Oxfordshire County Council supports people to live their lives as successfully, independently and safely as possible. We believe that people, regardless of age or ability, are best placed to determine the help they need to lead successful lives. Oxfordshire Support Finder tells you about support services across Oxfordshire to help you or a loved one lead the lives you want to. It is designed to give you help and advice so that you can find your own way to meet any needs for support and care. You might want to know about support to live at home, how to find out about support and care and how much you might pay for it, or how you can find the right place for you to live. 5 Getting out and about Community Information Networks Get involved, get informed, get in touch Do you want to find out about opportunities and support to help you remain independent and well? The Community Information Network is a service to help you find the information you need more easily. Your local Community Information Worker can provide free, independent face-to-face information and advice on social care, finances, local support services and much more. They ll also let you know about activities and opportunities in your area. For more information contact Age UK Oxfordshire on or visit oxfordshire/community-information-network Good Neighbour Schemes If you want help with transport, odd jobs, gardening, general errands, or someone to chat to, a Good Neighbour Scheme is for you. Good Neighbour schemes are local voluntary groups which offer a service in their community for those in need of help and support. The service aims to help you to stay independent, safe and well. Oxfordshire County Council is encouraging Good Neighbour Schemes in Oxfordshire to form and develop. There are now over 50 schemes up and running in Oxfordshire and new schemes are emerging all the time. 10 Oxfordshire Travel Advice Line Get free impartial advice on a range of transport options for those wanting to get out and about. Call , Monday to Friday, 9am to 5pm or [email protected] Oxfordshire Dial-a-Ride This minibus service, for people with mobility problems, will pick you up from your home. You can catch a minibus between 9am and 5pm but you must book in advance. To register for the service call or visit Bus passes You must be eligible to apply for a bus pass. To find out more, call Oxfordshire County Council s Social and Health Care team on Blue Badges The national Blue Badge Scheme helps people with severe walking difficulties that travel as drivers or passengers to park close to their destinations. Residents can apply for one of these parking places to be provided close to their homes or work. Oxfordshire Accessible Transport and Access Group The group works with bus and rail companies to make sure transport in Oxfordshire is accessible and affordable. It raises issues affecting disabled people with County and District councillors, works on national disability issues and contributes to a fortnightly disability column in The Oxford Mail. If you are a disabled passenger and have a complaint, the group will look into it for you and try to get the issue resolved. 11 Railcards A Railcard will give you discounts on rail fares, online or from a staffed rail station. There are several schemes for passengers with different needs, including carers. To buy your card online, find out about ticket prices or get help planning your journey go to call or [email protected] Shopmobility Shopmobility is a free service offering loan of wheelchairs and electric scooters to people who need them for shopping and sightseeing. Shopmobility services are available in various locations around Oxfordshire. To find out about services in your area, visit the Shopmobility website Public toilets - including accessible toilets Your District Councils (contact details on page 32) have a list of all public toilets in your area and can tell you how accessible they are. They ll also let you know about local shops and businesses which have accessible toilets. The Changing Places website has a map of toilets that are fully accessible and have the equipment that disabled people may need. This can be found at Some businesses in Oxford have agreed to let members of the public use their toilets whether they are buying anything or not. Their teams, made up of psychologists, community mental health nurses, social workers and occupational therapists, provide mental health services to adults in the community. If you feel you need a referral, please contact your GP. There are Memory Cafés, training courses for carers, and opportunities to socialise. Community-based dementia advisers and support workers are available across Oxfordshire. They provide tailored information, advice and support to help people live well with dementia. Oxfordshire Dementia Empowerment Group If you live with dementia and want to talk to others about issues that affect your life, join Oxfordshire Dementia Empowerment Group. Oxfordshire libraries are part of the national scheme Reading Well - Books on Prescription which is an approved list of books on mental health issues such as anger, anxiety, depression and stress. Local health practitioners can prescribe a book from the list or you are simply welcome to self-prescribe and borrow the titles that you think will help you. To find your local library visit Find out more at The Home Library Visiting Service The Home Library Service is a free service from Oxfordshire County Council working with Age UK Oxfordshire and Royal Voluntary Service that brings books, DVDs, music CDs, audio books and information about other library services to you at home on a regular basis. The service is offered to people of any age who are housebound and can t get to the library due to disability, illness or full time caring responsibilities. This includes people who might need the service on a short-term basis following debilitating illness or hospitalisation. 