I began my own ENT private practice in Knysna in I also manage an audiology practice, with a qualified speech therapist/audiologist who works on my behalf for the practice. To date I have 6000 patients on my books. I see on average 70 new patients a month, with about 200 private consultations a month in total. I have one dedicated full day ENT list a week at the Knysna Private Hospital. My patient spectrum is across all age groups, but with a very large retired and elderly population. The spectrum of disease is very like the NHS populations I have dealt with in the UK. State practice. I also hold an appointment as sessional Consultant ENT Surgeon to George Provincial Hospital, and Knysna Provincial Hospital, with sole responsibility for a population of about Pathology is typically Third World, with a very high incidence of complicated chronic suppurative otitis media and cholesteatoma. There is no other ENT state cover for a distance of 500 km on either side on Knysna. I see 100 patients per month, with two dedicated half-day lists per month. I do whatever I can, but have a close working relationship with the Department of ENT at Groote Schuur Hospital in Cape Town, where I refer cases I cannot manage locally. This more often than not is due to inadequate facilities for postoperative care in the local Provincial Hospital. I am on permanent call for the region, but luckily medical officers deal with most of the load. I am involved in a skills training program to enable medical officers to deal with more and more ENT emergencies. My case load varies from list to list, but all lists are overbooked, with a three month waiting list. A typical half day list would include two T s and A s, two grommets, a septoplasty, and a tympanomastoidectomy. The majority of otology cases I do are on this list. Interests. I am a generalist ENT surgeon, but my main interests are in adult nasal surgery and otology. I have some experience in Head and Neck surgery, but the exposure in private practice is not great.

3 I spent three weeks in 2003 locuming in England, and see this as an ongoing prospect to broaden my experience while keeping my private practice in Knysna going. My most recent locum was as a consultant for 6 weeks at Birmingham Heartlands Hospital, in January/February Training. I attend our annual Congress every year. I am part of an ENT Journal Club that meets monthly. I am involved in CPD talks and lectures for our local medical community on a regular basis. Innovations. In 2003 I initiated and arranged a Rotary-sponsored ENT week in Knysna, using visiting ENT consultants and registrars from Groote Schuur Hospital to perform surgery in dedicated theatres in the Knysna Provincial Hospital. 60 cases in all were performed, work that would have taken me several months to complete. Private medical suppliers provided microscopes, ENT drills and new technologies for trial during the week. The same program continues every year, becoming an annual event, and attendance at which is highly sought after by Groote Schuur junior staff. This program is looked upon as a model example of public and private medical cooperation, in a national environment where this ideal is spoken about an nauseum but seldom realised. In 2007 I was awarded Rotary s Community Service Award for this innovation. In the same year, Knysna Rotary won Best Community Project in the Southern African region for this program. Carespace.net. I am a founder and Managing Director of Carespace.net, ( a medical software innovations company aimed at enhancing medical practice. 4 than adult practice at Groote Schuur Hospital included blocks at the Red Cross War Memorial Children's Hospital, and sessions of student teaching. Pathologies encountered were across the board, with a high exposure to head and neck malignancy. The spectrum is typically third world in severity and extent. Trainees rapidly became familiar with ENT disorders in general, and the surgical procedures required. Training courses in FESS and temporal bone surgery were held annually, which I regularly attended.
