaching and training of under graduates and interns as well as clinical duties as assigned to them under Residency Programme. All postgraduate students will make (a) one poster presentation (b) read one paper at National/Regional/State conference and (c) publish one research paper during their course of studies. The Research paper should be published or accepted for publication by concerned or send for publication before appearing the 8
examination. The above academic commitments are in addition to carrying out a research project under a research guide as thesis for being submitted to the University prior to appearing the examination. vii) All postgraduates will maintain a logbook of their training. viii) All resident Post Graduate students of Degree and Diploma courses will be eligible for monthly stipend as per norms of the Institution. ix) All PG students are authorized to use the facilities of the Department library, Central Library, Telemedicine unit and Internet. PG Students are entitled for free medical aid as applicable. x) Transfer will not be given to any PG student during the course period. xi) No scholarships provided for any admission or reservation in admission be given for SC/ST/OEC. xii) Rules and regulations as prescribed should be strictly followed at the college, hospital and hostels. Smoking and use of alcoholic drinks are strictly prohibited. Students are expected to dress up neatly and decently. There will be regular value oriented classes/discussions in the campus. Students are advised to participate in these programs. These are instructions meant for higher learning and their objectives are not only to impart education but also to mold good character. A student handbook will be issued at the time of admission which will contain all the rules and regulations in detail. The students shall strictly abide by these rules and reputations. Students are expected to observe absolute discipline in their conduct and character. Indiscipline of any kind will never be tolerated. XVI. RAGGING Ragging is strictly prohibited. It is a criminal offence. Whoever commits, participate in, abets or aids ragging within or outside the institution shall be liable for deterent disciplinary action coupled with criminal proceedings. The Principal shall report such matter to the civil / police / University / Medical Council of India etc authorities. XVII. Private Practice Post Graduate students shall not engage in any private practices of any sort during the course of study. Violation if any noticed will end in severe disciplinary action. XVIII. FEES (Merit / Management / NRI) The fees payable for the course shall be remitted at the time of admission itself and periodically at the time of commencement of the respective year of the course. Students admitted under merit quota / management quota (open/nri quota) may pay the fees prescribed by the Government of Kerala for 9
the year 2015. The fees once remitted except caution deposit will not be refunded except when he / she is leaving the institution on allocation of a merit seat by the government. SPECIAL FEES: Special fees applicable under each category should be paid at the time of admission. XIX. Sports: All students are instructed to participate actively in the college sports and games activities. XX. PTA: The institution has an active Parent Teachers Association to which PG the students should also contribute to their best. XXI. REDRESSAL OF GRIEVANCES Grievances / complaints if any regarding selection / admission may be brought to the notice of the Principal / Management / Chairman of the college concerned, then and there itself, so that remedial measures will be taken if the grievances / complaints are genuine. COMMENCEMENT OF THE COURSE Classes will commence soon after the admission process is completed and date of starting the course will be informed as well as published in the website. ANNEXURE II BOND (In Government stamp paper worth Rs.100/-) Articles of this agreement is made, entered, agreed and signed on this day of May 2015 between the Principal, Medical College, Academy of Medical Sciences (ACME), Pariyaram, Kannur Dt, Kerala (1 st party) on one side and Dr. S/o/ D/o (2 nd party) and S/o/D/o (3 rd party) as Parent/Guardian of 2 nd party on the other side relating to remittance of the fee (excluding special fees) for the PG courses starting from the Academic year of 2015-16. The 1 st party agrees to admit Dr. (2 nd Party) S/o/D/o (3 rd party) to the course starting from the Academic year 2015-16, the 2 nd party and the 3 rd party signed below, are bound individually and jointly, to remit the fees fixed by the Executive Committee of the ACME on the instructions from the Government of Kerala or by a Court of Law at the beginning of the class/ course of each Academic Year in lumpsum.
