Membership Application 1. Passengers at 2 years old or above is eligible to become the member of TransAsia Legend Flight Club (referred as member ). 2. For applicants aged between 2 to 12, should be consented by the guardian. The guardian shall also be a member of TransAsia Legend Flight Club. Effective from the date of being the Member, the Member will enjoy many exclusive travel benefits and start to earn mileage for every TransAsia-operated flight. 3. TransAsia Airways may request the member to provide necessary documents for data review. *Please use the same Chinese and English name as it appears on your ID card or passport while applying. Membership Tiers and Upgrade Advancement There are four tiers of membership in thelegend Flight Club. Membership will be terminated without any notice if the Member has not collected any miles in three consecutive years. The upgrade day falls on the 3rd of every month. We will inform you of any membership change via e-mail notification. Please check your membership tire via your e-mail and TransAsia website. Members who are not able to meet the renewal qualification will be downgraded from Black to Platinum, Platinum to Golden, Golden to Purple, but without any change to the card number. 頁 3

Full-price domestic tickets bought via our website or domestic flight counters can be discounted. B. This discount cannot be used in conjunction with other promotions. Note 2: Members can earn birthday mileage of 1,000 miles when taking TransAsia-operated flights during their birth months. This privilege is available one time per year. A. Bithday bouns mileage will be credited into the Member s account on 10th day of following month, valid for two-year. B. Birthday bouns mileage is excluded in the class upgrade scheme. C. Note 3: Extra baggage allowance A. Refers to free extra check-in baggage allowance: (10 kg for domestic flights; 20 kg for Economy Class and 30 kg for Business Class of international flights) + 10 kg extra allowance. B. Extra baggage is limited to personal items, and items for commercial purposes are not allowed. C. This privilege is only available for check-in baggage on flights fully operated by TransAsia Airways from departure to destination. Code-sharing flights which not operated by TransAsia Airways and charter flights are excluded from this privilege. Note 4: Priority standby for international/domestic flights A. This offer is limited to the card holder. (Or, it can be shared with at most 3 accompanying children.) B. When the flight is fully seated, Black Members are placed on the waiting list prior to Platinum Members, Platinum Members are placed on the waiting list prior to Golden Members. C. This offer is not available for domestic flights during the Chinese New Year. D. Please register for standby as early as possible to protect your rights. E. Note 5: VIP lounge usage privilege Self-operated airport VIP lounges are at Songshan, Taoyuan and Kaohsiung Airports A. Black Card Member is limited to the card holder and one accompanying person while Platinum is limited to the card holder only. B. Members who travel with an international flight operated by TransAsia, are permitted to access VIP lounge. Please present the membership card upon airport check-in counters. C. Free access is also provided for at most three children or infants who are not yet two years old. Other regulations on lounge access may apply. D. Black Card Members have priority to access VIP lounge if space is limited TransAsia reserves the right to take the final decision. Note 6: Details on in-flight shop discounts A. Members should present their membership cards to receive the above benefits. For members who are not yet offered the card, please inform the check-in counter. B. C. Special items are excluded. D. Only one benefit applies at a time. E. This offer is limited to TransAsia operated flights. Note 7: Details on Golden Membership for the spouse A. Black card member should submit a Replacement Application for Membership Card Form. (Appendis II) B. An official document that confirms marital relationship, along with the Replacement Application for Membership Card Form should be submitted by fax, e-mail or mail to TransAsia Service Center. C. Gold Membership for the spouse is effective for the same period as Black Membership. D. A membership card will be sent to you in six weeks after the application. E. There is no charge for the first issuance. For replacement,please refer to Membership Card Replacement on page 9. Note 8: Upgrade Bonus A. Upgrade to Platinum Membership comes with 2,000 miles. B. Upgrade to Black Membership comes with 5,000 miles. C. Effective for two years, the offered mileage will be transferred to the member s account in seven days after upgrading. D. The mileage of upgrade bonus