16 Support and care where do I start? This Guide will give you a good starting point to help you think about options and provide you with information so that you can find solutions yourself to help you to remain independent and well. The Care Act The Care Act outlines how care and support should be provided in England. It came into effect on 1st April 2015 and replaced all existing adult social care legislation. The law focuses on promoting wellbeing and health with emphasis on personal dignity, physical and mental health, protection from abuse and neglect and control over day-to-day life. This has been Oxfordshire s approach for a number of years. Assessments The Care Act introduced a nationally agreed level of care and support needs that all councils consider when assessing the support they can give to individuals. It also introduced the right to an assessment for carers (see page 17) and independent support from an advocate if you have difficulty with participating in assessments or planning for your needs. If you have difficulty with day-to-day activities such as getting washed and dressed or getting yourself something to eat, you may be eligible for support from Oxfordshire County Council. Being eligible means that, because of illness or disability, you need support and care to manage some of the basics of everyday life, such as eating, using the toilet, maintaining relationships or keeping yourself safe at home. There are national criteria for the Council to use to work out if you are eligible. To find out if you are eligible you can have an assessment. This is an opportunity to understand your situation, the needs you may have and your strengths and capabilities, and will involve talking to you, your partner, carer, relatives or friends as appropriate to consider the factors that affect your health, wellbeing and independence. The assessment will include the outcomes that you want to achieve in your day-to-day life as well as consideration of any needs you may have for care and support. The assessment will also consider how the provision of care and support could help you achieve those outcomes and promote your wellbeing. You have the right to have an assessment regardless of your ability to pay for the support you may need. The assessment may be over the telephone or face-toface, and in the future you will also be able to choose to complete the assessment online. You can arrange for an assessment either for yourself or for someone else: via the Oxfordshire County Council website at by phone on ; or by [email protected] If the assessment finds that you do have needs for care and support which are eligible to be met by the Council, you will be helped to make a support plan. This will include the Council telling you how much meeting your needs is likely to cost known as a Personal Budget. Having a Personal Budget helps you to have control over your life, identify your needs and the things you want to do and, if you wish, to manage your own support. You can choose to have your Personal Budget paid to you as a Direct Payment, which means you can plan exactly how to use it to meet your needs. A Direct Payment is money paid into a person s bank account you have to spend it on meeting your care needs. Personal Assistants can help with everyday tasks like going to the shops, as well as personal care such as washing and getting dressed. 17 Looking after someone Caring can be very stressful but you are not alone. At any one time in Oxfordshire over 60,000 people are providing unpaid care for a family member, friend or neighbour. It is very important that you take time to look after yourself while you are caring for someone else. You may not see yourself as a carer - you see yourself as a son, daughter, partner, friend or neighbour. But if you regularly support and care for someone who could not manage without you then you are a carer and there is help available to support you to carry on with this vital work. If you provide unpaid support to someone who could not manage without your support, the law says you have a right to have your own needs assessed, even if the person you care for has refused support services or an assessment of their own needs. This assessment, called a Carer s Assessment, gives you the chance to say what you need as a carer, and to find out what support might be available to help you. The assessment can also give you information about services available, and other ideas for supporting you in your caring role. Your Carer s Assessment will show if you qualify for support services from the Council. Many carers will be entitled to receive a support plan and Personal Budget in their own right, for services that enable them to continue in their caring role while still being able to pursue their own interests or lifestyle. Carers will not be charged a contribution to services arranged through the Council. Carers Oxfordshire Carers Oxfordshire is an organisation set up for carers which provides free information and advice, specialist oneto-one support, a quarterly newsletter, the carer s discount card, access to support groups, training opportunities including Confidence to Care, moving and handling, and first aid plus support to help you balance work and caring responsibilities. It gives unpaid carers who are struggling financially, practically and emotionally access to the support they need so their quality of life improves and so they never