1 1. Introduction Introduction Thank you for participating in the Strategic Engineering Risk Assessment (SERA) project for the Department of Education and Early Childhood Development. The project is being implemented in partnership with the Victorian Managed Insurance Authority(VMIA). The web survey is used to gather high level risk information related to physical site based risks, specifically bushfire and fire. The survey is broken into three modules with each section expected to take about minutes to complete. Each module is divided into separate pages so that like questions are grouped together. You can print out the survey and your responses via the print function on your web browser. The easiest way to do this is to "print as you go" rather than complete the survey and have to navigate back through each page. Important: The online survey software allows you to fill out the survey in stages if you wish to come back to it later or research any particular response. Please ensure to finish the page you are on and click the "Next" button at the bottom of the page before exiting. This will ensure that your responses are saved. You can re-enter the survey using the link on the original . Many of the questions require responses against a relative scale (e.g. 1-5). Explanatory text is provided for some of questions but the majority should be self explanatory. It is important that you read each question carefully before selecting the most appropriate response. The survey includes the ability to provide additional narrative comments to support your selected responses. 2 Survey Structure The survey consists of approximately 78 questions in total, broadly divided into separate risk modules as described below. Each module has a series of sub pages divided into groups of similar questions. Site and Emergency Management: The Site and Emergency Management module is used to collect general information about the site in terms of location, size as well as information around whole of risk issues such as emergency management. Bushfire: Using elements of the 2009 DEECD bushfire self assessment, this module collects site information relevant to the bushfire exposure with a strong emphasis on managing change and emergency preparedness and response. Fire: This module looks at the site's vulnerability to an internal fire and assesses the risk in terms of emergency warning and evacuation, fire protection and detection, fire and smoke separation, and related management systems. Note: The survey is designed to collect risk information related to a single physical, continuous location. Schools/centres with multiple campuses, where the campuses are sufficiently physically separated such that they operate different emergency management plans, will need to complete a survey for each campus. A school campus that is split by a public roadway, for example, is still considered a single location. In the majority of cases separate invitations will have been sent to each campus via a separate nominated contact. If you are unsure whether all of your campuses have been invited to complete the survey then contact VMIA via nominating the campus and address and we will review and respond back accordingly. 3. Answering Questions Page 2
3 Answering Questions The majority of questions use a rating scale to measure the level of performance and confidence of the respondent. The rating scale is structured as follows: Strongly agree - indicates the highest level of performance and confidence. You should select this response where you can confidently state that you are meeting every aspect of the question across all relevant areas of the site. If your response to the question is a definitive "Yes" then select this rating option. Somewhat agree - indicates a reasonable level of performance and confidence. You should select this response when you have reasonable, but not total confidence that you are meeting all aspects of the question across the site. agree nor disagree - indicates a neutral level of performance and confidence. Respondents would typically select this response where there is still some improvement in either function or area covered (depending on the nature of the question) to get the highest level. Somewhat disagree - indicates a relatively low level of performance or confidence. Respondents would typically select this response where there was substantial improvement required to get to the highest level. Strongly disagree - indicates the lowest possible level of performance or confidence. Respondents would typically select this response in the absence of a particular procedure/practice or physical asset. If your response to the question is a definitive "No" then select this rating option. Don't know - Respondents should select this option when the information is is not available and any response would be a guess at best 4.