is not included in the upgrade scheme. Note 9: Award Gift A. Renewal of Black Membership comes with 5,000 miles. B. Mileage Accumulation 1. Mileage (from departure to arrival) is collected for every TransAsia-operated flight (scheduled flight) with the ticket issued by TransAsia. (1) Members can gain flight mileage after the membership is effective. If you have taken any TransAsia-operated flight within 6 months, please access TransAsia official website to claim the flight mileages at Member Service page. (2) Mileage stated below refers to the one collected by taking TransAsia operated (GE) flights. (3) For code-share mileage, please visit our website for detailed information. (4) Mileage will be collected and filed in the system 3 days after your flight. All mileage accumulated will be bearing a validity of 24 months. The operation of mileage expiration will be executed without notice. (For example, mileage collected on Dec 31st, 2012 will expire on Jan 1st, 2014.) (5) A personal electronic mileage summary (if application) will be sent on the thrid day of every month. Please stay informed by checking your e-mail and visiting our website. (If members do not have any e-mail address, please visit our webiste or contact our Member Services.) 2. In order to accumulate mileage correctly, please inform us of your member number or show your membership card upon purchasing/booking/check-in. Please also make sure your name is spelled correctly all the time. 3. Mileage calculation is based on the TPM (Ticket Point Mileage) of the International Air Transport Association (IATA). The TPM and our flight routes might be changed without any prior notice. B. The following tickets are not eligible for mileage collection: (1) Domestic/international chartered flights and feeder flights. (2) Discounted or complimentary tickets for groups tickets, chartered flights, babies, employees and 頁 7
commercial partners (not limited to legally discounted tickets for the elderly, as well as persons with disabilities and his/her accompanying person) are not eligible to credit the milage. (3) Any cause beyond control, mileage is not credited even if the Member is transferred to another airlines. (4) Your mileage cannot be shared with other members. (5) For further reference, please keep your ticket stub and copied boarding pass for 180 days. 頁 8
Membership Information and Mileage correction 1. Members may check their mileage or update their personal information in the following ways: (1) Visit our website (http://www.tna.com.tw) > Member Services. (2) Contact our Member Hotline : 02-2795-5768 (3) Contact our domestic/international offices or domestic airport counters. 2. Mileage corrections can be made in 180 days after the actual flight date. For correction, please follow these steps : (1) Login > Member Services > Mileage Information > Reclaim Mileage (2) Please email or fax TransAsia Customer Center the copied boarding pass and membership card, along with your contact information (phone, fax and e-mail). We DO NOT accept any mileage reclaim after the 180-day period. 3. TransAsia reserve the right to make the final decision regarding your mileage. Replacement of Membership Card 1. Please complete the Replacement Application of Membership Card Form (Appendix II), and submitted via e-mail or contact TransAsia Member Center for card loss or personal information update. 2. Award Redemption 1. Exchanging awards please refer to TNA website. (1) Qualifications of redemption provided under the scheme of class modification or special offers will be announced in Latest News, Legend Flight Club and Member EDM on our website. Please Note (2) For awards redemption, please log in to our website. (3) Under no circumstance will TransAsia re-issue, refund or change any expired mileleages. (4) All offers may be issued for scheduled flights operated only by TransAsia. (5) Always be informed of member benefits by visiting our website. 3. Award ticket redemption procedures (1) Make sure you have sufficient mileage to redeem the award ticket.. (2) You will be guided to the booking page. (3) Choose your flight. (You seat status is automatically confirmed. Award tickets only apply to the flights departing in no less than 48 hours. (4) Complete all required fields. Please make sure the passenger s name is on the award recipient list if he/she is not the owner of the mileage. (5) Please pay the extra national taxes and fees via your credit card (if applied). (6) When flight booking is completed, the valid mileage will automatically be deducted from your account, with an e-certificate code and ticket number sent via e-mail or SMS. (7) Please present all the information stated in item (6) at the check-in counter. 