feel alone in their caring role. The support they offer includes: help with benefit checks and forms; ongoing emotional support; trips and activities for adult carers; practical support to unpaid carers with issues such as discharge from hospital, accessing disability aids and equipment, and entitlement to social care; representation of unpaid carers at disability benefit appeals and tribunals support to children and young people aged eight to 17, who live with a parent or sibling who has a physical illness or disability, a mental health problem or is dependent on drugs or alcohol; emotional support to young people and children; free trips and activities for children and young people; and outreach with local employers so that unpaid carers who are trying to juggle their caring responsibilities at home with paid employment do not miss out on support. 18 Respite at home Home-based respite services give carers a break from their caring responsibilities. A care worker comes in to look after the person you care for, so you can have some time to yourself. Respite at home is considered to be a service for the person you care for. If they have a Personal Budget for their own needs (see page 16) they could use that money to pay for it. A financial assessment of the person being cared for will confirm whether they need to pay towards the cost of this service. Benefits you may be entitled to as a carer Carer s Allowance If you care for someone for at least 35 hours a week and you are over 16 you may be entitled to Carer s Allowance. However, you cannot claim it if you are in full-time education or employment and it can affect other benefits or pension you receive. Carer s Premium If you are over 65 you may qualify for Carer s Premium. This can entitle you to an increased amount of incomerelated benefits (such as Pension Credit, Council Tax Benefit and Housing Benefit). Carer s Credit Carer s Credit is a National Insurance credit which will let carers build up qualifying years for the basic State Pension and additional State Pension. This means that there will be no gaps in your National Insurance record if you have to take on caring responsibilities. Help in an emergency Emergency Carers Support Service is a support service for adult carers of adults in Oxfordshire. 19 Equipment, aids and gadgets Assistive Technology - devices to help you stay safe and independent Assistive Technology refers to devices that can be used to increase your independence and safety and help you with day-to-day tasks. It can also give you, your family, carers or professionals the peace of mind that help is available when you need it and can help determine how you are managing at home if you have memory problems. Assistive Technology can help you to: do something, such as remember to have something to eat or drink, take medication or go to an appointment; keep safe at home, through smoke, flood, or fall detectors; request immediate help; keep safe when you re out and about; and reassure family, friends and carers that you are safe when they are not with you. Assistive Technology ranges from very simple gadgets, such as medication aids (boxes with compartments for particular days of the week and times of day), to Telecare (devices that can automatically and remotely monitor emergencies and call for help). Telecare Telecare is a term used to describe devices (for example, movement, flood, gas, and smoke or fall detectors) that automatically send a signal when a problem is detected and calls for assistance. Personal pendants are also available as part of Telecare; when pressed, a fully trained operator assesses the nature of the emergency and reacts accordingly. Simple gadgets A range of gadgets exists to remind or enable you to do something, or alert a family member or carer living with you that you need help. These gadgets are sometimes referred to as standalone Assistive Technology. Here are some examples of gadgets that Oxfordshire County Council can help to provide: Memory prompts These are used to assist people with memory difficulties by providing daily memory prompts using personally recorded voice messages at specific times of the day to do specific activities, for example, take medicine or eat lunch. Movement activated memory prompts A movement activated message player allows a message to be recorded and played back when a person walks past the sensor, for example, take your keys with you and lock the door or make sure you have turned the cooker off. Location devices Location devices are designed for people who may be at risk of getting lost when they go out on their own and can help them to return home safely. They can be used to locate a person when needed, allow the wearer to call for help and alert friends and family if a person goes outside of areas they know and feel safe in, has a fall or uses transport. People can also have two way conversations using these devices. Will I have to pay for my Assistive Technology? If you arrange this through Oxfordshire County Council they will undertake a financial assessment to work out what, if any, contributions you would need to make to the cost of Telecare. Gadgets (standalone Assistive Technology) are provided to you based on your need for the equipment and do not require a financial assessment. 