Registration Information Sheet Registration is due Wednesday, June 17, 2015 at 8a.m (no exceptions) Session Dates: June 22, 2015 July 17, 2015 Location: Schuylkill Haven Area High School ***All sessions will be held at the Schuylkill Haven High School*** Time of classes: Session 1: 8:00am 10:00am Session 2: 10:10am 12:10pm Courses are scheduled by Program Coordinator. No student/parent request for session times are considered. Cost: $95.00 Courses offered: Mathematics Session Pre-Algebra, Algebra I, II, III, Geometry Science Session - Physical Science, Environmental Science, Biology, Chemistry* English Session English 7, 8, 9, 10, 11, 12 Social Studies Session American History I, II, World Cultures, American Government *Depending on enrollment Method of Instruction: The credit recovery program will be offered through a combination of teacher instruction and online learning through Blendedschools.net, an online learning resource. Blendedschools.net will be utilized by the students to fulfill course requirements. These requirements match the curriculum used at both Blue Mountain School District and Schuylkill Haven Area School District. Instruction during the first week will include directions on how the online learning format will be used. Students will access course material and assignments through the Internet at home or school. Most assignments will be submitted electronically.. Procedures and Regulations: All procedures and regulations are available in each school s High School Handbook. A course syllabus will be provided by instructor, which details course procedures and regulations. Attendance: Week1 Week 4 (06/22/15 7/17/15) Monday, Tuesday, Wednesday, & Thursday attendance is REQUIRED! Incentive Program: Thursday attendance not required, if student completes and passes all curriculum for the given week. Grading: Pass or Fail as established by School district grading scale School attire: As established by High School Handbook

Registration: 1. Registration deadline is June 17, 2015 at 8:00am. No Exceptions. 2. Payment is due at registration. Make checks payable to student s High School. a. Student s name must appear on check/money order b. No refunds after confirmation has been sent 3. No registration will be accepted on the first day of summer school. First Day of Classes: June 22, 2015 All students will report at designated session time to the auditorium entrance of the Schuylkill Haven Area High school for room location. Dear Parents and Students, Blue Mountain and Schuylkill Haven Area High Schools 2015 Academic Credit Recovery Procedure Information The credit recovery program is offered through a combination of teacher instruction and BlendedSchools.net. The students will access course material and assignments through the Internet. The course requirements match the curriculum used at both Blue Mountain School District and Schuylkill Haven Area School District. Instruction during the first week will include instruction of the online learning format. Students will access course material and assignments through the Internet at home or school. Most assignments will be submitted electronically. If at home Internet access is not available, the student may visit the public library or make arrangements with their teacher to use a laptop. Attendance: June 22, 2015- July 17, 2015 - Attendance is mandatory during the designated session time. WEEK 1-4: Monday through Thursday: MANDATORY ATTENDANCE! Incentive Program: Curriculum will be introduced in a weekly format. Students are not permitted to work ahead and will not be able to access online material other than the current week. Students who complete all weekly assignments and tests by their Wednesday session, and are passing with a 70% or higher, will not have to report for the Thursday session. Students will receive a signed pass from their instructor excusing them from the Thursday session. Failure to meet the attendance requirements set by the school district and the instructor will terminate the student from the course and the student will receive a failing grade for the course. No refund will be provided attendance violations or termination of the course. Student Progress: Students not adhering to the outlined course curriculum and benchmarks set by the instructor will be terminated from the course enrollment. The course syllabus will be provided on June 22, 2015. Students having technical problems accessing the online course material are required to be on campus the next school day at the designated time location to speak with the instructor and/or technical support personnel. No concessions will be provided for home computer problems or Internet access connectivity problems. Grading: Students will be assessed on campus with the instructor present. Assessments will be provided online or through traditional classroom methods. Students must obtain a passing grade in order to receive credit for the course. Family/District Agreement Enrollment in BlendedSchools.net courses will be based on an assessment of a student s academic background and a personal interview with school guidance counselor. The curriculum may be delivered either online or a blending of online and traditional coursework. Family Responsibilities: The student and family have read and signed an acceptable use/network user agreement. Students will abide by district policies and Procedure Information. Students will abide by the course requirements. Students who fail to make adequate progress may be removed from this program. District Responsibilities: The Course Instructor will be available via e-mail as needed. The Course Instructor will be available for any necessary face-to-face meetings at a mutually agreeable time between the instructor and the student and/or family. The Course Instructor will provide all necessary coursework through either online or a blending of online and traditional coursework.