4. Regulation of Award Tickets (1) To exchange award tickets for child member, please contact TransAsia Member Services. (2) Award tickets are not permitted to travel on embargo periods. periods, please see Latest News on our website. (3) Members are responsible to pay all taxes imposed by government or other charges imposed by carriers related to award tickets. (4) Award tickets should be used to the specified flight. (5) To cancel the award ticket, contact the Member Services during business hours (on weekdays: 08:00-19:00; on holidays: 08:00-17:00) at least one day before departure. Mileage that has expired when returned will deducted. (6) Award tickets should be used to the specified date and flight. To return unused or uncancelled tickets, our rules of Ticket Return may apply. In this case, no mileage will be returned. (7) Validity of award ticket: Flights Expiry date for return (return date should be specified when the ticket is redeemed) Domestic Valid for 60 days from the date of departure International Valid for 90 days from the date of departure (8) A free award ticket holder may not request other offers or a class upgrade and flight mileage is not credited for award tickets.. (9) If our flights are cancelled for reasons beyond control, the Member can request for mileage return. The returned mileage will be valid for another 90 days. (10) TransAsia reserves the right to limit the number of complimentary tickets to a flight. 頁 11
Upgrade Voucher Redemption Procedures (1) Please make sure that you have sufficient mileage and the ticket information of your original booking. (Your seat status is automatically confirmed. Upgrade award only apply to the flights departing in no less than 24 hours.) (2) Choose your flight, and then mileage redemption and a confirmed seat in the upgraded class will be automatically completed. (3) Upon class upgrade, valid mileage is deducted from your account. Please present the upgrade code and your ticket when you at the airport check-in counter. *After upgrade, the original booking is not valid anymore. If you want to cancel the upgrade and go for another option, please re-book your flight. Requirement for Upgrade Award (1) Log in to TransAsia Member Services page for class upgrade 24 hours before flight departure. (2) One-way tickets to scheduled international flights are entitled to a free upgrade to business class. Group, charter, infant, free or airline and travel industry discount tickets are not eligible for class upgrade. (3) For children s upgrade, please contact our Member Services. (4) Infant tickets are required to pay a fare difference for the upgrade.. (5) If the ticket presented at the airport check-in counter does not comply with the rules for upgrade, the member must abide by the airport staff s reconnendation: A. Purchase another ticket. B. Pay fare difference between the two classes. C. Check in according to the ticket class. (6) The upgrade voucher can only be used by the card holder, and it is available for the specific flight. Request to extend or refund mileage for an expired upgrade award will not be accepted. (7) If the flight is cancelled or returned for reasons beyond control, mileage is returned to your account and shall be redeemable for another 90 days. (8) An upgrade voucher is not permitted to use on embargo periods. For details about embargo periods, please see Latest News on our website. 頁 12
(9) To cancel your upgrad award ticket, please contact us at least 3 hours before flight departure. The system will automatically re-deposit the original vaild mileages and the upgrade voucher will be canceled. (10) Late cancellation is considered giving up the upgrade. In this case, mileage is not returned. (11) TransAsia reserves the right to limit the number of passenger upgrades subject to the status of flight capacity. 頁 13
Awards Transfer 1. Nominee Registration (1) To protect membership privileges, when members transfer awards, the information of nominee (all nominees must be members) should be submitted to TransAsia in advance. (2) To register nominees: A. Members may access to TransAsia website-member Services to register nominees. B. Each member may register up to 6 nominees. C. The new/updated nominee can get their transferred mileage after the registration is completed. D. The new/updated nominee list cannot be canceled after approved. The valid list is permanent until the transferor specifies its expiry date. 2. Awards Transfer/Cancellation (1) Mileage is transferred as an item. (2) With sufficient mileage in their account, the nominee (as the Member) should enter a validated nominee list, and they may not retransfer the award to a third person. (3) The transferor may request to cancel the process within 30 days. The transferor cannot cancel the request if it s overdue or accomplished by nominee. (4) Unless otherwise provided, the nominee should redeem the mileage in 90 days. If the nominee fails to redeem or denies, in written, the mileage is considered as abandon by both parties and not returnable. (5) After cancelling mileage transfer, the mileage will be returned and withdraw the e-certificate. Expired mileage while returning will be deducted simultaneously. (6) Member may access to TransAisa website to apply or cancel mileage transfer. 