1 The Barbara Lewis King Foundation Scholarship Application 2015 The purpose of the Barbara Lewis King Foundation Scholarship is to invest in the future of young men and women through a scholarship program at Hillside International Truth Center, Inc. In addition, the Foundation seeks to encourage and support the development of civic responsibility and leadership skills in participating students/applicants through church service, community service, commitment to academics and creative expression. The Foundation is geared to graduating high school seniors. Student (Applicant) must: Eligibility Requirements 1. Provide a record of community service: volunteering at school, church, and the community. The leader or director of the organization and or service must document a record of your service in a letter. 2. Demonstrate a dedication to academics, with a minimum of a 3.0 GPA or a meaningful expression of creativity, and/or additional leadership skills. 3. Provide proof of graduation and acceptance to a 4-year college/university, community college, or technical school. 4. Complete the attached application, which must be received by the committee on or before 12:00 noon, Sunday May 10, As a part of the application we require a brief essay, a questionnaire form, an official sealed school transcript, two written recommendations from a school official and one recommendation from a community leader who knows you well. The Scholarship Procedure Scholarships shall be monetary in nature and shall be derived from a church-based fund supported by voluntary contributions, and approved fundraisers. Scholarships will be provided one time and are not renewable. Applicants will be scheduled for an interview with the Barbara Lewis King Scholarship Committee and Dr. Barbara Lewis King. Recipients nominated for the scholarship award will be notified by mail. The number and amounts of scholarship distributions will depend upon the number of participants in the Program and the availability of scholarship funds. 1

2 The Barbara Lewis King Foundation Scholarship Application 2015 Application Instructions Please return the completed application packet to a member of the Barbara Lewis King Foundation Scholarship Committee, or place in the Scholarship Mailbox outside the Church Office on or before 12:00 noon on Sunday May 10, The mailing address is, Hillside International Truth Center Inc., 2450 Cascade Rd., Atlanta, Ga , Attention: Scholarship Committee. Applications that are mailed must be received by Sunday May 4, Your application packet must be in a manila envelope with your name on it. Printable copies of the application are available at The application packet must include in the following order: 1. Completed application information form. 2. A recent 5x7 picture (head shot) of yourself in appropriate attire, with your name printed on the back of the picture. Please see attachment of actual size of 5x7 photo. 3. Provide a one paragraph response to each of the following questions, A through E. (see attachment page): A. The scholarship is named in honor of Dr. Barbara Lewis King. Please write an essay on Dr. Barbara s accomplishments and how they have enlightened or inspired you. B. What impact has your faith made in your life and how do you plan to use it? C. What are your career goals and your strengths as they apply to this career? D. List and describe accomplishments, awards, affiliations, volunteer and extracurricular activities, interest, and hobbies. E. Why should scholarship assistance from Hillside International Truth Center Inc. be awarded to you? 4. Submit a ½ to 1 page, 12pt. font, and single-spaced essay on a topic of your choice. 5. A copy of your acceptance letter from the college/university, community college, or technical school. 6. A sealed copy of your official school transcript. 7. A letter from the leader or director of the organization where you have provided community service. 8. Two sealed letters of recommendation, from teachers, professors, or school officials. Your name should be placed on the top left hand corner of all documents/envelopes, and placed on the back of your photo.
