2 Our Wellness Program Overview 12 Weeks To A Healthier Employee Our 12 week (3 month) aims to jump start your employees journey to better health. Based on the Healthy Food Spiral, our program is easy to start and easy to follow. Our wellness program combines classroom coaching with digital on-line support for a balanced experience. We developed the Healthy Food Spiral to be unlike anything else that is available. The unique Food Spiral approach gives your employees, of any age or health concern, the ability to make the right food choices. It takes into account dietary challenges, such as diabetes or hypertension, that people face on a daily basis. Our goals are to empower your employees to take control of their dietary challenges, giving them all they need to create a healthy workplace and culture of wellness. Which is paramount to improving health care costs and preventing disease in your company.

4 Components of the HFS Wellness Program Six (6) In-Person Coaching Sessions During our twelve week wellness program participants are required to attend (6) six in-person classes in a classroom setting. Moderated by a certified member of your staff, these 90 minute classes help create a sense of community amongst the participants which studies have shown helps keep them motivated during the program. We have created a detailed class curriculum with annotated teacher guides ranging from eating habits, food labels, power of fiber navigating a grocery store, overcoming dietary challenges, and the power of simple exercise. Class scheduling and notication will be handle via by the Food Fitness First online education portal system.

5 Components of the HFS Wellness Program Six (6) Online Coaching Sessions In addition to in-person classroom classes, we have six online classes that participants can take on their own leisure. Each week participants will be provided access to that week's interactive training modules as well as a PDF manual download. The modules will play on a desktop or mobile device. This allows your participants to access the training from any place at any time. To gauge comprehension of the modules a post test will be required. With the results being ed to the program adminstrator. Class scheduling and notication will be handle via by the Food Fitness First online education portal system.

6 Components of the HFS Wellness Program Before & After Health Assessment At the first class we set up 12 health assessment stations. These stations will create a complete baseline assesment of the participant's health. We measure weight, body measurements, exercise, blood pressure, medication, smoking and alcohol use. Using that assessment, participants will be tracked and their data collected throughout the program. We suggest that several of the pilot employees consist of management staff, so that corporate support for the program is shown. Based on the assessment result, each participant will receive a Favorable Fitness Factor. The Favorable Fitness Factor is a scorecard of health, used to determine a participant's a meal plan, exercise information, and dietary challenge information. At the final meeting an "after" assessment is done again. And final results are shared. Suggested data for the company to compile and track: sick days, healthcare costs (year previous and ongoing for comparison), absenteeism, % employees enrolled, participation in Food Fitness First, Inc. and other input.

7 Components of the HFS Wellness Program Quick Start Exercise Guides & Tips Each participant starts out with basic exercise plan based on their Favorable Fitness Factor number, age, dietary challenges and targeted goals. During the entire program they will have online access to information that will give them insight into building strength, flexibility and endurance with weight lifting and stretches. They will learn how to boost heart and blood vessels with endurance to help achieve and maintain a desirable weight.

8 Components of the HFS Wellness Program Online Support Database & Help Desk Throughout the entire wellness program administrators and participants will have access to complete on-line database full of support articles, insights, recipes, menus and advice to help answer any question they might have. In addition, users can seek further guidance by submitting a support ticket to a member of our certified Food Fitness First staff. Support tickets are usually answered within hours.

9 Components of the HFS Wellness Program Ready to Go Menus & Meal Programs Ready to Go Menus: We have hundreds of preset menus to get participants started right away. Access to these menus and meals make for easy preparation of breakfast, lunch and dinner. All menus are are available through our on-line education portal. These menus are assigned based on the participants Favorable Fitness Factor number and dietary challenges. As the participants make there way through the program they are taught how to create and formate their own meals. Healthy Meal Programs: Healthy meals can be established in the cafeteria utilizing the Food Fitness First Food Spiral as the centerpiece.

10 Components of the HFS Wellness Program Food Spiral E-learning Modules In order to jump-start the program, as a prerequisite each participant will be required to watch a 6-part online training on using the Food Spiral. Each participant will be provided with a full set of interactive training modules, as well as a PDF manual. The modules will play on a desktop or any mobile device. This allows your participants to access the training from any place at any time. To gauge comprehension of the modules, participants will be required to take a post test. The results will be automatically ed to the program administrator for data collection.

11 Fully Annotated Teacher Guides We make it easy for wellness program administrators. Food Fitness First will provide all class moderators with detailed course modules, class curriculum, booklets and power point presentations for all 12 classes. We provide these materials online or as pdf downloads for easy access. As an incentive for enrollment, we recommend a reward to the employee who completes the program, increases their Favorable Fitness Factor score, or any other measure the hospital wants to use.

14 A healthy work force is a more productive work force With our 12 week wellness program, we aim to give you the resources to create a healthy workplace and culture of wellness which is paramount to improving health care costs and preventing disease in your company. Food Fitness First, Inc. will provide a perfect wellness program to improve the health of its employees as well as the community. A good wellness program are known to help companies save money on healthcare costs. An example is the Johnson and Johnson Co. Their wellness program provides a free health assessment, physical, free classes on wellness topics and onsite fitness programs. There is a 90% participation rate and the company reports a 5 million dollar cost savings in reduced healthcare costs each year. Actual cost savings for most wellness programs varies and may require several years to realize, but tangible savings like improved morale and decreased absenteeism, are almost immediate. Overall, the industry reports a $3-$6 savings for every dollar spent on wellness. Food Fitness First will provide nutrition and health information for your employees. 15 Why Choose Food Fitness First? Founded by registered dietitians alongside a team of other health care providers, the focus of FFF is to provide simple and understandable diet and health information. FFF developed new tools to quantify nutrition information and individualize it. These include the Food SPIRAL and the Favorable Fitness Factor. The Food SPIRAL is a simple way to look at the quality and quantity of each food group and the amount that is healthy to consume. The Favorable Fitness Factor is a tool that looks at the person as a whole and their current health status. It is then used as a starting point for improvement.
