1. Scpe T detail the general safety prcesses t be fllwed fr all persnnel cnducting wrks r requiring entry int cnfined spaces which are the respnsibility f the Prt Hedland Prt Authrity. 1.1 Applicatin This dcument is applicable t all cnfined spaced which are n r within areas managed by the Prt Hedland Prt Authrity. 2. Definitins Cnfined Space any area that is partially r fully enclsed that is at atmspheric pressure during ccupancy and: Has limited access and egress, r May have a cntaminated envirnment, r May have harmful levels f xygen, r A persn may be engulfed, and Is nt primary designed as a place f wrk JHA Jb Hazard Analysis, a methd f assessing the hazards assciated with a task and implementing cntrl measures t enhance the safety f all persnnel Cnfined Space Permit Issuer PHPA Persnal authrised t issue permits t enter cnfined spaces Trained Persns Persns that have cmpleted accredited cnfined space training packages and have been deemed cmpetent 3. Jb Hazard Analysis The Prt Hedland Prt Authrity requires that all entries and wrk t be undertaken in a cnfined space be subject t a JHA. Sme examples f things the JHA shuld take int accunt are utlined in Appendix 1: Cnfined Space Hazards. The JHA shuld als cnsider whether it is pssible t undertake the wrk in manner which wuld nt require persnnel t enter r wrk within the cnfined space, r whether any ther cntrl measures that relate t the hierarchy f cntrl culd effectively be utilised. 4. Requirements It is Prt Hedland Prt Authrity requirements that all cnfined spaces which exist are: 1. Identified as per AS2865 2009 Cnfined Spaces 2. Cnfined Spaces must be labelled with a permanently affixed sign t ensure all wrkers can identify the cnfined spaces n site immediately. The signs must be maintained in gd repair 3. All prcess inputs and energy surces int the cnfined space must be islated befre entering Page 3 f 8

4. A prcess must be used fr sampling the air in the cnfined space t ensure that air quality is suitable fr entry 5. All persnnel must be suitable trained and qualified fr wrk in a cnfined space 6. A strict sign in / sign ut prcess must be fllwed 7. All hazards are identified n the JHA prir t entry and if a new hazard is encuntered while wrks are ccurring, the wrk is suspended until it is cmmunicated thrughut the team 5. Respnsibilities Individuals have the respnsibility t: Fllw this prcedure Advise the persn in charge f any issues r cncerns Attend training which is prvided Be aware f any hazards that may be within a cnfined space and the PPE requirements prir t any entry Supervisrs f the wrk have the respnsibility t: All requirements f this prcedure and the cnfined space permit are met That all persnnel n the jb have, signed nt the permit, and; Are suitably trained and cmpetent prir t entry Have been taken thrugh the JHA and are aware f any hazards that culd be present Knw the emergency prcedures t be used Are aware f the PPE required t be used prir t entry Use all f the PPE required Ensuring atmspheric testing has been undertaken and is ccurring as utlined in the JHA The Cnfined Space Permit Issuer has the respnsibility t: Ensure the islatins are suitable fr the cnfined space wrk Ensure the permit has been crrectly cmpleted and is understd Check the JHA and cntrls identified fr the hazards listed That they and the wrks supervisr are fllwing the requirements f this prcedure Atmspheric Testers have the respnsibility t: Fllw this prcedure Cnduct the testing in the prescribed manner detailed in AS2865-2009 Cnfined Spaces Ensure they are trained in Cnfined Space Entry Recrd the results n the cnfined space entry permit 6. Identificatin f Cnfined Spaces Cnfined space definitins are utlined in AS2864-2009 Cnfined Spaces. Page 4 f 8

The criteria utlined fr a cnfined space: Is a partially r fully enclsed space; Is at atmspheric pressure during ccupancy; Is nt intended r designed primarily as a place f wrk; May have restricted means fr entry and exit; and That may have: An atmsphere which cntains ptentially harmful levels f cntaminant; An unsafe level f xygen; r Cause engulfment All areas meeting these criteria must be cnsidered as a cnfined space and treated as a cnfined space. At the identificatin stage an assessment shuld be made f the types f risks in the cnfined space t ensure that the: Islatin; Entry Requirements; Type and frequency f air testing; and Persnal Prtective Equipment Needed t perfrm the initial entry and inspectin is knwn. 7. Labelling f Cnfined Spaces A cnfined space must be labelled with suitable signage t enable identificatin by all peple. The sign must be permanently affixed t every access pint t the cnfined space and be legible. Signage shuld cnfrm t AS1319 Signs fr the Industrial Wrkplace. 8. Islatins All cnfined spaces shuld be islated frm any prcess inputs and energy surces prir t entry r inspectin wrk inside the cnfined space. The inputs may include; mechanical, electrical r chemical/gas surces, these must be clearly identified n the JHA cmpleted fr the jb. 9. Atmspheric Testing Atmspheric testing must be undertaken prir t any wrk r entry int a cnfined space. The atmspheric testing must be undertaken by a trained, cmpetent persn using a crrectly calibrated meter. The meter used must be capable f analysing the air t identify any f the cntaminants in the cnfined space. The meter shuld be maintained strictly in accrdance with the manufacturer s specificatins. The testing must be cnducted in a methdical way t ensure the entire space is suitable fr entry, the tester must ensure that there are n air pckets which may have a different cmpsitin t the rest f the space. If pssible the test is t be undertaken frm utside the cnfined space, using prbes if pssible. If entry is required the tester must wear all required PPE identified in the JHA, dnned befre making entry int the cnfined space. The results frm the testing must be recrded n the cnfined space entry permit. Atmspheric testing must be repeated at intervals identified by the JHA and recrded n the cnfined space permit. Cntinuus mnitring may be required in sme situatins, fr example if ht wrks are being undertaken the cnfined space t ensure xygen levels d nt fall belw safe levels and there is n build up f ther cntaminants. If cntinuus mnitring is ging t ccur this must be identified in the JHA and readings recrded n the cnfined space permit at regular intervals as defined by the JHA. 10. Cnfined Space Sign On/Off Prcess All persnnel wrking n a jb where cnfined space entry is required must sign n t the jb by cmpleting the team member sign n sheet, which frms part f the cnfined space permit, Additinally persns assigned as the standby persn must sign n t the apprpriate sectin and sign ff nce relieved f standby duties. All persnnel effecting entry int the cnfined space must cmplete the cnfined space entry lg, it is the respnsibility f the standby persn t ensure that all persns entering the cnfined space are signed in immediately upn entry and signed ut immediately fllwing their exit frm the cnfined space. 11. Training All persnnel must be suitably trained befre entering a cnfined space; the training must cnfrm t the criteria set ut in AS2865-2009 Cnfined Spaces. There are three types f training assciated with cnfined space wrk; 1. Entry Training fr peple entering a cnfined space, regardless f the reasn 2. Permit Officer Training fr persnnel wh issue cnfined space entry permits 3. Atmspheric Testing Training fr persnnel wh will undertake atmspheric testing fr cntaminants in a cnfined space 11.1 Entry Training Entry training must cver the fllwing: Definitin f a cnfined space Identificatin f a cnfined space Hazards f a cnfined space Specific site prcedures and arrangements Emergency prcedures Persnal and team respnsibilities Page 6 f 8

11.2 Cnfined Space Permit Issuer Training Cnfined space permit issuer training is fr Prt Hedland Prt Authrity staff wh issue permits fr cnfined space wrk.