3. Notes: (1) TransAsia will disqualify any Member who violates Section A or Section B, and revoke all credited mileage. A. Members hereby agree not authorize to sell/buy, exchange, award, borrow/lend, transfer or mortgage mileage and upgrade vouchers via any media, including but not limited to the Internet or magazines. B. Member hereby agrees not to divulge any information to sell, purchase, barter, bestow, lease, loan, assign and deposit the mileage credits or electronic upgrade 頁 14
certificates in internet, newspaper or magazines or make others to do so. 頁 15
Special Notes (1) This Handbook is effective from Jan 1st, 2015. Starting from this date, members who gained their membership before Dec 31st, 2014 will be governed by the provision herein. Appendix III: Terms and Conditions of Legend Flight Club Application 1. Passengers over 2 years old are welcome to apply for the Membership of the TransAsia Legend Flight Club (hereafter referred to as TransAsia ). For accurate records, please don t make multiple requests. 2. For children at the age of 2 to 12, the custodian, who shall also be a Member, shall provide consent to his/her application. 3. TransAsia may request the Member to provide necessary documents for data review. 4. The membership cards should under no circumstance used by other people, including but not limited to transferring or lending. 5. A password is required to access our Member Services. Any lookup or service request is considered personally made by the Member. The member is strongly advised to securely keep his/her account name and password. 6. Any changes to the address and telephone number can be made directly by the Member on our website. The Member can also request us in written or by phone for such changes. 7. TransAsia shall not be liable to any loss of event messages and special offers resulting from the Member s failure to immediately inform the change to his/her address and e-mail, nor shall TransAsia be liable to any delay due to e-mail loss. 8. The Member is entitled to mileage accrual for every TransAsia-operated flight. Each flight record is filed for two years from the travel date. 9. Membership will be terminated without any notice if the member has not collected any miles in three consecutive years. 10. Membership is suspended once the Member passes away. 11. Mileage accrual, redemption and welcome packages are governed by the Legend Flight Club Membership Handbook. 12. To gain privileges, the Member is required to present his/her membership card and identification during check-in and boarding. TransAsia reserves the right to cancel any privileges in the event that the Member fails to present such documents. 13. TransAsia reserves the rights to change any special events and details for the Members. Such changes will be posted on our website and in-flight magazines without any notice. 14. Qualifications for other welcome packages provided under the scheme of class modification or special offers will be announced at any time in Member Services and Mileage Notification on our website. TransAsia reserves the right to interpret all terms provided herein or rights to awards. 15. Violation of any regulations and procedures of TransAsia s special offers, or false information may lead to suspension of membership. 16. TransAsia reserves the right to take reasonable legal procedures against violation of the terms and conditions provided herein, and the Member will be deprived of his/her membership. 17. The Member agrees to authorize TransAsia and its affiliates to record, collect, process, use, transmit, disclose, transfer or share the Member s information (collectively called Use of Information ) for the purpose of product, service and offer promotion, provided that such use is allowed by application laws and the terms stated herein. The Member also agrees that TransAsia provides such information for any person that has entered a partnership agreement with TransAsia or sales representative that is authorized by TransAsia 18. The Member can at any time sign into our Member Services to cancel TransAsia s use of information for joint promotion. Or, the Member can contact our Member Services to update his/her information. Please note that once authorization is cancelled, the Member might not gain any services and offers that is provided on the basis of joint promotion.
