1 Academic Excellence Scholarship Application Now in its fourth decade of fostering academic excellence, Cuyahoga Community College is proud to continue its annual award of the Academic Excellence Scholarship Program for recent high school graduates. Awardees have consistently remained amongst the highest academic performers at the College. In selecting students for the Academic Excellence Scholarship Program, the College has always partnered with its high school colleagues. Cuyahoga Community College offers its students a faculty focused on teaching, learning and student success. Academic and student services include outstanding technology learning centers, small classes, counseling, advising and tutoring. Students are selected for the Scholarship on the basis of excellence in scholarship, leadership, and service and character. Three major criteria are used to select awardees. Scholarship Requirements: Plan to attend Cuyahoga Community College Fall or higher grade point average (GPA) in high school, on a 4.00 point scale o Submit an official high school transcript to verify grade point average Complete scholarship essay Complete the Free Application for Federal Student Aid (FAFSA) Written letter of recommendation from high school faculty or administrator (optional) Scholarships are awarded for $1,750 per academic year. **Scholarship is renewable for one (1) academic year, based on recipient s maintaining minimum grade point average at Cuyahoga Community College of 3.0, after the first year. 2 High Tech Academy Completion Scholarship Application The purpose of the High Tech Academy Completion Scholarship is to assist High Tech Academy (HTA) students in continuing their enrollment at Cuyahoga Community College (Tri-C) to complete their Associate degree after high school graduation and potentially transferring to a four-year institution. The scholarship initiative encourages access and success for Cleveland Metropolitan School District students. Scholarship are awarded to students who plan to major in health careers pathway programs and other majors at Tri-C. With the financial support to complete an Associate degree, more students will be better positioned to achieve their academic goals and continue on their educational pathways toward the completion of four-year degrees. Selection Criteria: High Tech Academy graduate (Class of 2015) earned a minimum of 24 college credits hours, as a high school student, at Cuyahoga Community College. HTA graduate earned his/her high school diploma, while enrolled as a student in HTA. HTA graduate earned a minimum 5 college grade point average, at Cuyahoga Community College, while enrolled in HTA. HTA graduate has not yet earned an associate degree. HTA graduate has maintained good-standing status with Cuyahoga Community College. Student must be enrolled in at least twelve (12) credits per semester for the entire scholarship period (two semesters Fall 2015 and Spring 2016). Student must be continuously enrolled, focused on the completion of an associate degree, and not have earned over 93 credits (which is 150% above the number of credits required to earn an associate degree). Scholarship Award: Eligible candidates will be awarded up to the cost of tuition and books (up to $4,250) per year. The scholarship is renewable for up to one (1) additional academic year. Scholarship is awarded during the Fall 2015 and Spring 2016 semesters, funds may be available during the Summer semesters. Instructions to apply: Complete the Free Application for Federal Student Aid (FAFSA) after January 1, Complete scholarship application and essay requirements (attached). Submit a copy of the Academic Plan completed by a Counselor or faculty member. Return the completed scholarship application and all requirements stated above by March 1, 2015 to the Student Financial Aid & Scholarships Office at any campus. Applications received after this date will be considered on a funds available only basis. ** Students who have previously earned an Associate or Bachelor s degree may not be considered. 4 Honors Program Fellowship Application The Tri-C Honors Program welcomes high-achieving students committed to an exceptional learning experience. We emphasize scholarship, leadership, research, creativity, and critical thinking through active learning, including community involvement and other forms of experiential learning throughout the Honors curricula. Key benefits to enrolling in the Honors Program Fellowship include: A special emphasis on academic planning, faculty mentors, and portfolio-building to reflect student work. Collaborative planning for intent of transfer to an upper-tier university. Honors Program students have been accepted at prestigious institutions including Cornell University, Case Western Reserve University, The Ohio State University, John Carroll University, The College of the Atlantic, and the University of California Berkeley. Small class sizes to ensure specialized attention. Engagement with other Honors students through service-learning, Phi Theta Kappa membership, Tri-C Student Success Symposium participation and Honors activities. Selection Criteria: Be enrolled as a full-time student, freshman for Fall 2015 Have a 3.5 GPA or higher high school grade point average (Dual enrolled/ PSEO students must also have a 3.5 GPA at Tri-C) Meet the Honors Program placement criterion for English or Math (see chart below) ACT SAT Compass English English Honors Math Math 1510 or higher OR Scholarship Award: Eligible candidates will be awarded up to $4,250 per year to cover the full cost of Tri-C tuition, fees, and books. Instructions to apply: Complete the Free Application for Federal Student Aid (FAFSA) Complete the scholarship application and essay (attached). Return the completed scholarship application and essay to the at any campus. Applications received will be considered on a funds available only basis. A limited number of students will be accepted into the Honors Program Fellowship. *The scholarship is for two years (one year renewal), provided the student maintains minimum enrolled credits and grade point average each semester. The scholarship is valued up to $4,250 per year, and cannot be combined with other Tri-C focused academic or tuition based scholarships. Scholarship award may be used for tuition, fees, and books.