20 Occupational therapy Oxfordshire County Council s Social and Health Care team takes referrals for occupational therapy, which can help identify if you would benefit from any pieces of equipment, from simple solutions such as a walking aid or a second stair rail, to major adaptations such as walk-in showers. If you have difficulty with day-to-day activities such as getting washed and dressed or getting yourself something to eat, an assessment by a social worker can help you find out if you are entitled to a Personal Budget to pay for the support you need. Disabled Facilities Grants A Disabled Facilities Grant can help eligible disabled people to adapt their homes to make their day-to-day life easier. This is a means-tested grant - whether you can receive it depends on your other sources of income. To apply for a Disabled Facilities Grant you must first be assessed by an occupational therapist (see above). Essential Repair Grants This is a means-tested grant for home owners. An Essential Repairs Grant of up to 5,000 can be made available to help with essential repairs to the home. Flexible Home Loans This loan scheme for owners and occupiers aged 60 and over is designed to help improve the comfort, safety and security of, and help with repairs to your home. Small Repairs Service Available to owners, occupiers and tenants who are aged 60 and over, disabled and suffering from a long-term health condition. This service covers small plumbing, electrical and basic household repairs. The labour charge is 15 + VAT plus the cost of any materials used. Support and care at home All home care providers are regulated and inspected by the Care Quality Commission which publishes inspection reports and new quality ratings on its website When considering a service it s always a good idea to check their report. Before you make any decisions regarding the provision of care, be sure to contact several providers and ask for a Service User s Guide and their charges and a draft contract between you and the care provider. Daily support and care A carer can call in daily to assist with day-to-day tasks. Visits are generally available from 7am to 10pm. Some people will need more than one visit per day. Night services can also be provided. The hourly rate depends on the services required, the time of day and the location. It can also provide respite for regular carers and short-term support following a hospital stay. The standards you can expect If you receive support at home from a care agency, these standards will set out what you can expect. If you are a care agency, these are the standards that are expected from your staff and the standards that Oxfordshire County Council will use to monitor the quality of your services. Home care staff will: introduce themselves when they arrive; know you and your support plan; be trained to deliver the support you need; and always deliver support to a good standard. When your home care worker visits you, they will: focus their attention on you; be pleasant and treat you with dignity and respect; do their very best to arrive on time and let you know by telephone if they are going to be late; tell you when they are leaving; check to see how they can best support you at the start of each visit; not rush you - they will help you at a pace that suits you; carry out all agreed tasks; make sure that you are comfortable at all times; communicate and discuss topics that interest you; check that you are happy with the support you are given and encourage you to tell us how they can improve; show you your care record if you ask for it; and be aware that their visit may affect other household members. These standards have been jointly written by people who receive care in their home and home care support agencies. Money and paying for support and care Care funded by the Council One call to the freephone number will enable the service to build a profile of exactly what type of care you re looking for, while taking into account your personal needs and interests. Do I qualify for local authority financial assistance? If you have been assessed as needing a care home place and your capital is below 23,250 (correct at 1st April 2015 but may change) you may be entitled to financial support from your local authority. To find out more, contact Oxfordshire County Council s Social and Health Care team on Do I have a choice of care home if the Council is paying? Yes and it can even be in a different county. The home you choose must be suitable for your assessed needs, comply with any terms and conditions set by your local authority and not cost any more than they would usually pay for someone with your needs. What if a home costs more than the local authority is prepared to pay? The local authority will allow a third party to top-up your fees as long as they are able to do so for the long term. You are not allowed to top-up the fees yourself from capital below 23,250 (correct at 1st April 2015 but may change). My partner needs care, how does this affect me financially? The Council will do a joint financial assessment for you and your partner, and will also carry out individual assessments on whoever needs care. The contribution you are asked for will be whatever the lower amount is from these assessments. If you receive joint benefits you will only be offered a joint assessment. Paying for your own residential care If you are funding your own care because you are not eligible for local authority funding there are other forms of financial assistance you may be entitled to. The important thing is not to try to do it alone, always seek professional advice. 37 Having your say How to make a complaint or comment If you are unhappy or worried about any adult social care services, it is important to let someone know. Usually talking to someone in the service can put it right. You can talk to the people who provide your care or their managers. They will have a process in place for managing complaints and listening to concerns that are raised. Wherever you raise a concern it should be listened to and taken seriously.
