Located in the elevator pit. Can be heard only faintly unless you are near door on first floor. Alarm cannot be silenced or reset. Indicates flooding of pit is likely. No response instruction posted. Call trustee immediately. Contact Otis elevator Co for service. Sewer ejector pump alarm. Located in basement hall just above basement class room door. Alarm is loud. Cannot be silenced unless the main Sewer ejector pump breaker is shut off. This action will deactivate the pumps and the main rest rooms and Chapel handicap rest room will be out of service. Posted response: Call trustee or Monroe piping for emergency service immediately. Follow up response : Post signs on affected toilets out of service. Contact Heritage Christian that Handicap toilets in chapel are out of service. Handicap toilet near our general office is not affected by this sump alarm and still is available. Fire alarms. I believe that a fire alarm with result in the elevator being deactivated. I am not sure if we have alerted people in building that they must use stairs or ramp to exist the building in event of a fire alarm evacuation.. The proper response for any fire alarm is to evacuate building immediately. Any notification call to trustees or others should be made after leaving the building. I fully agree on the need for documentation and training, which should be set up as soon as possible who can set this up? Are we planning to eventually tie the 3 local alarms into the central monitoring system? If this had happened when the church was unoccupied, I d like to have the monitoring service alerted so they can call someone. Where are the 3 local alarms, and are they all easily audible from the office (with the office door closed)? Don 1

Georgia contacted me immediately and told me we had an alarm bell ringing and the staff did not know if it was a fire alarm or a local alarm. The staff was confused about the proper response for local alarms. When I arrived I found that the alarm was the high water alarm in the sewer sump. Upon investigation I found that the cause was not an alarm malfunction. The pumps were not running and the alarm was sounding loudly in its new location. This alerted the Heritage Christian staff who called the office. Further investigation revealed that the local sewage ejector pump switches may have been inadvertently shut off. I think that this could possibly have occurred on Saturday May11 during work day. The rising water in the pit eventually caused the high water alarm to activate probably as the result of toilet use on Monday morning. The pumps started immediately when the local pump switch was activated. Following the event the office staff asked me to conduct a training session on the proper alarm response for the office staff and pastors. I will try to do this next week. I tested the pumps and alarm system and the system seems to be working properly. The good news is that the sewers ejector pump alarm works as intended. The relocated alarm was heard and proper action taken. The bad news is there is a lot of confusion about the various alarm systems in the building and the proper alarm response. It was clear to me when I arrived that some people thought the sump alarm sounded like the fire alarm and were confused as to how to respond.. The Heritage staff response action was to evacuate the day care program from the building. They did not return until later that day. After giving some thought to this I think we need to develop a written set of required alarm responses for each of the alarms in the building. An alarm response book should be placed in the office and training conducted. I have posted a written set of required response actions for the sewer alarm in the basement next to the sewer alarm bell. The fire alarms can be easily identified by both the loud noise and the strobe lights will be flashing throughout the building. If there is any doubt the fire annunciation panel located at the north narthex door will be lighted during any Fire alarm actuation or fire system trouble. There are three non fire local trouble alarm bells in the building beside all the fire alarms that are located throughout the building. Local Trouble Alarms ( Non Fire) ( not connected to central alarm monitor) Elevator sump pit alarm Boiler room ground water sump alarm Sewer ejector pump alarm classroom no written response instructions written instructions posted near alarm at trustee locker door. written instruction posted now posted near alarm outside basement I think we need to train our staff and the Heritage staff and all the Trustees on Alarm identification and the proper alarm response. In most cases the proper response for local trouble alarm will be to call a Trustee immediately. Telephone numbers for trustees list should probably be posted near local alarms. 2

3 For a fire alarm the proper response is to leave the building immediately and assemble in parking lot.. The central alarm monitor will have alerted the fire dept. For non fire alarms the follow up response varies but may require the main rest rooms to be taken out of service. The Heritage staff should probably be notified for any local alarms. Perhaps we should delay the formal staff training until we get all the action required written and all the trustees can review and weigh in on the proper alarm response protocol. I would like your thoughts. I am troubled by the confusion over the fire and local alarms response and identification. We need to make sure the proper response is for all to evacuate building immediately if the fire alarms activate. Obviously the breakers were OK so what went wrong? As for the order of how events occur. At this time only a fire related condition or alarm system trouble condition cause an alert to the central office. In the case of a fire the first thing the central office does is alert the fire and police departments. Once that is done then they start at the top of the call list. Once they make contact with someone on the list that is the end of any further contacts. They don t ask for any passwords but simply verify you are the appropriate contact and tell you what is happening. I believe the contact list and order is me, DeVoss, you and Larry. That can be easily verified with Infinity. There is no need to contact anyone once outside since that is taken care of. If someone wants to call somebody else that s fine but there may be enough going on this would simply create more confusion. We don t need more people arriving if it is really an emergency. Only a fire condition alarm will activate the internal alarms and strobes. Any other condition is simply reported to the central office. In the case of a fire condition the response of anyone in the building is to evacuate and wait for the authorities. Since there are currently only local indications of problems with a sump the only response should be to notify a responsible Trustee. If it is the sanitary sump then all restrooms except for the pastors and lower level narthex rooms should be blocked off so no water is allowed into the drains until the problem is corrected. The kitchen drains are also OK to use. I don t know how loud the boiler room alarm is but the elevator alarm should not bother anyone. Generally if an alarm is activated it is probably not a good idea to allow anyone other than somebody familiar with the system to reset it. That way when the responsible party arrives they can see the problem. If it is reset and cannot be duplicated then there is no way to know what caused the condition in the first place. If the boiler sump alarm is too loud then 3

4 simply close the door which should make it bearable until somebody arrives. If it is the sanitary sump then they will just have to live with the noise for a while. Georgia asked me to Train the office staff and pastors on the proper alarm response on this Wednesday or Thursday ( if any alarm goes off). For non fire alarms the only alarm that can readily be silenced by a laymen is the boiler sump. I will tell them that the best response for any non fire alarm is to contact a trustee asap. For a fire alarm the alarm monitor will be calling the office or one of us. My understanding is that the fire dept will be called by the monitor if the alarm is a smoke or pull actuation or they get no response from the list for any other alarm.. If fire alarm rings I think the expected response for the staff and occupants should be to evacuate.. A call to the trustees should be made asap after leaving the building. For a fire trouble alarm I believe the office or one of us on list will be called by the monitor and they should give the pass code. if the office gets the call they should contacted one of us or a trustee asap. Only the sanitary sump activates the bell. Once tied into the alarm either will notify the central office but only the sanitary will sound the bell. The actual alarm that sounds for the 4

5 elevator is in the shaft and is hard to hear unless you are very near the elevator. If it goes off then we need to get in the shaft and see what s going on. Speaking of sump alarms I notice there is now an alarm on the boiler room sump also and that should probably tied into the alarm system when we activate it also. I will investigate what it will require to get a contact closure for that also. I just thought if the sanitary went off yesterday, was it my fault? I did turn off the breakers inside the control box to replace a blown fuse and maybe I didn t turn them back on. I thought I tested them but I may have messed up and didn t turn them back on. If so Oops my bad!!!!!!
