1 THE GEORGE WASHINGTON UNIVERSITY HOSPITAL WOMEN S BOARD, INC. NURSING & PUBLIC HEALTH SCHOLARSHIP APPLICATION PACKET Introduction: The George Washington University Hospital Women s Board, Inc. (Women s Board) serves various charitable interests of The George Washington University Hospital (GWUH) and the community at large. As we recognize the important role of nursing and public health in the relationship of health and healing, we are offering this nursing and public health scholarship to help serve the needs of GWUH patients and families. The selected scholarship recipients will receive $4,500 in scholarship award money ($2,250 distributed in December 2012 and $2,250 distributed in May 2013). Please follow the instructions below and complete all portions of the application packet. Application materials that are submitted via , send to JanaGWUH@gmail.com by Saturday, October 6 th, Recommendations and transcripts have the option to be sent via US post. They must be postmarked no later than October 6 th. Scholarship Criteria: 1. Education. Enrollment in an accredited nursing program (LPN/RN/MSN/NP/Ph.D.) or a school of public health. 2. GPA. Minimum grade point average (GPA) of 2.75 out of 4.0 for LPN, RN and BSN and 3.2 out of 4.0 for all others. 3. Official Transcripts: Official transcript (s) with the college or university seal sent by US mail or sent as a secured, certified PDF by submitted directly from the school. 4. Employment. Currently employed and a minimum of 1 year continuous employment completed at GWUH as of October 6, Candidate must work a minimum of 60 hours per month at GWUH. 5. Recommendations. Two letters of recommendation (Form included must be sent by US mail or must be submitted by directly from recommender). 6. Financial need. There is no minimum or maximum income requirement. However, the Women s Board strives to serve those in need. 7. Application Form with Essay section. (Form included must be submitted via .) Completion of one essay question. 8. Résumé: Include a professional résumé. (Résumé must be submitted via .) 9. Interviews. Final selection will be made based upon candidate interviews with the Scholarship Committee. Selected candidates must interview in person in the evening on October 25 th, Awardees. An individual may only receive this scholarship a maximum of two (2) times. Page 1 of 6

2 Instructions: 1. Your application form with essay and professional résumé, must be ed. Please convert your completed documents into PDF when possible, before submitting. Complete all portions of the application and avoid using see résumé on any parts of the packet. Please follow the instructions on the essay sheet for writing and submitting the essay question. The essay section is located at the end of this form. 2. Submitting your recommendations by US post: Print out two forms for recommendations and complete the first page of both forms with respect to the Privacy Act. Obtain two recommendations one must be from an immediate supervisor. The letters of recommendation must be placed in sealed envelopes with signatures from those completing the recommendations across the sealed back flaps of the envelopes. The recommendation letters must be mailed directly to the postal address below. Submitting your recommendation forms by Complete the first page of both recommendation forms with respect to the Privacy Act and submit by with your application. The actual letters of recommendations must be ed by the person writing the recommendation on your behalf and ed directly by them to the address provided above. 3. Submitting your transcripts by US post: Make arrangements for your school(s) to send the official transcripts directly to the address listed below. All transcripts must include the official school seal.
